Cyndi, her death rubber Omniflex and Chi: As I mentioned in my entry for December 18, 2011 post entitled: ‘Breeding diaphragms and ‘death rubber’ Cyndi has gotten her first death rubber device, a Milex Omniflex silicone diaphragm, which she pulled from her mentor’s still warm body after she died during a surface SCUBA encounter at Splash where she was senior Towel Girl. It first appeared that she was asphyxiated due to a tank mix-up, but that didn’t seem credible because of her years of experience. An autopsy determined that Inga died from a ruptured aneurysm in the brain which no one knew she had and which the pathologist thinks was triggered by her increased blood pressure during orgasm when her air ran out. Cyndi who was being trained by her was initially devastated, but in the following weeks with her ritual ‘purification’ at Salisbury Cathedral on the recent Winter Solstice she seems to have fully recovered.

I’ve noticed that when Cyndi has Inga’s Omniflex inserted she is confident, her voice sounds more self assured and she is more willing to take greater risks comparable to what Anya and I routinely take. However, the most fascinating thing is that she has become far better in reading the ‘tells’ that men give off and able to anticipate what they both want and need. That is the same thing that happened to me when I first wore a rubber encasement suit in which a friend I had at school suffocated during a sexual encounter. I found I could concentrate and think things through far quicker and easier when I was wearing that encasement suit because, I realized latter, I was channeling the former owner’s mental energy. Somehow becoming sensitive to the aura of the previous owner’s Chi.

Now I have become even more sensitive in that regard and can channel a former owner’s energy by wearing something as small as a dead woman’s cervical barrier if she died while wearing it during a sexual encounter when her pleasure and positive life force were aligned. So both Cyndi and I have become sensitives able to channel certain dead women’s energy that still inhabits intimate items associated with their deaths.

Rubber Lust: I’m seeing the same thing that happened to me happen to Cyndi when she is wearing Inga’s Omniflex. I call it ‘Rubber Lust’ because I become far more sexually assertive when I have a cervical barrier inserted and that’s true in spades if I’m wearing one of my death rubber devices and if I’m fertile or menstrual (as I am now, I’m CD12 and should ovulate this coming Thursday) I’m sexually insatiable. Of course her Omniflex is a shallow water barrier, only effective as a dive-sex sports-shield at depths above 30 feet.

Cyndi already has a new 70mm latex flat spring that she was fitted for, but what she needs is a 70 mm Death Rubber latex flat spring device that will effectively protect her from having water or air bubbles forced into her uterus at depths below 30 feet so I’m on the lookout for fatalities among UNLV students, escorts, escort trainees, towel-girls, women guests at The Lorelei and submissives who are sent to Adolph for training who wear latex diaphragms and enjoy risky sex. A 70mm is a common size and as the health risks of hormones are becoming more widely publicized cervical barriers are becoming increasingly popular for women in the fields in which I specialize and especially with women rubberists.

Women at risk: I’ve been asked by readers of our local obituaries and crime news in Nevada why more of the deaths I write about don’t make the news. Some of course do, but for the most part the women I write about were engaging in something extremely dangerous or illegal or both often involving breathplay during penetrative sex while wearing a gasmask and rebreather bag or SCUBA gear and underwater. For a woman participating in that level of sport as an amateur there is a steep learning curve and w/o proper training a substantial percentage drown, are asphyxiated or end up with permanent brain damage.

And occasionally an entrepreneur is found at the bottom of a pool behind a foreclosed house or hanging by a vaginal hook from the balcony as a warning to others if she was caught poaching on the Organization’s territory. And a few are put through a wood chipper and the bits sprayed into a pond of quick lime.

Guests at The Lorelei are cautioned about how strenuous breathplay during penetrative sex (especially dive-sex) can be and even though they must pass a stress test to ensure they have the stamina to fully participate the warning doesn’t seem to put anyone off. There have been two fatalities, one before the test was required and one afterward. Both were German women, who refused to admit they were not mentally up to the challenge of having their air shut off during orgasm under 15 feet of water and panicked. Adolph believes both of them were frightened to death when their air was shut off during orgasm even though they knew it was going to happen. So while it seems unlikely at first that I will be able to find a 70mm latex flat spring death rubber for Cyndi, or that she can find her own, the chances are considerably greater than one might expect.

Flat spring sports-shields/gas-guards, their strengths and weaknesses: The flat spring rim sports-shields that almost all professionals use has its own set of pros and cons. I think the strengths of flat spring device is that they are made of natural rubber latex which has nearly the heat transfer characteristics of skin-on-skin contact and the dome is much stretchier than silicone, the scent of a latex barrier left to marinate in vaginal fluids for more than 24 hours is wonderfully strong and pungent and a properly fitted and correctly inserted FS rim device is nearly impossible for a partner to under-thrust or displace. And it can be safely and effectively worn on dives to any depth.

The drawbacks of latex diaphragms are that they can’t be used by women or couples who have latex allergies. The flat spring rim is best used by women with strong muscle tone so it isn’t recommended for women who have delivered vaginally. A latex device left inserted for more than 24 hours has a much stronger and (some say) unpleasant scent than a silicone device left inserted for the same interval and latex can be relatively easily and quickly destroyed by greasy or oily medicines or lubricants. Even when well cared for the dome of a latex device will eventually deteriorate, harden and crack from old age as readers can see from the photo of one of my latex All-Flex diaphragms that is more than 15 y/o that accompanies this entry. In my experience a well cared for silicone diaphragm will typically last two to three times as long as a latex one. However, a latex diaphragm is usually about half the price of a silicone one.

Death Rubber: It is also the first instance of a trainee wearing a professional quality sports-shield (gas-guard), the flat spring latex Reflections diaphragm although it’s not used. It just gives the trainee an opportunity to wear her FS diaphragm to get used to it and see that (when properly fitted and correctly inserted) it’s so comfortable she forgets she is wearing it. We have found that calling a cervical barrier a sports-shield is seen by students as a more user-friendly term than gas-guard. So far there haven’t been many trainees who have latex allergies and the ones who do usually drop out of the course since so much of a professional escort’s work can involve wearing some amount of rubber fetishwear. I keep a close eye on the trainees who wear the same size cervical barriers I do so that If any should have a fatal accident while training (Rare, but it does happen, especially during advanced training.) I think Adolph, Gepetto, yourself and the St. Lucy's administration are in the right to limit or block cell phone use in intimate areas. You wouldn't want to have your clients in compromising positions, would you? In fact, I think there is a new commercial series for the LVCVB talking about people who tweet or Facebook their experiences over there in Vegas. Training venues and personal electronics: In the training areas (gyms, pools and encounter sallies) at St Lucy’s and our escort center as well as Adolph’s facilities (Splash, and The Lorelei) no personal electronics are allowed for security and privacy reasons. Birth control pills are 99% safe: Doctors

“JEDDAH – Birth control pills are safe and do not cause medical complications such as blood clots and infertility, according to doctors who attended a medical seminar here recently.

Dr. Hisham Arab, secretary general of the Saudi Obstetrics and Gynecology Society (SOGS), said that studies conducted over the last six months by the Saudi Food and Drug Authority (SFDA) showed that there is no relationship between the pills and blood clots. He added that other family planning pills on the Saudi market are also safe.

Dr. Abdul Hafiz Khoja, a family medicine consultant, said rumors about the safety of certain products were the result of a “dirty trade war” between rival pharmaceutical companies.

Khoja said that SOGS had recently issued a medical bulletin to all obstetricians and gynecologists in the Kingdom confirming the integrity of such pills and outlining supporting scientific evidence.

“All the latest experiments and research conducted by many research centers and scientific bodies around the world have revealed and confirmed that family planning pills are 99.99 percent safe. These medicines are also known to prevent women from contracting some diseases especially cancer.”

Khoja said that research from the American Food and Drug Administration (FDA) showed that these pills are safer than other forms of contraception such as intrauterine devices (IUD) and injections. The FDA also rejected rumors that using such pills for long periods may cause infertility.

Dr. E’etedal Idrees, a faculty member at Dar Al-Hekma College, said the medical rumors are dangerous. “We should verify the news we receive from unknown sources especially from the Internet.”

He said that he tested birth controls pills such as Jasmine and found it to be safe. “I also talked to the SFDA here and asked them if they ordered the withdrawal of such pills from the Saudi market. Their response was negative.” ”

Personal Comment: This was written after the publicity in mid December when an FDA review board recommended stronger warnings on oral contraceptives containing the progestin drospirenone and on Ortho Evra, the Patch. It puts a different spin on the fact that an estimated 10 in 10,000 women will have a serious adverse event while taking pills containing Drospirenone. You have to love the male dominated society’s arrogant certainty in the opening sentence, even though it’s stretching the truth. There is a small (but absolute) increase in blood clots associated with all combined (estrogen/progestin) contraceptives and the risk with some progestins, like drospirenone, is greater than with others.

Edited on January 15th to reply to comment: First, Google/Blogger has (again) inserted bugs into the system while trying to introduce the Two Level comments feature, which in their wisdom they’ve made mandatory! Apparently the coders don't have the time to thoroughly test such a complicated feature before deploying it... Sigh! Brenda, the Nigg Stone and a sea change

A New Follower: I have a new blog follower, ‘Brenda’; who says she is a Pictish Princess, possibly a cousin many times removed, and who has left me a fascinating comment – not yet posted - on my blog entry: The Winter Solstice and other Holidays [January 09, 2012]. Since it concerns bringing new genes into the Ancient’s bloodline as I am being encouraged to do I thought I would mention her approach while my sponsors within the Priesthood examine her background as there aren’t many women Priestesses or Pictish Princesses especially in north eastern Scotland.

Unlike Sister Brenda I am (so I have been told) a Druid Priestess a much older title. Perhaps that translates to the equivalent of a Pictish Princess, but I haven’t been told that, at least not yet. I think The Nigg Stone (at Nigg Old Church on Cromarty Firth) where Brenda says her most recent fertility ritual was held, is one of the earliest and best examples of Pictish stone carvings and is thought to date to the 8th century AD, much more recent than Stonehenge, but still a step on the path to the present for my British ancestors. I was amazed to find her description of the Pictish ceremony coincided in considerable detail with my ceremonies for St Lucy’s students to welcome them into womanhood when they reach menarche.

My purification rituals not differing appreciably from the ‘purification’ process by which Brenda relates Pictish novitiates are cleansed - by combining menstrual blood, the symbol of renewed fertility, and male genital fluid in young females as an offering to the Gods of fertility - suggest to me that I’ve been channeling ancient Druid ceremonies w/o realizing it. As returning readers will remember, Jack is working on his Doctorate in Archeology at Oxford. So when he and I visited Aberdeen last summer he had one of the Duchy’s helicopters take us to see significant ancient sites on Scotland’s North East coast and Nigg Old Church was one location. Not because the church is significant, but because it houses the Nigg Stone which is pictured at the top of this entry. In her comment Brenda wrote:

“The men, save one, then retired to the church for ceremonies around the Nigg Stone, praying, circling and masturbating one another. We women remained outside…”

It can be seen from the photo that it’s not possible for anyone to circle the Nigg Stone in its present setting so perhaps they just stood around it as best they could. I suppose they used natural lamb condoms since women weren’t allowed in to clean up the splatters. Since Brenda wasn’t allowed inside the church to see the male portion of the ceremony perhaps she misunderstood about circling the Nigg Stone. Until recently I never understood the spilling of so much male seed at fertility rites rather than depositing it in every sacred vessel available either by willing desire on the part of the woman or by force since there were so few breeding females. That was the case in ancient times since the death rate was so high for women during childbirth so it was important to keep as many of us as possible pregnant to advance the bloodline. Today however, the criteria have changed since from what I understand there are so few women with suitable DNA, which would leave the lesser men with nothing to do but masturbate rather than produce children with weaker genes.

A change of attitude: I was surprised to receive a letter yesterday from the Duke, Jack’s father. The old bull has recovered from the heart attack he had while fucking me this past summer. Now it seems there has been a sea change in his attitude toward me and he wrote to tell me how pleased he is that Jack and I are together again! WTF!

A woman before the dive on which she miscarried

Others misfortune and me: Having recently found that I’m descended from ancient British roots on both sides of my family I think that my genetic heritage may explain why I have the cravings for thrills and risk taking I do. For more about that see my January 9, 2012 post: The Winter Solstice and other Holidays.

Some time ago a good friend and faithful reader asked why I crave taking risks for thrills and write about violence, death and tragedy. It’s taken me a while to think about that and until recently I wasn’t sure I had an answer. My therapist (unaware of my family history until recently) thought I had a death wish because I seem to gravitate to the most extreme sexual encounters involving breathplay while wearing a gasmask and rebreather bag or during dive-sex or some other bizarre sexual encounter to get my thrills and he’s still not entirely convinced otherwise.

By wearing pointe shoes as standard footwear (a fairly harmless kink) while at work as my ballet companies AD and choreographer or in the boardroom of the company I own and even while shopping I’m not only able to enjoy my pointe-shoe fetish in public, but have developed a public persona that projects my artistic ability. While within the business community my rivals who thought the ‘cunt in toe-shoes’ would be a pushover discovered how wrong they were when I handed them their asses in one business deal after another when the real-estate and casino bubble burst here four years ago.

My shrink still thinks my fetish for wearing ‘death rubber’ (which I’ve managed to collect from the bodies of women who died during sexual encounters) reflects my need to make up for the loss of my mother when I was a child. He sees my belief that the dead owner’s Chi (which still inhabits such an intimate rubber device) provides me with her spiritual protection when I insert it in my vagina, as a psychological coping device which calms my fears and gives me confidence in my ability to get through dangerous situations safely. I think there is far more to it than that since wearing death rubber seemingly has protected me from serious injury in difficult situations since I began wearing it about five years ago. I’ve been tested by endocrinologists for chemical imbalances and every test has been within ‘normal’ limits so I’m thinking my craving for danger is hereditary and I’m sensitive to the ancient beliefs that every item has certain powers that can be used by sensitives to advance their own devices and desires.

In Vegas I seem to have been passed over for the contract to tidy up after most of the Organization’s periodic efforts to discourage others from encroaching on the profitable portions of their diversified businesses. Or, perhaps they are just using more stealth and guile rather than blunt force. That’s not as much of a loss as it might first appear as I have become so busy with other projects that Organizational clean-up was becoming a problem for me. But I’m still being called when the cleanup involves women so my access to a primary source of death rubber is still in tact which I’m relieved to know.

The dangers of diving while pregnant: The men with us in the Caribbean were Chris (with Cyndi), Chuck, who is our Gyn (with Anya) and Jack (Lord John) with me. I try never to travel w/o a Gyn since we are all so sexually active especially since Chuck saved Cyndi’s life. It was nearly three years ago when Cyndi had her miscarriage while diving the cove with Chris who was then her bodyguard and lover, when she was almost 15. Long time readers may remember my writing about it at the time in my blog entry on Yahoo 360 for January 12, 2009. She was on the pill and hadn’t realized she was pregnant and while in the pit she began to cramp and hemorrhage as she miscarried. Fortunately Chris got her to the surface as we were entering the cove and Chuck was able to stop the bleeding and administer antibiotics until she could be seen by her family’s doctor who was flown in by RAF fighter jet from the UK.

This time we had The Wanton Lass IV, His Graces sailboat, that’s docked at Spanish Town meet us at Charlotte Amalie on St Thomas where we landed ‘Limnaea’, my G550, at Cyril E. King (STT) with its 7,000 ft runway, and take us around to the north side of Virgin Gorda where we anchored it in the cove as a diving platform. The next morning (after ballet class for the women) we took the Lass’s zodiac out to the yacht disappointed that another boat had come in to ‘my’ cove after dark and anchored for the night. The young couple was already in dive gear as we arrived and after a brief hello they began diving the cove as we boarded the Wanton Lass. Most of the cove bottom is sand at a depth of 60 feet after a steep drop-off from the beach except for the south western end which drops off to 100 -120 feet and has the aquifer emptying into the pit which reduces the salinity of the water there and the current has cut a deep channel that leads out of the cove to the edge of the shelf and empties into much deeper water.

After we put on our dive gear we entered the water and as we began our descent we could see the woman was in trouble and her partner was trying to get her out of the pit, but they were caught in the current. Neither of them was wearing a buoyancy compensator and were over weighted for the fresh water they found themselves in. By the time Chuck, Chris and I got to them the man had dropped his weight belt and was struggling to keep the reg in the woman’s mouth while trying to unfasten her weight belt as they were being swept into the channel and out to sea. Chris helped the man while Chuck, Jack and I grabbed the woman and we all inflated out vests to rise out of the channel so we could swim out of the current. Once we were out of the current we ascended to the surface and had the zodiac come and pick us up. We took the couple aboard the Lass and stripped off the woman’s wetsuit. She had water in her lungs but managed to get most of it out while cramping severely and passing fetal tissue.

My trip to see Taryn in the UK: Again things didn’t go the way I had planned. The Winter Solstice celebration at Stonehenge (on 12/22) went well with the temperature in the mid 50s F. It Was Cyndi’s first at the ancient site and in celebration we had a private ceremony for her in the undercroft of Salisbury Cathedral just a few miles down the road before we went on to Salisbury plain to the stone circles. Even though Cyndi has no Druid blood a Druid priest – as a favor to me – was willing to ‘purify’ her for the sunrise ceremony. The purification consisted primarily of her being filled with his seed after being entered from behind, and I was involved since I presented her to the priest for purification while wearing the sacred Druid Priestess’s circlet of beaten gold lent me for the occasion. The Priest placed the circlet on my head after my hair was let down and brushed out and even though the circlet weighs about 12 troy ounces it fit perfectly and was fairly comfortable. I am thankful that I don’t have to worry about security for the Druid regalia. Such parts of it that remain are cared for by the Druid Elders. Cyndi was in no danger of becoming pregnant by the priest even though she was CD7 and fertile because she has a GyneFix IUD implanted.

For those readers wondering about my being a Druid Priestess – though a relatively minor one - the position was a surprise to me. I didn’t seek it (it’s hereditary) it was thrust upon me and I only found out about it in the last few months. It seems my natural father, a Welsh miner who immigrated to the coal fields of Western Virginia and who became very rich from investing in coal bearing property, was from a community of nearly pure Druid stock that had migrated into the mountains of Wales after leaving Stonehenge. When Uncle John (as he was known to me) impregnated my mother he did not know, nor did anyone else, that she had a high percentage of Druid blood in her family as well so that I have one of the highest percentages of Druid genes of any woman alive today, or so I’ve been told by geneticists who have studied the ancient people and who helped locate me.

After the Spring Equinox I was sought out by the Druid Elders and it was explained that the old ways are dying out and my participation was required to sustain the bloodline such as it is and the Elders have been encouraging me to breed with a suitable mate, which I’m reluctant to do. Its not that I mind the act of breeding I just don’t want to have a baby. I think my Druid genes may account for some of my ability to foretell some things and make use of dead women’s Chi and perhaps may also account for my fondness for risk taking. Learning about my family genealogy in the last few months has helped me understand why I’m the way I am more than the five years of therapy I’ve had, though I think the therapist prepared me for the role I might play in preserving the Druid bloodline. I’m not entirely sure what is expected of me as a junior member of the Druid aristocracy, but the Chief Elder said I would be eased into my ceremonial role depending on the availability of other priestesses. I think they see me primarily as a breeding stock which if that’s the case may be a problem for me.

After Stonehenge: we went to Taryn's home to the north east of Cambridge Christmas and it was marvelous! One of the stonemasons working on the renovations (before she moved into the Manor) called it ‘Cunt Castle’ and the name seems to have stuck with her circle of friends, though not with the locals, or at least not to her face. While we were there Cambridge was all abuzz with Prince Phillip in residence at Papworth hospital for four days having cardiac stents placed and with HM and other Royals coming and going to visit him. We all wish HRH a full and speedy recovery and he seems to be doing well and is back at Sandringham with The Queen as this is written. Of course with her husband's illness and now the body of a 17 y/o Latvian girl being found within three miles of Sandringham House Her Majesty isn’t getting the restful winter escape at her Norfolk estate she had hoped for.

With the tube strike and shopping crowds at after-Christmas sales we stayed away from London, so Taryn, Cyndi, Anya and I went to Paris for two days and saw the Repetto ballet shop decorated for Christmas with some of the most beautiful tutus! We returned to find that my male partner Jon (one of my younger stallions with seemingly inexhaustible stamina) had a ruptured appendix and had had an emergency appendectomy so he was out as a sexual consort. Fortunately My Summer Lover Jack - John, Marquis of Marle - heard I was at Taryn’s and came calling. I thought he had escaped me, but found he had become available again as Sylvie, the aristocrat that his family had picked for him to marry, hadn’t lasted in his affection nor had her stamina to accompany him diving or in bed. So when I say we were happy to see each other again it’s something of an understatement! I was delighted that Jack accepted my request that he accompany us to my place on Virgin Gorda for a few days of sun and diving.

Breeding diaphragms: Since there has been an increase in the number of men asking for breeding encounters with our escorts I’ve started training the girls to use a ‘breeding diaphragm’. For rape fantasies I take a different approach when providing contraceptive protection that ‘accidentally’ fails. Someone unfamiliar with cervical barriers might ask why not use a cervical cap, such as FemCap for breeding? The Ringlets: A group of UNLV women graduate students in their mid 20s calling themselves the Ringlets because they all use NuvaRing have recently become interested in dive-sex. Fitting the Ringlets: In addition to needing a gas-guard for upper reproductive tract protection - in case the vagina floods during submerged penetrative sex – a NuvaRing user needs something to prevent her ring from being hooked and pulled out by a partner’s penis as he withdraws after ejaculation while she is in the water. Having a ring pulled out is not uncommon especially if the man is well endowed and it is easily found in the bed clothes or in the car after vanilla sex. However having a ring pulled out while the woman is swimming is a different matter entirely. Submerged couples minds are often on air consumption and other things after the ecstasy of an underwater orgasm so a missing ring is easy to overlook and if noticed at all difficult to find afterward on the bottom of a pool and impossible in open water. In addition I recommended that the Ringlets each take a spare ring and got them all two 12 applicator boxes of 10 ml DiveGel prefilled disposable applicators as well as a box each of FC2 condoms. Is Your Birth Control as Safe as You Think?

Since NuvaRing hit the market, more than 700 women have filed lawsuits, claiming it has caused potentially life-threatening blood clots. The battle may change the way millions of women prevent pregnancy.

Christen Childs woke up on September 12, 2009, in the pitch dark of early morning with what she thought was a pulled muscle in her leg. She reached down to massage the cramp, trying to fathom how her left calf could be so achingly sore when she hadn't made it to the gym in weeks. This was a Saturday — by Monday, her leg was swollen and hot, and when she tried to stand, jolts of pain shot up to her spine. She consulted her brother-in-law, a doctor, and he told her to go to the ER immediately. He suspected what an ultrasound would confirm: Childs, 26, had a blood clot in her leg — left untreated, it could kill her. "When the doctor diagnosed me, I started crying," she says. "I was stunned by how close I had come to dying."

Death would come closer still. Later that day, the clot broke apart and traveled to her lungs. Straining to breathe, she remained bedridden in the intensive-care unit as blood thinners were injected into her stomach four times a day for six days. By the following weekend, she was released, instructed to return weekly for hematology checks to make sure new clots hadn't formed. Meanwhile, her doctors ran a battery of tests to figure out what exactly went wrong. After all, Childs didn't have any of the risk factors associated with blood clots: She didn't smoke, wasn't overweight, and had no family history of clots. After six months on blood thinners, Childs was finally back to her old self, with one exception: Her doctors wouldn't let her go back on NuvaRing, her birth control, which they determined was the likely culprit behind her near-fatal illness.

NuvaRing is one of the world's most popular forms of non-pill birth control, and recent data indicates that more than 5.5 million prescriptions were written in the U.S. in 2010, according to IMS Health, a health-care information company. But in recent years, serious questions have arisen concerning its safety. To date, almost 1,000 cases of possible NuvaRing-related blood clots have been reported to the FDA, and more than 700 women in the U.S. are currently suing Merck, NuvaRing's manufacturer, for downplaying its health risks. Most of those lawsuits have been consolidated into a massive, multidistrict federal case that's expected to go to trial in Missouri next year. The NuvaRing case is being closely watched by physicians and the pharm industry. If the litigants persuade a jury that Merck undersold the device's risk for blood clots — a big if, to be sure — it could have far-reaching consequences for several contraceptives on the market.

• When it was approved by the FDA in 2001, NuvaRing became the world's first vaginal birth-control ring. Inserted just once a month, it dispenses hormones directly into the bloodstream. Unlike oral contraceptives, which require taking a pill the same time every day, NuvaRing is marketed as a pill-free, no-muss/no-fuss alternative. Just pop it in and forget about it (the ring remains in for three weeks and comes out for a week during your period). That pitch clearly resonated with women. Last year, NuvaRing generated $559 million in international sales for Merck.

It's worth noting that all hormonal birth control with added estrogen carries a risk of blood clots. In the 1960s, the estrogen used in the earliest pills raised the risk of clots more than nine fold. A decade later, a second generation of pills containing a much gentler dose of the hormone, coupled with the synthetic hormone progestin, had been developed. Though these second-generation pills are effective and safe, drugmakers have sought new forms to market as better able to control conditions like facial hair and acne. NuvaRing contains a version of the third-generation progestin desogestrel. There's even a fourth-generation progestin, said to alleviate symptoms of PMS; it's the essential component of Yaz and Yasmin, two top-selling oral contraceptives.

The problem? Some researchers say third- and fourth-generation contraceptives — including those containing desogestrel — raise the risk of blood clots without adding any benefit. In 2007, the consumer watchdog group Public Citizen petitioned the FDA to ban oral contraceptives containing desogestrel. Although NuvaRing wasn't named in that petition because it was still fairly new, the director of Public Citizen's Health Resource Group, Dr. Sidney Wolfe, reached an unequivocal conclusion: "We've told people not to use these drugs and have advised women that the safest contraceptives are the older, second-generation ones." The FDA didn't act on Public Citizen's claims, even though during its own review process, the agency concluded that NuvaRing's label should "clearly reflect safety concerns about an increased risk" for blood clots.

In 2009, the British Medical Journal published two investigations of desogestrel and fourth-generation progestins that seemed to bolster critics' concerns. Both studies found that women taking third- and fourth-generation pills were almost twice as likely to get a blood clot than those who took second-generation contraceptives. Frits Rosendaal, Ph.D., an epidemiologist at Leiden University Medical Center in the Netherlands who has contributed to more than a dozen scientific papers on desogestrel, coauthored one of the studies that found a higher risk for clots in NuvaRing than in second-generation birth control. "To me, it's incredible that [Merck] used desogestrel and not a second-generation progestin. Why not go for the one with the lowest risk?" says Rosendaal.

But Merck and other scientists dispute claims that NuvaRing poses a higher risk than other contraceptives. In NuvaRing's product information pamphlet, Merck acknowledges the findings, but adds, "Data from additional studies have not shown this twofold increase in risk. It is unknown if NuvaRing has a different risk of [blood clots] than second-generation oral contraceptives." That position infuriates NuvaRing's critics. "Nine years after the product was put on the market, they still say the risk is unknown. To say, 'We just don't know' is not sufficient," says Thomas Lamb, a North Carolina — based attorney whose firm represents more than 50 women around the country who have filed suit against NuvaRing, Yaz, and other third- and fourth-generation contraceptives.

Among those wanting answers is Robert Bozicev, 37, of Tom's River, New Jersey. His 32-year-old wife, Jackie, had begun using NuvaRing shortly after the birth of their daughter in 2007. One Friday morning, as he stood in the kitchen making breakfast for their two kids, he heard her puttering about in their second-floor bathroom. As she was about to step into the shower, she called his name. He answered her, preoccupied with their son, 2, who was demanding another waffle. After she cried for him a second time, barely able to get out his name, he dashed upstairs and found her on the floor, unable to breathe. "She was in a lot of pain, rolling around, trying to get onto her stomach," he says. "My son was there, watching the whole thing." To his horror, Jackie started turning blue.

When an ambulance arrived, emergency techs tried to restart her breathing. Bozicev was gripped with terror. Doctors continued to try to revive her in the hospital but eventually had to give up. She died that day. The cause of death: massive pulmonary thromboemboli due to deep vein thrombosis. In other words, she died from a blood clot. Anxious to learn more about what had happened, Bozicev searched online and came upon reports linking NuvaRing to an increased risk of clots. Knowing Jackie had been healthy before she started using the device (she wasn't overweight and didn't smoke), Bozicev concluded that NuvaRing was to blame, and filed a wrongful-death suit against Merck in March 2008. "Even to this day — and this is three and a half years later — I still can't believe it happened," he says. Jackie's clothes remain in her closet. He is too heartbroken to take them out.

NuvaRing isn't the only pill-free contraceptive beset by litigation. When Ortho Evra hit pharmacy shelves in 2002, it was the first birth-control patch of its kind. Its selling point: Simply slap a Band-Aid-like adhesive on your arm, stomach, or back each week. Within two years, sales had swelled to nearly $400 million. But soon after, studies emerged indicating that the blood concentrations of estrogen in Ortho Evra users might be much higher than previously thought, resulting in a greater risk of clots. That's when the lawsuits piled up. In November 2005, Ortho Evra, under an agreement with the FDA, added a black-box warning to its packages stating that patch users are exposed to roughly 60 percent more estrogen than the typical pill user, resulting in a potential "approximate doubling of risk of serious blood clots." The warning also stated that the risk might not increase at all, but it was still enough to scare off doctors. By year-end 2010, Ortho Evra's sales had fallen 69 percent to $124 million.

Merck, hoping to avoid a similar fallout, is standing by its product. "The decision was made to use this particular form of progestin [in Nuva Ring] because of the long history of safety and efficacy," says Dr. Michelle Vichnin, one of Merck's medical directors. When contacted by Marie Claire regarding the pending lawsuits, a Merck spokesperson stated: "The company intends to defend itself against these lawsuits." Possibly helping their case: In March 2010, researchers from the Cochrane Collaboration, a respected international organization that reviews published scientific data, analyzed 10 randomized studies of NuvaRing involving 3,000 women. They found that only two women in the study group developed clots, and one was later found to have a genetic risk.

Often lost in the discussion is just how unlikely the average healthy woman's chances of getting a blood clot are, whether she takes a second- or third-generation birth control. The risk of blood clots for women on a desogestrel-based contraceptive is about 30 out of 100,000, according to some studies — still less than half a percent. You're more likely to get struck by lightning over the course of your lifetime. And that risk is still several fold less than that of a woman who is pregnant or postpartum. Another issue undermining the claims against Merck is the scope of prior studies, which didn't look at a large enough pool of NuvaRing users, according to Dr. Andrew Kaunitz, a professor at the Department of Obstetrics & Gynecology at the University of Florida in Jacksonville. (Kaunitz consults for several pharmaceutical companies, including Merck.) To investigate the effect of something so marginal, he says, scientists need to conduct much bigger studies on women who do not have any known risk factors for clots. "When you're looking at rare events, even small biases can throw off your findings," says Kaunitz.

Larger studies are on the way. Merck is currently underwriting a massive trial of some 30,000 NuvaRing and pill users — the largest, most comprehensive study of the contraceptive to date.

The company says it expects to publicize results next year. "The study was started because we wanted to answer questions that were being discussed in the medical community," says Merck's Vichnin. Meanwhile, all eyes are on the impending litigation against fourth-generation pills like Yaz and Yasmin, also under fire for their potential blood clot risks and set to go to trial in September.

So what should NuvaRing users do in the meantime? Don't be afraid to ask your doctor questions about which birth control is right for you, given your medical history and lifestyle. There are close to 100 oral and pill-free contraceptives on the market today, yet many women take whatever their gynecologist recommends, no questions asked. Says epidemiologist Rosendaal, "It's not like there aren't options."

Clot Control

A healthy woman's risk of a blood clot increases dramatically if she takes birth control with estrogen. Here's what you need to know to diagnose and prevent a clot:

• What is a blood clot? A tiny buildup of platelets and blood plasma proteins caused by decreased blood flow to the lower extremities. Pregnancy, obesity, or being squashed like a sardine on a flight to Hong Kong all allow blood to pool, which can lead to a clot. On top of that, estrogen from oral contraceptives increases the levels of four of the blood's 12 clotting factors, making clots more likely, says Barbara Dehn, a nurse practitioner with the Women Physicians OB/GYN Medical Group in Mountain View, California.

• Telltale symptoms: Cramplike leg pain and red, swollen, or warm skin. Chest pain, shortness of breath, and heart palpitations may signal a pulmonary embolism (meaning a piece of the clot has broken off and traveled to the lungs). If you have any of these symptoms, get to the emergency room. Doctors will give you blood-thinning drugs; in an extreme emergency, you'll get an injection of a "clot-busting" drug like TPA (tissue plasminogen activator).

• Preventive measures: Drinking 8 ounces of water every two hours when on a plane will make a clot less likely. And do five minutes of toe raises or calf flexes every hour while traveling, says Dr. Mark Melrose, an emergency room physician and cofounder of Urgent Care Manhattan, a walk-in medical practice in New York. If you're pregnant or have had a clot before, wear support stockings (thick, tight panty hose that encourages circulation). And don't smoke — it increases the risk by a factor of nine, according to one study.

• Genes count. Get a "clotting workup," a blood test to detect inherited clotting disorders like factor V Leiden or a protein C or S deficiency. The conditions are rare, but if a family member has had a clot, get tested. (It costs about $200 if your insurance doesn't cover it.) Dehn recently told a patient whose sister had had a clot-related stroke to take the test before starting birth-control pills with estrogen. It turned out she had factor V Leiden, an inherited clotting disorder found in, on average, 5 percent of Caucasians; 2 percent of Latinos; and 1 percent or fewer of African-Americans, Native Americans, and Asians. There will be no hospital tours this year: I’m sorry to say that changes in hospital administration have made it impossible for me to take a few of my dancers to give intimate performances at military hospitals for wounded service personnel this year. We were fortunate to have been able to tour hospitals for the last few years to add a little spicy holiday cheer to the recuperating veteran’s lives. I was hoping our tour would become a holiday tradition, but I guess not.

Christmas on Virgin Gorda: Anya, Cyndi and I along with our boyfriends are going to my place on Virgin Gorda for the holidays. But first there will be a quick trip to see Taryn in Cambridge and go to Stonehenge for the Winter Solstice on Thursday December 22nd. Cyndi has never been to a Winter Solstice celebration at Stonehenge and she is looking forward to that as well as seeing the Duchess, her grandmother, again and we are all looking forward to swimming with the dolphins if a pod is in the cove this year. My ballet company is well into our Christmas performances and since St Lucy’s is on holiday for several weeks it’s given me the flexibility to take some time off. Recent FDA activity: Recommendations have been made concerning Ortho Evra and BCPs containing Drospirenone. Neither of which is a surprise. While the risk of serious side effects is greater when using these hormonal methods it isn’t at a level that would cause them to be banned. The decision to deny off-the-shelf availability of Plan B to teens under the age of 17 is not surprising either. While making Plan B available to younger girls has merit I think HHS is picking its battles giving first priority to getting Catholic institutions to provide employees with birth control in their health insurance policies. HHS/FDA can always come back later and approve sale of Plan B to girls younger than 17.

HHS blocks sale of Plan B to teens 16 and younger w/o an Rx: Bloomberg - Dec 7, 2011 “Teva Pharmaceutical Industries Ltd. (TEVA) failed to win U.S. clearance for the first over-the-counter emergency contraceptive after an Obama Cabinet member overruled the head of the Food and Drug Administration on the issue.

Secretary of Health and Human Services Kathleen Sebelius ordered Margaret Hamburg, the FDA chief, to reject the application by Petach Tikva, Israel-based Teva, citing potential sales to girls under age 17. Hamburg, in a statement, said she was ready to approve over-the-counter sales based on “well- supported” and “science-based evidence.” The FDA said it’s the first time the HHS has reversed one of its decisions.

The product, called Plan B One-Step, reduces pregnancy risk if taken within three days of sex. As a result of Sebelius’s order, it will continue to be kept behind pharmacy counters and sold without prescription only to women 17 and older. Personal comment: I’m not sure that this sort of publicity will substantially affect the sale of combined drospirenone oral contraceptives. In Europe it’s been known that there was a higher incidence of blood clots for years and sales continue strong. Knowing that drospirenone pills cause 10 adverse events in 10,000 women Vs 6 adverse events in 10,000 for pills made with other progestins doesn’t seem to be that serious, unless you are one of the additional four.

On the other hand, the sale of Ortho Evra has declined considerably since it was determined several years ago that there was an average of 60% more estrogen in an Ortho Evra patient than if they were taking an oral contraceptive with 35mcg of estrogen per pill. From what Ex-patch users tell me it is the high average level of estrogen which cause swollen painful breasts and heavier periods that have caused women to come off or avoid Ortho Evra since there are so many combined pills now available that have 20mcg of estrogen or less per pill.

Too much information? Birth control choices abound

“WASHINGTON (AP) - Worried about birth control in light of headlines about side effects from Yaz and the patch? Women have a lot of options that are safe and effective, including some that are even more reliable.

You can choose a contraceptive that's used daily, weekly, monthly, once every three months, once every three years, even once a decade.

Yet almost half of U.S. pregnancies are unintended _ and experts say confusion and uncertainty despite all the options is a big reason.

"We have a whole generation now of young adults, the vast majority of whom are sexually active, who are in a fog about modern contraception," says Sarah Brown of the National Campaign to Prevent Teen and Unplanned Pregnancy. "They don't know enough to make a reasonable choice."

Indeed, in a recent survey, the campaign found 42 percent of unmarried 18- to 29-year-olds said they knew little about birth control pills and two-thirds knew little about even more effective long-lasting contraceptives. A third said they believe there's a good contraceptive for their personal needs but they don't know which one.

Porn and the Prentif cap

Any producer who has his actress take fifteen minutes to languidly insert her Prentif as foreplay while her boyfriend/client is watching or has it pulled out by her attacker during her rape or has her lover eat her cervical mucus out of the dome is guaranteed a best seller.

Then too here is the ‘Geezer factor’ in niche-market porn especially in Europe where the Prentif was far more popular in the 50s than diaphragms and I’m told by the marketers that men of a certain age and style remember it fondly even though they now need performance enhancing meds to participate. Another advantage of a cap or diaphragm being used in porn is that unlike birth control hormones it is a physical object that can be inserted during a tender moment for protection or roughly pulled from the heroine or victim’s vagina during a rape and shoved into her mouth to emphasize the danger she is in. And for those into asphyxia a Prentif can be shoved down the woman’s throat as a very dangerous form of breath play because she can easily suffocate while her attacker is concentrating on planting his seed in her belly. With the increased interest in cervical barriers for use in porn – a lot of the resurgence has been because of the ‘Mad Men’ effect of retro TV shows that got the country longing for what seems a simpler time – our clinic is seeing a substantial increase in requests to fit Prentif caps.

Porn and poor cervical barrier technique: Porn actresses don’t rely on the effectiveness of a diaphragm or cap for contraceptive protection. Many of them have been sterilized or have had IUDs inserted so that an unplanned pregnancy from on-cam intercourse is very unlikely. That makes it far less important that most porn actresses be correctly fitted with a barrier that will remain securely in place through multiple acts of intercourse and for at least six hours afterward. All they need is one that won’t give them a UTI and won’t fall out while they are on-cam. Nor do they need to adhere to correct barrier insertion technique during a video production and that can leave the men and women watching the video with the wrong impression as to how to properly protect a woman using a cervical barrier.

The problem comes from the way the porn actresses are playing with their barriers and appear to become aroused long before insertion. As returning readers know a woman needs to insert her cervical barrier BEFORE she becomes aroused! That’s because during arousal the vagina lengthens (tents) as the uterus and thus the cervix is drawn up and out of the way of a thrusting penis. A vagina that is fully tented can make it very difficult or impossible to place a cervical barrier correctly over the cervix and therefore makes an unplanned pregnancy far more likely.

I’ve tried talking to several of the major producers of cervical barrier porn and they just laugh and say they are interested in getting their audience as hot as fast as possible and a porn-princess moaning and rubbing her barrier over her engorged labia wet with natural lube while she moans and writhes in ecstasy as she plays with herself is a critical part of the production and there won’t be any changes to correctly depict proper insertion technique. I suspected that was the answer I was going to get, but it was worth a try. It does make it more difficult to get vanilla women wanting to portray Magna Cum Loudly or other porn princesses to use their barriers correctly if they will need them for contraception and not just as a sex toy.

I’m trying to steer the porn actresses and their costumers toward diaphragms or FemCap and away from Prentif because the porn industry is not interested in contraception and as I mentioned above Prentif is now a device in short supply and the remaining caps should be used for women who intend to use them correctly for contraception and as gas-guards for upper reproductive tract protection during dive-sex.

Christmas break barrier fittings: At St Lucy’s it’s the end of Fall semester and we are finishing the fit checks – and replacements where necessary - of the cervical barriers the students wear before they go home or travel on Christmas break. While many of the upper form students were sexually active before arriving at St Lucy’s none had enough experience to have caused their vagina’s to stretch appreciably. While they have grown in the months they have been immersed in a strict ballet and pelvic training regimen the pelvic floor muscles have toned considerably making the vagina deeper and tighter. Fit checks are important before the increased use of cervical protection while away from the school’s sexual health support system. It’s also time for an ultrasound to verify the proper placement of students IUDs and for full STI panels to make certain the students are all healthy before sending them back to their families

The students in my Advanced Sexual Techniques class use their barriers as gas-guards for dive-sex while others on hormones use them for flow control for vanilla sex during their pill-periods or if they have a stringless IUD inserted. Stringless GyneFix IUD implants have become much more popular this year because they allow GyneFix users to safely wear a diaphragm or FemCap as a gas-guard during dive-sex. For anyone wondering the smallest Prentif, with an inner diameter of 22 mm, is too large for most teens to wear but they can wear a 22 mm FemCap because the FemCap seals against the walls of the vagina and not the cervix. So Prentif caps are left for the use of experienced adult women.

UNLV’s Fall semester also ends on December 17th and many of the sorority girls I know have had our clinic check the fit of their diaphragms and will be leaving mid week rather than on that Saturday so there has been a surge in barrier fit checks and some requests for STI panels for them as well. The students come to us to avoid having these items show up on their UNLV Student Health statements that are sent to their parents.

Checking Slit-kits: Before traveling for the holidays is also the time to check and restock everyone’s skit kits to ensure condoms, spermicide, antibiotics and emergency contraception pills are all fully stocked and in-date. There are two effective EC specific pills:

Plan B One-Step (a single pill containing 1.5 mg of the progestin levonorgestrel) which is effective for up to 72 hours, three days, after unprotected sex or a contraceptive failure. Plan Bs effectiveness decreases the longer after the event it’s taken so it should be taken as soon after the event of concern as possible. Women on oral contraceptives may continue taking them on their normal schedule after taking Plan B. In the U.S Plan B is available to women 17 years of age or older behind the counter w/o a prescription.

The second and newer pill is available in the U.S. as ella and in Europe as ellaOne (a single pill containing 30 mg of ulipristal acetate, a progesterone receptor modulator) which is effective for up to five days after the event of concern. Women on hormonal contraceptives should not rely on the effectiveness of their pills, but use a reliable barrier method of contraception after using ella, for any subsequent acts of intercourse that occur in that same menstrual cycle.

For EC our clinic now recommends ella, because of its longer effectiveness interval and at St Lucy’s its cost is covered by the student’s Student Health policy.

Thanksgiving weekend side effects: It’s now been a week since I had my Thanksgiving smorgasbord of studly men for the women in my circle to enjoy in safe and comfortable surroundings. Local self-defense expert Jennifer Cassetta has created the Stiletto Self Defense program that demonstrates how women can protect themselves.

She developed the techniques after she was attacked several years ago while walking on the street late at night.

"He grabbed me from behind and in that half a sec, like, I knew in my head from my training that I had an instant to choose whether to be a victim or a victor and I turned into what I liked to call a 'she beast,' basically went crazy like I was going to attack him," Cassetta said.

According to RAINN, the Rape, Abuse, & Incest National Network, someone in the U.S. is sexually assaulted every two minutes.

The 5 foot 1 inch third degree black belt in HapKido, a Korean martial art, was able to chase off her attacker while wearing high heels. She began teaching the program in 2006 using her 12 years of experience.

"I wanted women to just know enough of the basics to be able to tap into their inner power and ward off a predator," Cassetta said.

Cassetta explained her ABC's of self-defense: Awareness, Boundaries, and being Centered and Calm in the face of adversity.

"We have on our bodies five different weapons, pretty much, and they start with the hands, elbows, knees, feet and your head," Cassetta said.

As for those heels, Cassetta suggested wearing comfortable ones that you can run in if necessary.

"The first thing you do with those sexy stilettos is stomp your heel right down on the foot," Cassetta said. "Hopefully it will go right through the foot. You can also take the foot and go up the groin with that stiletto, as well."

Cassetta said, remember to let out your inner "she beast" and be loud, calling attention to your situation.

"It all starts in the mind and it's a mind body spirit class ... if you think you are powerful enough to get away in a situation like that, most likely you can," Cassetta said.

Another tip: think like a predator.

"How can you avoid being a target? You want to stand up straight make sure you are looking around, shoulders back,” she said. “You see girls texting and headphones. None of that.”

Two 20-something's taking her class were inspired by what they learned.

"I walk around with my headphones and disappear into my music, and I never thought I am very vulnerable at that moment," Stephanie McCall said.

Tina Pareedes said the class challenged her habits, as well.

"I usually avoid looking at people in the eye walking around the street but once you get that in your head to just go after someone in the face, yea," Pareedes said.

Cassetta holds local workshops once a month but is also available by appointment for special classes. She recently held a class in one part of Los Angeles where there was a spike in sexual assaults.

"I have mothers, a lot of mothers, who have girls going off to college that will call me and schedule a class" Cassetta said.

Personal Comment: I’ve always liked the idea of fighting back and using whatever is at hand as weapons and punching a 3 or 4 inch stiletto heel through an attacker’s foot can be pretty effective. Of course if you miss you lose the advantage of surprise that you are actually fighting back. Another tactic I like if I’m in flats or sneakers is to kick back into an attacker’s knees. If you break the knee he’s down and not going very far then you can safely run. Of course too there is the Wal-Mart Black Friday weapon of choice, pepper spray, if you have it where you can get at it, like in your hand as you walk to your car.

We had all the dive computers set up by our Divemaster so even the teens could safely use Nitrox II at the bottom of my 68 ft deep pool, The Pit. At a max depth of 68 feet there should be little danger of oxygen toxicity while using 36% oxygen gas to reduce nitrogen loading and so extend bottom time for underwater sexual encounters. This has worked extremely well and we all had at least one bottom encounter before breakfast.

THE 1920s notion of a “hemline index,” in which the economist George Taylor posited that skirt lengths rise and fall in relation to the economy, suggests that fashion is socially determined. In a modern twist, a report about the direction of high heels, issued by I.B.M., proposes that fashion can now be determined through social media.

To promote its software and consulting services, I.B.M. announced that its computer analysis of “billions of social media posts” pointed to a downward trend in heel heights. This was surprising, the company said, because heels usually go up during an economic downturn.

While an intriguing thesis, it bears some fact-checking.

First, I.B.M.’s case: By mapping the most influential participants in online conversations about shoes, the company was then able to eavesdrop on a dozen key bloggers. It found that the median heel heights mentioned on those sites dropped to two inches this year from seven inches in 2009.

But did women really wear seven-inch heels in 2009? Is that physically possible?

Trevor Davis, who led the I.B.M. survey, argued: “The absolute number is not really what is of great interest here. It is the relevant movement.”

There is, indeed, anecdotal evidence of a decline in heel height, but mixed opinions about whether that has anything to do with the economy. Colleen Sherin, the senior fashion director of Saks Fifth Avenue, wasn’t buying it. Yes, flats are going to be big for spring, but so are wedges.

“I know that people like to take an economic read from heel heights, skirt lengths and selling red lipsticks,” she said, “but it is just the cycle of fashion.”

Elizabeth Semmelhack, the senior curator at the Bata Shoe Museum in Toronto, has proposed that heels grow higher in a bad economy, citing the introduction of platforms during the Great Depression and their reappearance during the oil crisis in the 1970s and again during the dot-com bust. But even that was a “casual observation,” not gospel, she said.

Valerie Steele, the chief curator at the Museum at F.I.T., said the evidence does not hold up, even if people are talking about it online.

“You can have absolutely vertiginous heels and, at the same time, sell billions of ballet flats,” Ms. Steele said. “It all goes back to that Mark Twain quote: ‘Lies, damned lies and statistics.’ ”

Personal comment: Seven inch heels? The IBM computer must have been programmed to search fetish sites and even then there are only a few of us who can actually wear ballet-boots or pointe shoes, the heelless ultimate in high heels, to walk around en pointe confidently in them. I tend to agree with Valerie Steele. However, I will say that in 2008 when the stock market went into free-fall the number of senior business men who sought to escape the economy by immersing themselves in pointe shoe and ballet-boot sexual fantasies available in Vegas skyrocketed so it became difficult to find enough elite talent to service the demand for ballet-sex. “Why Do Oral Contraceptives that Cause Monthly Bleeding Still Predominate?

Healthcare providers' attitudes about menstruation and medically induced amenorrhea are key factors.

Extended or continued use of oral contraceptive pills (OCs) remains low despite obvious advantages of menstrual cycle regulation and symptom management. To determine the factors that influence physicians' OC prescribing habits, researchers analyzed survey responses from 211 obstetrician-gynecologists (OB/GYNs) and family medicine physicians (FPs) in Oregon. Physicians were asked whether they prescribed extended-use (hormone-containing pills for >28 days with scheduled withdrawal bleeding) and continuous use (hormone-containing pills indefinitely without a scheduled withdrawal bleed) OCs, and, if yes, how often. A 5-point Likert scale was used to assess attitudes toward menstrual suppression.

In all, 90 respondents (44%) identified their principal specialty as OB/GYN, and 115 (56%) as FP. Most respondents who reported ever prescribing OCs also said that they prescribed extended- or continuous-use OCs sometimes (50%) or often (23%). OB/GYNs were more likely than FPs to suggest these options (92% vs. 60%), as were urban (79%) versus rural (58%) physicians.

OB/GYNs and urban physicians showed greater acceptance of medically induced amenorrhea as well as more-favorable attitudes about health and economic benefits of suppressing ovulation and safety of extended-use OCs. Most respondents (68%) did not realize that menstrual-related symptoms are most common during the placebo week, and 17% of respondents (most of whom were FPs) were unaware that endometrial thickening does not occur with extended-use OCs. After adjusting for physicians' age, practice location, knowledge, specialty, and sex, the only factor that was significantly associated with prescribing extended- or continuous-use OCs was attitude toward medically induced amenorrhea.

Comment: The gaps in knowledge among the respondents are surprising and underscore the need for ongoing education about menstrual suppression. The authors acknowledge that their small sample size (particularly of rural providers) curtails the generalizability of these results. Moreover, nurse practitioners, physician assistants, and certified nurse midwives were not included. Despite these study limitations, the benefits of extended- and continuous-use oral contraceptives remain clear — and the attitudes of healthcare providers are critical in promoting acceptance of these dosing regimens as mainstream options. — Anne A. Extended cycle pills in real life: The first combined (estrogen/progestin) oral contraceptive pill designed to be taken continuously with no hormone free days is Lybrel. Lybrel has .90 mcg of the progestin levonorgestrel and 20 mcg of the estrogen ethinyl estradiol and is taken continuously 365 days/yr, in 28 day packs (13 per year). For women who can adjust to the continuous dose of hormones – it can take three or four months for a significant percentage of users to stop experiencing irregular bleeding – being period free can be liberating if the woman has been experiencing heavy or painful periods or significant PMS. Another positive for Lybrel is that it uses a very low dose of an older progestin, levonorgestrel, which has a lower incidence of causing blood clots. Most other extended cycle pills have 84 active and 7 placebo pills limiting a woman taking them to four pill-periods a year. Some of those are Seasonale, Jolessa, Quasense, Seasonique and LoSeasonique and they all use the progestin levonorgestrel.

The down side of extended cycle pill is that in addition to taking a long time to adjust to being continuously on hormones levonorgestrel does not have the long half-life of the progestin drospirenone used in Yaz and Yasmin and as a result the hormone levels can more easily drop below the effectiveness level if a woman is late with or vomits her pill. Too, there is no monthly assurance that the user is not pregnant by the arrival of ‘Aunt Flo’. Unless a user is cautious enough to test every few weeks she has no way of knowing if she is pregnant until she begins to develop symptoms such as noticeable breast changes, lethargy and repeated nausea.

Mifeprex is about 95% effective in terminating pregnancies so she should be fine. In two weeks she will need to go back to the clinic to make certain that everything is normal and her reproductive tract has recovered.

Merck’s new single rod contraceptive implant

New Version of Contraceptive Implant Is Easier to Insert. Nexplanon Is Designed to Be Put Easily Under Skin by a Health Care Professional

"Nov. 10, 2011 -- The drugmaker Merck has introduced a new version of its long-acting contraceptive implant Implanon, designed for easier insertion and removal by health care professionals.

The new version of the contraceptive is called Nexplanon. Like Implanon, it is a progestin-only hormonal birth control delivered through a flexible rod the size of a matchstick. It is inserted under the skin of a woman's upper arm to prevent pregnancy for as long as three years.

But unlike its predecessor, the new version the rod is made to be easily visible by X-ray, CT, MRI, or ultrasound.

This was done, in part, to make removal of the rod easier when it cannot be located manually.

Inserting Nexplanon

The contraceptive rod still must be implanted by a specially trained health care professional. But a new inserter makes that process much simpler, says ob-gyn Jill Rabin, MD.

Rabin is the head of urogynecology at the Long Island Jewish Medical Center-North Shore LIJ Health System. She has trained other health care professionals to use the new device.

"Instead of four to six steps, the new inserter has two," she tells WebMD. "It is very hard to place it improperly and next to impossible to put in too deeply."

Implanon was first approved for use in the U.S. in 2006, after almost a decade of use in more than 30 other countries.

Effectiveness of Contraceptive Implants

Contraceptive implants are among the most reliable birth control devices, says Juan C. Arjona Ferreira, MD. He is a senior project leader for diabetes and endocrine research with Merck.

"When implanted correctly, this method is more than 99% effective with less than one pregnancy occurring for every 100 women using it for a year," he says.

The implantable contraceptive prevents pregnancy for up to three years but can be removed at any time, making it a completely reversible, long-acting form of birth control.

It can also be used about three weeks after childbirth and by women who are breastfeeding about one month after delivery.

Nexplanon prevents pregnancy in three ways: by inhibiting the release of the egg from the ovaries, changing the mucus of the cervix to keep the sperm from reaching the egg, and making it harder for a fertilized egg to implant and develop.

The most common side effect seen with implantable contraception is change in menstrual cycles. Some women have no periods on the birth control, while others have irregular bleeding or heavy bleeding.

A Merck spokesman says the new version of the contraceptive implant is less likely than earlier versions to be inserted improperly or too deeply, which can lead to scarring and nerve damage."

Personal comment: Gyns and NPs at our clinic have all been trained in the greatly simplified implantation process and our clinic will be one of the first to offer Nexplanon to patients who are good candidates for the progestin etonogestrel, the same one used in NuvaRing.

Daylight saving time will end in the U.S and Canada at 2:00 AM this coming Sunday, November 6th. Clocks will be set back one hour for areas that are participating in DST and we all get an extra hour of sleep Sunday morning.

So what does this have to do with oral contraceptives? Not much if you are taking a combined (estrogen/progestin) pill because being ‘late’ by one hour due to coming off DST will make no appreciable difference. That is also true for other combined hormonal methods such as Ortho Evra (the patch), and NuvaRing (the ring).

However, where being an hour late taking an oral contraceptive could make a difference is for women on progestin-only-pills (AKA POPs or the mini-pill) because of the very low dose of older progestins with relatively short half-lives in that kind of pill which requires taking an active pill at the same time every day.

For women on most brands of mini-pills like Ortho Micronor, Nor-QD, Camila, Errin, Jolivette and Nora-BE that use the progestin norethindrone and Ovrette and its generics that use the progestin norgestrel it’s important that the hour gained from the end of DST not cause that days pill time to be more than 3 hours late. There is a need to use a backup method such as condoms and spermicides for the next 48 hours whenever a progestin-only oral contraceptive is taken 3 or more hours late.

For women on the mini-pill Cerazette which uses the much longer half-life progestin, desogestrel, taking the pill one hour late should not make a difference because with Cerazette you can – according to the patient information leaflet - take a pill up to 12 hours late and still be protected from pregnancy.

A Halloween fright: The UNLV sorority’s Hex or Sex party over the Halloween weekend at Adolph’s estate in the Spring Mts was a huge success with nearly $20,000 raised for charity in the four days the girls worked the mansion. Last Call for Adderall: Where'd all the 'neuroenhancers' go?

“There are a bunch of Connecticut college students who are likely getting a little jittery right now, and it’s not from taking too many drugs. Their problem is a shortage of one of their favorite “neuroenhancers,” commonly known as Adderall.

Adderall is a drug made up of “mixed amphetamine salts” that’s commonly prescribed for people suffering from attention-deficit hyperactivity disorder (ADHD, also known as attention-deficit disorder or ADD) to help them concentrate better.

The months-long shortage of the drug is obviously a pain in the ass for lots of people, adults as well as children, who need Adderall to deal with their medical condition.

But Adderall has become an illegal FAVORITE OF college students looking for more effective ways to pull off all-nighter marathon study and writing sessions or to be able to both party and make classes the next day. The fact it’s a federally controlled drug hasn’t reduced its underground popularity one bit. By some estimates, Adderall use on college campuses is at the 20 percent level or higher.

Reports of Adderall supply problems have been making headlines across the nation for some time now and Connecticut hasn’t been immune.

“It’s probably been going on for a couple of months,” says Rick Carbray, a licensed pharmacist with the Apex Pharmacy in Hamden. He says the shortages in Connecticut at the moment involve “instant relief” Adderall tablets in various dosage categories.

“There’s no problem with Adderall extended relief,” he adds, but that’s no comfort for patients looking for the instant relief tablets.

Marghie Giuliano, executive vice president of the Connecticut Pharmacists Association, says reports from the group’s members across the state indicate that “supply has been sporadic.”

The Federal Drug Administration website lists a number of different reasons for the Adderall difficulties, depending on which manufacturer is involved.

The listed causes include chemical “supply issues,” “inadequate finished product supply to support current demand,” “increase in demand,” and “manufacturing delays.”

The FDA normally limits the amount of ingredients available to companies producing amphetamine-based drugs, but federal officials say those companies can apply to have those limits raised.

There were reports earlier this year of shortages of the extended-relief form of Adderall made by Shire, and that the company boosted prices for that drug by 20 percent or more. A spokesman for Shire says there’s no current shortage of Adderall Extended Relief.

Carbray says the problems at the moment seem to involve shortages of 5 milligram and 20 milligram tablets of the instant-relief variety. The result is that pharmacists are having to mix and match different dosage combinations to get patients the right amounts for their prescriptions, according to Carbray.

He says it stands to reason that, if legitimate users are having trouble getting Adderall, illegal users like college students will be having even more difficulty. That’s because virtually all illegal Adderall usage involves someone getting the drug from a person who has a prescription.

One California study found that full-time college students in the 18-22 age group were twice as likely to have used Adderall as non-students. A recent report in a University of Virginia newspaper quoted one illegal Adderall dealer as saying sales of the drug spike just before mid-terms and finals.

While students who use Adderall as a “neuroenhancer” claim it helps them stay awake and focused for long periods of time, use of the drug comes with some ugly risks. Side effects from overdoses can include confusion, uncontrollable shaking, depression, irregular heartbeats, hallucinations, upset stomach and loss of appetite, even vomiting and diarrhea.”

The Doorman: The sorority Hex or Sex Halloween party that has gone on for the last four days has been a huge success; not only because of the body-condom clad rubber-chicks, but because of the venue. Push for ‘Personhood’ Amendment Represents New Tack in Abortion Fight

“A constitutional amendment facing voters in Mississippi on Nov. 8, and similar initiatives brewing in half a dozen other states including Florida and Ohio, would declare a fertilized human egg to be a legal person, effectively branding abortion and some forms of birth control as murder.

With this far-reaching anti-abortion strategy, the proponents of what they call personhood amendments hope to reshape the national debate.

“I view it as transformative,” said Brad Prewitt, a lawyer and executive director of the Yes on 26 campaign, which is named for the Mississippi proposition. “Personhood is bigger than just shutting abortion clinics; it’s an opportunity for people to say that we’re made in the image of God.”

Many doctors and women’s health advocates say the proposals would cause a dangerous intrusion of criminal law into medical care, jeopardizing women’s rights and even their lives.

The amendment in Mississippi would ban virtually all abortions, including those resulting from rape or incest. It would bar some birth control methods, including IUDs and “morning-after pills” that prevent fertilized eggs from implanting in the uterus. It would also outlaw the destruction of embryos created in laboratories.

The amendment has been endorsed by candidates for governor from both major parties, and it appears likely to pass, said W. Martin Wiseman, director of the John C. Stennis Institute of Government at Mississippi State University. Legal challenges would surely follow, but even if the amendment is ultimately declared unconstitutional, it could disrupt vital care, critics say, and force years of costly court battles.

“This is the most extreme in a field of extreme anti-abortion measures that have been before the states this year,” said Nancy Northrup, president of the Center for Reproductive Rights, a legal advocacy group.

Opponents, who were handing out brochures on Saturday to tailgate partiers before the University of Southern Mississippi football game in Hattiesburg, said they hoped to dispel the impression that the amendment simply bars abortions — a popular idea in Mississippi — by warning that it would also limit contraceptives, make doctors afraid to save women with life-threatening pregnancies and possibly hamper in vitro fertility treatments.

The drive for personhood amendments has split the anti-abortion forces nationally. Some groups call it an inspired moral leap, while traditional leaders of the fight, including National Right to Life and the Roman Catholic bishops, have refused to promote it, charging that the tactic is reckless and could backfire, leading to a Supreme Court defeat that would undermine progress in carving away at Roe v. Wade.

The approach, granting legal rights to embryos, is fundamentally different from the abortion restrictions that have been adopted in dozens of states. These try to narrow or hamper access to abortions by, for example, sharply restricting the procedures at as early as 20 weeks, requiring women to view ultrasounds of the fetus, curbing insurance coverage and imposing expensive regulations on clinics.

The Mississippi amendment aims to sidestep existing legal battles, simply stating that “the term ‘person’ or ‘persons’ shall include every human being from the moment of fertilization, cloning or the functional equivalent thereof.”

A similar measure has been defeated twice, by large margins, in Colorado. But the national campaign, promoted by Personhood USA, a Colorado-based group, found more receptive ground in Mississippi, where anti-abortion sentiment crosses party and racial lines, and where the state already has so many restrictions on abortion that only one clinic performs the procedure.

In 2009, an ardent abortion foe named Les Riley formed a state personhood group and started collecting the signatures needed to reach the ballot. Evangelicals and other longtime abortion opponents have pressed the case, and Proposition 26 has the support of a range of political leaders. Its passage could energize similar drives brewing in Florida, Michigan, Montana, Ohio, Wisconsin and other states.

In Mississippi, the emotional battle is being fought with radio and television ads, phone banks and old-fashioned canvassing.

Among the picnicking fans being lobbied outside the stadium in Hattiesburg on Saturday, Lauree Mooney, 40, and her husband, Jerry Mooney, 45, U.S.M. alumni, disagreed with each other. She said that she is against abortion but that the amendment is “too extreme.” Mr. Mooney said he would vote yes because “I’ve always been against abortion.”

Shelley Shoemake, 41, a chiropractor, said the proposal is “yanking me in one direction and the other.” She knows women who had abortions as teenagers, and feels compassion for them. “I’ve got a lot of praying to do” before the vote, she said.

Mississippi will also elect a new governor on Nov. 8. The Republican candidate, Lt. Gov. Phil Bryant, is co-chairman of Yes on 26 and his campaign distributes bumper stickers for the initiative. The Democratic candidate, Johnny DuPree, the mayor of Hattiesburg and the state’s first black major-party candidate for governor in modern times, says he will vote for it though he is worried about its impact on medical care and contraception.

No one can yet be sure of how the amendment would affect criminal proceedings, said Jonathan Will, director of the Bioethics and Health Law Center at the Mississippi College School of Law. Could a woman taking a morning-after pill be charged with murder?

But many leaders of the anti-abortion movement fear that the strategy will be counterproductive. Federal courts would almost surely declare the amendment unconstitutional, said James Bopp Jr., a prominent conservative lawyer from Terre Haute, Ind., and general counsel of National Right to Life, since it contradicts a woman’s current right to an abortion in the early weeks of pregnancy.

“From the standpoint of protecting unborn lives it’s utterly futile,” he said, “and it has the grave risk that if it did get to the Supreme Court, the court would write an even more extreme abortion policy.”

Bishop Joseph Latino of Jackson, Miss., said in a statement last week that the Roman Catholic Church does not support Proposition 26 because “the push for a state amendment could ultimately harm our efforts to overturn Roe vs. Wade.”

Conservative Christian groups including the American Family Association and the Family Research Council are firmly behind the proposal.

Dr. Randall S. Hines, a fertility specialist in Jackson working against Proposition 26 with the group Mississippians for Healthy Families, said that the amendment reflects “biological ignorance.” Most fertilized eggs, he said, do not implant in the uterus or develop further.

“Once you recognize that the majority of fertilized eggs don’t become people, then you recognize how absurd this amendment is,” Dr. Hines said. He fears severe unintended consequences for doctors and women dealing with ectopic or other dangerous pregnancies and for in vitro fertility treatments. “We’ll be asking the Legislature, the governor, judges to decide what is best for the patient,” he said.

Dr. Eric Webb, an obstetrician in Tupelo, Miss., who has spoken out on behalf of Proposition 26, said that the concerns about wider impacts were overblown and that the critics were “avoiding the central moral question.”

“With the union of the egg and sperm, that is life, and genetically human,” Dr. Webb said.

Keith Mason, president of Personhood USA, said he did not agree that the Supreme Court would necessarily reject a personhood amendment. The ultimate goal, he said, is a federal amendment, with a victory in Mississippi as the first step.”

The ‘Personhood’ Initiative

“A ballot measure going before voters in Mississippi on Nov. 8 would define the term “person” in the State Constitution to include fertilized human eggs and grant to fertilized eggs the legal rights and protections that apply to people. It is among the most extreme assaults in the push to end women’s reproductive rights.

The aim is to redefine abortion and some of the most widely used forms of contraception as murder, obliterating a woman’s right to make childbearing decisions under the 1973 Supreme Court decision Roe v. Wade.

Besides outlawing all abortions, with no exceptions for rape or incest or when a woman’s life is in danger, and banning any contraception that may prevent implantation of a fertilized egg, including birth control pills, the amendment carries many implications, some quite serious.

It could curtail medical research involving embryos, shutter fertility clinics and put doctors in legal jeopardy for providing needed medical care that might endanger a pregnancy. Pregnant women also could become subject to criminal prosecution. A fertilized egg might be eligible to inherit money or be counted when drawing voting districts by population. Because a multitude of laws use the terms “person” or “people,” there would be no shortage of unintended consequences.

A similar ballot measure was handily rejected by Colorado voters in 2008 and 2010. But, in Mississippi, which has already imposed so many burdensome restrictions that the state has only one abortion clinic, there is a real possibility that voters will not react as wisely. Voter approval could energize similar “personhood” initiatives in half a dozen other states, including Florida and Ohio.

Both the Republican and Democratic candidates for governor in Mississippi have endorsed the measure, even though some traditional leaders in the anti-abortion battle, including National Right to Life, have declined to do so, viewing it as a reckless strategy that could lead to a defeat in the Supreme Court. This extreme measure would protect zygotes at the expense of all women while creating a legal quagmire — at least until the courts rule it unconstitutional, as they should.”

Personal comment: If the knuckle draggers who are pushing ‘personhood’ for fertilized eggs have their way women in Mississippi and other radically conservative states will be continuously under suspicion, except perhaps for Nuns. I can’t imagine that legislation so terribly ill-advised would pass, but in the Deep South you never know. Hopefully women who use birth control will vote against ‘Personhood’ as the backers will almost certainly ban hormonal contraceptives and IUDs as abortifacients because with ‘rights’ granted to a fertilized egg the possibility of an implantation failure would be murder.

How would they know a fertilized egg didn’t implant? It is very difficult to detect an egg expelled from the uterus. Would ‘Fertility Police’ be established to test all women between the ages of 12 and 50 on a weekly basis for pregnancy so the government could ensure any fetus was carried to term? And the conservatives say they want smaller less invasive government! Go figure!

“Worrisome increases in thrombotic risk associated with hormonal contraceptives are not limited to those containing drospirenone, according to an FDA report issued Thursday.
Compared with hormonal contraceptives with relatively low estrogen doses, norelgestromin/ethinyl estradiol transdermal patch and etonogestrel/estradiol vaginal ring products were both associated with increased risk for venous thromboembolism (VTE), as were birth control pills containing drospirenone, according to the retrospective database analysis.

FDA researchers ran the analysis on records maintained by Kaiser Permanente for members in California and state Medicaid programs in Tennessee and Washington, covering a total of more than 835,000 women who received contraceptive prescriptions from 2001 to 2007.

After adjusting for demographic and other factors, the analysis indicated the following relative risks, with low-estrogen hormonal contraceptives as the reference:

• Drospirenone products: 1.74 (95% CI 1.42 to 2.14)

• Norelgestromin/ethinyl estradiol transdermal patch: 1.55 (95% CI 1.17 to 2.07)

• Etonogestrel/estradiol vaginal ring: 1.56 (95% CI 1.02 to 2.37)

Younger women (those up to age 34) appeared to be at higher risk for VTE for all three product types, the study showed.

And the clotting risk was not limited to VTE -- arterial thromboembolism was significantly increased among women 35 and older taking drospirenone products for the first time.

Thrombosis has long been a concern for hormonal contraceptives of all types and doses. But a number of studies have suggested that the risk differs among the multitude of product types now available.

Drospirenone came into the FDA's crosshairs earlier this year when two studies found that the VTE risk was doubled or tripled relative to levonorgestrel-containing pills.
Previous studies had also identified possible increases in risk associated with the patch and vaginal ring products, although the FDA had not formally announced a safety review, as it had done for the drospirenone products.

All of the previous studies were relatively small and fell short of the quality needed to form the basis for regulatory action, the FDA indicated.

Late last month, after reviewing the preliminary data from the Kaiser/Medicaid analysis, the agency said it still had not reached a conclusion about the degree of risk. That remains the case, according to the FDA this week.

It plans to present all the data to its Reproductive Health Drugs and Drug Safety and Risk Management advisory committees at a joint meeting on Dec. 8.”

Personal comment: Birth-control pills that contain drospirenone include Bayer's Yaz, Yasmin, Beyaz, Safyral; Sandoz's Syeda and Loryna; as well as Barr Laboratories' Ocella, Watson Pharmaceuticals' Zarah and Teva Pharmaceuticals' Loryna. Etonogestrel is also used in the single rod implant Implanon.

“All males starting at age 11 should receive the HPV vaccine Gardasil to protect themselves against sexually transmitted forms of human papillomavirus, the cause of most cervical and anal cancers as well as most mouth and throat cancers, a Centers for Disease Control and Prevention advisory committee voted today.

Thirteen members of the committee voted in favor of extending the HPV vaccine recommendation to young boys, and one member abstained. The recommendation now goes to the director of the CDC and the secretary of the U.S. Department of Health and Human Services for final approval.

The CDC already recommends routinely immunizing girls with a three-dose vaccine beginning at age 11 or 12, before they become sexually active, although they can be vaccinated as young as age 9. The agency previously issued a so-called permissive recommendation giving boys and young men from ages 9 through 26 the option of receiving the vaccine.

The prospect of requiring that preteen boys and girls get vaccinated against a sexually transmitted infection has drawn the sharpest outcry from some parents, who fear that vaccinating preteens might encourage promiscuous behavior. Vaccination policies also have become an issue in the 2012 presidential campaign, with several GOP candidates objecting to mandates for HPV vaccination.

Many infectious disease specialists welcome universal vaccination as a key tool in reducing the toll of preventable diseases.

"Vaccination is perhaps the greatest invention of medicine," said Dr. John Sinnott, director of the Division of Infectious Disease at the University of South Florida. "It is a tragedy that this vaccine has become politicized." But Dr. Lawrence Stanberry, chief pediatrician at New York Presbyterian Morgan Stanley Children's Hospital, said parents support universal recommendations more recommendations targeting groups at higher risk.

"Recommending universal immunization for girls and making the recommendation for boys permissive sends parents mixed messages," Stanberry said. He offered a fairness argument for recommending vaccinations for both sexes. "Girls acquire the infection from boys and it seems appropriate, even fair, for boys to share responsibility for maximizing community [herd] immunity," he said.

Nearly a dozen experts contacted by ABC News cited "herd protection" from HPV-associated diseases in both women and men as a main reason they supported universal immunization.

William Muraskin, an urban studies professor at Queens College in New York, said it's hard to determine which men and women will benefit directly from vaccination. "The HPV vaccine if given before males become sexually active will also protect those who will become homosexual or bisexual," Muraskin said. Routine vaccination of all young men protects "an important subgroup that otherwise will be at significant risk but cannot be identified until it is too late."

The panel is expected to issue a policy statement on the importance of vaccinating men who have sex with men, because of the risk they could develop anal cancer from HPV.

Two HPV Vaccines on The Market Protect Against Multiple Strains

There are two HPV vaccines currently available, both of which protect against the two principal cancer-causing types of HPV, HPV 16 and HPV 18. By CDC estimates, the two strains cause 15,000 malignancies in women and 7,000 in men every year. The two strains account for an estimated 70 percent of cervical cancers, about 70 percent of vaginal cancers and most vulvar cancers. HPV 16 by itself causes 85 percent of anal cancers.

In addition to protecting against HPV 16 and HPV 18, Merck's Gardasil protects against two other disease-causing strains, HPV 6 and HPV 11, responsible for 90 percent of genital warts as well as cervical cell changes. GlaxoSmithKline's Cervarix protects against HPV 16 and HPV 18, but also has been found to confer some protection against three other cancer-causing strains, HPV 31, HPV 33 and HPV 45.

Dr. Diane Harper, director of the Gynecologic Cancer Prevention Research Group at the University of Missouri-Kansas City called it "misguided to think that all boys will gain any health benefit from HPV vaccination."

She contended that "mass vaccination for the prevention of the other HPV-associated cancers puts large numbers of people at risk for harms from vaccination compared to both the personal and public health risk of anal, penile, and oropharyngeal cancers."

In a background memo leading up to the vote, the CDC estimated that routinely vaccinating 11- and 12-year-old boys would likely be cost-effective. If 1 million 12-year-old boys were vaccinated, over the course of a lifetime, they would prevent 2,381 cases of mouth and throat cancer; 633 cases of anal cancer and 169 cases of penile cancer, assuming the vaccine was 75 percent effective against those conditions.”

Personal Comment: I’m so pleased that the CDC will probably recommend Gardasil for boys! All the men in our casino’s escort training program are required to have completed the entire three injection series of Gardasil vaccine before completing the course. And male escorts used as lab instructors for St Lucy’s students (in AST as well as the more vanilla Contemporary Sexual Health) must have completed the full injection series before they can become a trainer at St Lucy’s.

“ON a recent summer evening at Mr. Black, a dance party held every Tuesday at Bardot on North Vine Street, Sean Wagner was towering above the crowd. Mr. Wagner, 23, is tall even in socks (6-foot-2), but that night he had some extra help.

On his Size 11 feet were a pair of eight-inch bright neon green lace-up stiletto ankle boots, procured from the Ladies Studio Exotic Shoes on Hollywood Boulevard.

He jokingly dubbed them “my usual hiking shoes.”

Mr. Wagner was otherwise dressed in men’s clothing: a loose-fitting black tank top with a pair of tapered black pants. He had a neatly trimmed beard, and a pair of black-framed glasses sat perched on his nose.

“I never leave the house with less than eight inches on my feet,” he said cheerfully. “It helps you see over the cattle.”

Mr. Wagner was not the only man wearing high heels (but no other women’s clothing) that night. At Mr. Black, Luke Nero, a promoter, estimated that 10 or more men were traversing the dance floor in a pair of pumps. “I went to a loft party yesterday, and there was a guy in normal shorts, normal tank and really hot red pumps. That’s it!” he said. “Everyone was like, ‘Oh my God, I love those shoes!’ ”

In a way, Mr. Wagner and his stiletto-wearing cohorts are repeating history. Until Napoleon banned them, high heels were considered a sign of nobility in France during the 18th century and were favored by men as well as women; long before Louboutin, Louis XVI donned five-inch red-accented heels depicting wartime battle scenes.

In 2009 the Atlanta hairstylist Derek J became famous after appearing on “The Real Housewives of Atlanta” wearing women’s pumps with jeans and a sweater, the same year that the designer Rad Hourani sent male models down the runway in boot heels reminiscent of Prince and ’70s glam rockers like David Bowie and David Johansen of the New York Dolls.

At Mr. Black, two best friends, Coy Barton, 24, and Mark Cramer, 25, who go out together as a duo dubbed Coma, were dressed alike in buttoned-up white shirts, dark gray dress slacks pegged at the ankle — and black leather ankle boots with peep toes that showed off their black painted toenails. “I’m in Steve Madden, he’s in Chinese Laundry,” said Mr. Barton of their shoes, “These were $115. Mine were, like, $170.”

Expensive, yes, but nothing compared with the price of Gregory Alexander’s prized pair of Balenciaga six-inch wooden wedges: $2,000. They were safely ensconced in his closet. That evening, Mr. Alexander, 26 and a host at Mr. Black, had paired Yves Saint Laurent’s Imperiale platform stiletto ankle boot (original retail price: $1,395) with a leather motorcycle jacket, tight black jeans, a white shirt and a skinny black tie. “They were a pretty penny,” said Mr. Alexander, 26, referring to the YSL’s. “They were a Valentine’s present to myself. I had them engrave a card for me, too.”

Mr. Alexander, who runs a popular party called A Club Called Rhonda, said he owns about 30 pairs of women’s heels. He wears a women’s size 11, and just barely fits in most designer shoes.

Mr. Wagner, meanwhile, said he often resorts to paying for custom-made heels for his larger feet.

“I love the height,” he said. “It helps when you’re in a club. I’ve bought Louis Vuitton. I’ve bought Gucci. But a lot of designers don’t go high enough for me. I found a company in Arizona that will do 15-inch heels for $3,000.”

Jeff Paice, a clothing designer at Mr. Black who had dressed up his buttoned-up black shirt and black pants with a pair of sandal wedges, said he was bored with the usual choices. “There’s nothing for guys,” he said.

Mr. Barton agreed: “I literally look at girls and think, you have so many options. You have jumpers, you have skirts, you have dresses, you have pants, you have shorts. Boys have pants and shorts. Or suits and a shirt.”

None of the men interviewed considered themselves to be in drag. “I always make it very clear that I am a man, and I’m not trying to portray an illusion to anybody,” Mr. Wagner said.

Though some would call it a form of drag, he added, “As far as we’re concerned, this is just bringing a look to a club — which is what you are supposed to do.”

“I wish society was more acceptable of men wearing heels,” Mr. Paice said. “I think it’s fun. I think it makes a statement.”

Mr. Alexander cited reasons for wearing high heels many women have known since they were invented: “It’s a power thing. You’re higher than everybody else. You make more sound. You walk a different way. It makes your legs look better.”

He added: “I don’t ever take them off. I even drive in them — stick shift.”

Last summer, he broke his ankle trying to jump a fence to get into a party while wearing a pair of heels. “I was in a cast for four months,” he said. “They told me I should never wear heels again, obviously.”

“But I don’t know,” Mr. Alexander added, admiring his YSL-clad feet. “I’m back.””

“CHRISTIAN LOUBOUTIN has launched his first ever book, a beautiful tome featuring the designer's most iconic styles as well as an insight into his influences and photos from his personal archives.

"Rizzoli approached me to work on a book together and it happened to be perfect timing as my 20th anniversary was approaching," Louboutin told us this afternoon. "Without a doubt my favorite element is the photography of the shoes by Philippe Garcia. In researching the book, I was also obliged to revisit my own personal archive of photos and sketches and I was impressed at the amount that I had saved from those early days.

"The most important thing to me when considering the photography was to ensure my favorite pieces from the 20 years photographed in a beautiful way. The choice of photographer was crucial - I didn't want to work with a fashion photographer who would be likely to photograph them in a seasonal way as a book has a much longer life than a magazine. The book also includes photography from a past collaboration with film director David Lynch and we made a conscious decision to shoot the shoes and bags chosen for the book against a light background to contrast against their darker aesthetic."

The book is divided into six chapters; the first documents Louboutin's biography, noting his early work for Chanel and Yves Saint Laurent; the second looks at the various interiors of the label's international stores; the third charts 20 years of Louboutin design, the fourth features intimate photographs of the designer's Paris and Egypt homes, while the fifth and sixth explore his collaboration with prestigious director David Lynch.

Louboutin has also recruited some of his favorite models and famous friends to contribute, including Dita Von Teese, Kristen Scott Thomas and Mika.”

The Pill decreases Sexual satisfaction, but Boosts Relationship Happiness

Popping a birth-control pill could significantly impact your long-term relationship choices, new research suggests.

Comparing the relationships of women who were on the pill when they met their partner with those who weren't, researchers found that pill users were less sexually satisfied, but happier overall with their relationships than women not taking birth control when they first met their mate.

"These results are in line with what we and other groups have seen in the lab," study researcher Lisa DeBruine, of the University of Aberdeen, told LiveScience in an e-mail. "Although they're less sexually satisfied, they are MORE satisfied with nonsexual aspects of their relationship, including a partner’s financial provision and support."

Polling the pill

The study, performed by lead author Craig Roberts, of the University of Stirling in the U.K., polled about 2,000 women from all over the world, including the U.S., U.K. and the Czech Republic. The average age of the women was around 37, and to make sure the levels of relationship commitment were comparable, they were asked about their relationships with the fathers of their first children.

The women filled out surveys about their relationships and sex lives. Oddly, these couples who met while the woman was on hormonal birth control were also more emotionally satisfied and were more likely to stay together, on average about two years longer. If those couples did separate, the break-up was more likely to be initiated by the woman.

While the nonsexual parts of their relationship were satisfying, these women showed increasing sexual dissatisfaction during their relationships, while there was no change in nonusers. This increasing sexual dissatisfaction could lead to a tipping point between emotional and financial satisfaction and a desire to be sexually satisfied, the researchers suggest.

Pill use and attraction

Though the biology behind this phenomenon is unknown, DeBruine believes it's most likely related to the hormones found in the pill used to control fertility. "The biological mechanisms are almost certainly linked to hormones," she said. "Hormones influence our bodies and behavior in many ways, so we're still researching exactly how hormones affect mating behavior."

These hormones influence what characteristics women are attracted to in a mate. Studies have shown that a main component of attraction is a desire to find a genetically dissimilar partner, specifically in genes called the major histocompatibility complex (or MHC) that are involved in the body’s immune system. Having a large selection in these genes improves the immune system and can make for healthier offspring in the long run.

When women are on the pill, however, they are in an "eternally pregnant state," meaning they aren’t ovulating and may be wired to instead to seek out genetically similar men — essentially, the genetic equivalent of a relative — because evolutionarily, family would help raise the baby. This desire for genetically similar men was found in a study published in the journal Proceedings of the Royal Society B: Biological Sciences. That same 2008 study found that women not taking hormonal birth control prefer genetically dissimilar men and those with higher testosterone levels and more masculine features.

And supporting the lower sexual satisfaction in birth-control couples, previous research has indicated that females with genetically similar partners express lower sexual satisfaction and have a higher interest in having sex with someone who isn't their partner; essentially, they think about cheating.

Though the new study didn't test genetic similarity, they found that women who had been on a progesterone birth-control pill when their relationship started expressed sexual dissatisfaction and desire to cheat.

Complex relationships

Jonathan Schaffir, a doctor at Ohio State University Medical Center who has studied the link between hormonal contraception and sexual desire but who wasn't involved in this study, suggested that trying to tease out the effect of hormonal birth control on something as complex as mate choice can be misleading.

"It's not likely that a small change in a single biological parameter will make a big difference," noting that the differences the study found, while statistically significant, were small. He said a forward-looking study, or an investigation of possible biological effects of the hormone treatment would have been more convincing. [The History and Future of Birth Control]

"This type of retrospective study cannot fully explain all of the complex processes that shape relationships," DeBruine told LiveScience. "Our findings do suggest that these processes play out slightly differently in women who were and were not using the pill when they met their partner."

Schaffir worries about how women might interpret these findings. "Sometimes people give good forms of birth control a bad name by suggesting that they might have [these kinds of] effects," Schaffir told LiveScience. "I don't want women to have the impression that they shouldn’t use reliable birth control because of dubious psychosocial effects."

The study was published today (Oct. 11) in the journal Proceedings of the Royal Society B: Biological Sciences.

Personal comment: Students at St Lucy’s have always been counseled to use effective methods of non-hormonal of contraception where possible as we feel young women should be able to experience the full range of their femininity available only though the ebb and flow of their natural hormonal cycles. Hormonal contraceptives are only used to treat heavy or painful periods, acne or other conditions where appropriate.

HPV-Related Throat Cancers Increasing, Study Finds

The percentage of throat cancers caused by the human papillomavirus has increased significantly in the U.S. since the 1980s, according to a study in the Journal of Clinical Oncology, the New York Times http://www.nytimes.com/2011/10/04/health/research/04hpv.html?_r=2&ref=health reports. HPV type 16, which also causes many cases of cervical cancer, is sexually transmitted, including during oral sex.

For the study -- which was funded by the National Cancer Institute, Ohio State University and the Oral Cancer Foundation -- researchers tested tumor samples from 271 patients diagnosed with throat cancers between 1984 and 2004. About 16% of samples from the 1980s found strains of HPV, compared with nearly 72% of cases found after 2000. Overall, throat cancers caused by HPV increased from 0.8 cases per 100,000 people in 1988 to 2.6 per 100,000 people in 2004, the study found.

According to the Times, there are fewer than 10,000 cases of throat cancer annually, and most people with HPV do not develop cancer. HPV-related throat cancers are more treatable than those not caused by the virus. People with throat cancers caused by HPV have a median survival of 131 months, compared with 20 months for people whose cancers are not related to the virus (Grady, New York Times, 10/3).

Risk Highest Among Men

The study found that the risk of HPV-related throat cancer was greatest among men, though the researchers did not determine why. An HPV vaccine is approved to protect against cervical cancer in girls and women and to protect against genital warts and anal cancer in both sexes. Maura Gillison, a head-and-neck cancer specialist at Ohio State University and senior author of the study, said protection against oral HPV has not been studied in either gender. A spokesperson for Merck, which makes the HPV vaccine, said the company does not plan to conduct an oral cancer study.

She said that oral cancer always has been a bigger threat to men than women and that women's incidence is steady while men's is rising. The quadrivalent cervical cancer vaccine (Gardasil) is more cost-effective than the bivalent vaccine (Cervarix) if the two are equally priced, concludes a study published on bmj.com today. This is despite the fact that Cervarix may provide better protection against cervical cancer.

However, the authors stress that considerable uncertainty remains about the differential benefit of the two vaccines.

Human papillomavirus (HPV) is a common sexually transmitted infection seen most often in young women and adolescents. There are more than 100 types of HPV - some cause genital warts, but others cause cancers including cervical cancer.

Two HPV vaccines (Cervarix and Gardasil) are currently available. Both protect against HPV types 16 and 18, which cause over 70% of cervical cancer cases, as well as several other types of cancer. Gardasil also protects against types 6 and 11, which cause the majority of genital warts as well as a rare disease called respiratory papillomatosis.

In 2008, the UK government chose the bivalent vaccine Cervarix for its HPV vaccination programme, based partly on analyses by Mark Jit and his colleagues at the Health Protection Agency suggesting that Cervarix would have to be £15 to £23 cheaper per dose to be as cost-effective as Gardasil. However, the choice of vaccine will be re-evaluated when the current tender for the vaccination programme ends.

Since then, further evidence has emerged to differentiate the two vaccines. For instance, Cervarix appears to give better protection against cervical cancer caused by HPV types other than 16 and 18. Gardasil has now also shown protection against vulvar, vaginal and anal cancer.

Using this latest evidence within a mathematical model, Mark Jit and his team set out to re-evaluate the two vaccines to inform the next round of vaccine tendering.

They found the price differential between the two vaccines was larger than their 2008 analysis. Based on the most recent evidence, Cervarix would have to be £19 to £35 cheaper per dose to be as cost-effective as Gardasil, mainly due to a lack of protection against genital warts.

When all differences between the two vaccines are considered, "the quadrivalent vaccine (Gardasil) is still more cost effective if the two are equally priced," conclude the authors. However, they stress that "considerable uncertainty remains about the differential benefit of the two vaccines."

An accompanying editorial says that modeling can help decision makers, but ultimately the tender price is the final determinant of cost-effectiveness.”

Personal comment: St Lucy’s was fortunate that we had good advice when we chose Gardasil as protection against cervical cancer as now it has been found to protect against genital warts as well as vulvar, vaginal and anal cancer. This year all new St Lucy’s students must have completed the series of three Gardasil injections over an interval of six months before they are allowed on campus for orientation. All our male partner/instructors have had the full Gardasil regimen as well to provide maximum protection to themselves and their student partners. Gardasil has recently become a topic of debate among candidates for the Republican nomination for the 2012 presidential election. The conservative candidate Michele Bachmann seems to be very poorly informed and continues to blunder on about mental retardation and the dangers of vaccines. Sigh!

New St Lucy’s entry requirements, Pt II: I was remiss when posting about changes to St Lucy’s entry requirements for the 2011/12 school year. I failed to mention that all entering students must have had at least four years of ballet training consisting of not less than four hours a week for at least fifty weeks per year. We aren’t a ballet academy, but the physical and mental development (poise, grace, discipline, confidence and pelvic muscle development) of the students is so closely tied to ballet training and discipline that any less and a student has a very difficult time keeping up. One might think that this requirement would dissuade parents from enrolling their daughters at St Lucy’s, but that hasn’t been the case as highly qualified applicants far outnumber the seats available for new students. Parents (especially their mothers) are determined to begin preparing their daughters years in advance to get into St Lucy’s.

Pelvic Familiarization for post-menarche students: Because the health syllabus at St Lucy’s is heavily weighted toward teaching female sexual skills we require all post-menarche students to take a pelvic familiarization (PF) course. This course covers menstruation, feminine hygiene products and their proper use, reproductive tract devices for contraception (IUDs, condoms and cervical barriers), dealing with the hymen and sex toys. The course purpose is not only to familiarize the student with the range of products and devices available, but to have her become comfortable and confident about touching her own genitals so she won’t be afraid or embarrassed when she takes Contemporary Sexual Health and Advanced Sexual Techniques. To that end we teach and recommend the students masturbate to learn what arouses them and what doesn’t so they can guide a male partner/instructor when that time arrives.

An anxious student’s first look at her trainer’s equipment

There is no money in that for me, but it gives me an opportunity to scout the sorority women for possible candidates as escorts. It’s a way for an attractive and adventuresome student to make money if she has talent and can get with a good service. However, with the economy like it is the competition for the available opportunities is brutal.

We are training the girls with RBs connected to their gasmasks first. Then when they become confident in that application we will teach them the delights of rebreather bag use during dive sex, which is more complicated. Rebreather bags can be very dangerous since they are used frequently during solo sex and it is easy to get into trouble by trying for a personal best in continuous valve closure during asphyxia play. What we advise is during solo play that the student not shut the valve sealing her rebreather, but put her finger over the vent so that if she faints her hand will drop away and the open valve will allow her to breathe fresh air. Passing out during rebreather solo play with the valve closed has led to a lot of deaths and some that should have died as they ended up in a persistent vegetative state from lack of oxygen to the brain and will be on a ventilator for ever, unless next of kin decided to turn off the ventilator.

Under water rebreather bags: Our way of using an underwater rebreather bag is different from surface rebreathing in that the bag is connected to the exhalation port on a modified FFM that has had the exhalation valve removed and replaced by one at the end of the UW rebreather bag and a manual valve beyond that which can block exhaling into the water. There is also a valve on the air hose to the second stage reg on the FFM (or for added difficulty the tank valve can be used) so that the wearer can shut off her own air.

For normal breathing the valve at the end of the rebreather bag is open so that exhaled air can be released. To activate the rebreather the valve on the end of the rebreather is closed which allows the diver’s exhalation to fill the bag. Then the wearer’s air is shut off by means of the in-line valve or the main tank valve. We prefer a trainee use the in-line valve because shutting off the tank valve prevents buoyancy control by means of inflating her BCD or drysuit a complication a rebreather student doesn’t need at first. Once there is no air through the DV the wearer sucks air from the rebreather bag and the level of carbon dioxide quickly increases to the point where clear thinking is impossible and unconsciousness is imminent. She or her dive buddy has to be ready to restore to her second stage reg before she becomes unconscious. That it’s why we recommend never having solo dive-sex with a FFM fitted with a rebreather bag. There is almost no room to recover from a breathplay mistake while wearing a rebreather bag fitted FFM.

Colored oval crotch gussets: Now that autumn is here (the autumnal equinox was on 9/23) I’m glad to be getting back into fall leggings and convertible tights. I love my custom legwear with its brightly colored oval cotton lined crotch gussets. Contraception at St Lucy’s: As returning readers know the faculty and students at St Lucy’s take contraception very seriously. Using an effective method of birth control correctly permits worry free participation in intimate aspects of the curriculum as well as recreational sex. There is of course the problem of STIs some of which have become life threatening in the last 25 years with the possibility of contracting HIV. We minimize the likelihood of students being exposed to STIs by having the males employed as instructors and lab partners for students tested frequently with full STI panels and since it’s possible that students can have sex with men other than their instructor/partners the girls are tested as well. Rather than male condoms Student Health advises students to use the nitrile FC2 condom while being intimate with men who have unknown sexual histories. The FC2s wide anterior opening offers far better protection for the student’s vulvar tissues than if her partner wears a male condom. Emergency contraceptives, Plan B one step (effective for up to 72 hours after a birth control failure) and ella (effective for up to 120 hours after a birth control failure) are available free of charge to students and female faculty. And if all else fails terminations using Mifeprex or by means of a menstrual extraction are paid for by insurance every female at St Lucy’s has through Student Health.

Sorority Tampon troubles: I was talking with Taryn’s friend [I wrote about her in my entry for April 14, 2011] who is a second year student at UNLV. She said the school’s Student Health found that three members of her sorority’s cosplay club had their menstrual supplies, specifically tampons, tampered with. This is of particular interest to me at the moment because today I’m CD3 and bleeding heavily myself. Of course I’m not using tampons I’m using a diaphragm for flow control and I make certain that my menstrual and contraceptive supplies can’t be tampered with. Tampering with something as intimate as a woman’s menstrual protection is pretty creepy.

The problem they had is that the three members have come down with pelvic infections that are traceable back to the fact that they were menstrual and all using tampons, though there were three different kinds being used. It’s not Toxic Shock Syndrome (TSS) which immediately springs to mind when someone mentions an infection traceable to menstrual products, its chlamydia. Chlamydia is probably the most common STI and is known as a "silent" disease because the majority of infected people have no symptoms and it can be spread by oral, anal or vaginal sex. One of the infected women has multiple partners, but is careful to use condoms and says she had only vaginal sex and was symptom free when being routinely tested for STIs when she tested positive. The two other women are in monogamous relationships. One was having pelvic pain and the other had no symptoms, but both went to be tested when their sorority sister unexpectedly found she had chlamydia.

Because both monogamous couples were in a very early stage in their relationships they were just starting hormonal birth control and waiting for the hormones to build up to an effective level so they were still using condoms and their male partners hadn’t been infected. At that point all the sorority sisters were tested and no one else tested positive. The infected women were treated with a single dose of azithromycin and are recovering. Student Health sent the samples to the pathology lab on campus for testing where it was discovered that the samples that tested positive had a tracking agent in the bacteria showing that the strain of bacterium, Chlamydia trachomatis, had been lab cultured by a biology student in the PhD program indicating that the strain had somehow escaped from the laboratory.

Someone in Student Health thought to test the tampons the infected women were using, perhaps because the men in the women’s lives were free of the infection and the bacteria was home grown and they were looking for something that served as a delivery medium. They tested two partial boxes of tampons; the other woman had finished her partial box, and in both boxes they found several tampons with punctures in the individual wrappers and when they tested the tampons they found the chlamydia bacteria with the marker! So then the question became how someone, almost certainly a woman, got access to the women’s menstrual supplies. They are checking the backgrounds of the cleaning crew from a local maid service the sorority uses occasionally, the lab assistants and have questioned the sorority members individually and haven’t turned up anything that they are willing to discuss. But the intentionally spread infection has increased the member’s paranoia and led to all the bedroom doors being fitted with good locks and being kept locked when their occupants aren’t there. Going a step further the members checked all their other menstrual and contraceptive supplies for tampering and found no indication of tampering.

Birth control pill recall: On 09/15/2011 due to a packaging error Qualitest Pharmaceuticals, a manufacturer of generic BCPs, voluntarily recalled certain lots of Cyclafem, Emoquette, Gildess, Orsythia, Previfem and Tri-Previfem. As a result of this packaging error, the daily regimen for these oral contraceptives may be incorrect and could leave women without adequate contraception, and at risk for unintended pregnancy. I especially like the high quality seams and zippers that don’t pull apart and are easy to open and close regardless of how tight the garment is.

Hoods and AST at St Lucy’s: For reasons of anonymity and safety St Lucy’s students are required to wear hoods during all interaction with their partner/trainers. I’m partial to high quality (Polymorphe) back zip latex hoods for both surface and dive use, but fabric hoods are permitted for surface encounters if they are sufficiently opaque. However, I feel that any student wearing a fabric hood doesn’t have the commitment to BDSM that a St Lucy’s girl should demonstrate if she expects to successfully complete the AST course. That’s because part of a students training is to learn to tolerate the heat that can be generated inside a latex hood during sexual intercourse. I feel it’s an exercise in concentration, to be able to experience the scent of sweat dampened latex as she broils in rubber while her trainer is thrusting into the depths of her tight flat belly and still enjoy an orgasm. In the heated training pools at St Lucy’s where thermal protection isn’t necessary Polymorphe chlorinated latex catsuits, hoods, gloves with weighted silicone pool-pointes and FFMs are worn by students and female instructors for ease of bottom bonding while training with gas-guards and rear entry relief zipper penetration. Once a student becomes comfortable with vanilla dive-sex then we add dive-sex while wearing an inflatable penis gag.

Latex hood and FFM seals: We have chosen the latex seal option for our FFM face seals so that worn sealed against a latex hood the mask’s cavity-rim seal develops very strong suction so that when underwater even w/o the spider tightened the seal vacuum will hold the mask on the diver. Latex seals are not as robust so don’t last as long as silicone ones, but since the school FFMs are only used in training/encounter pools where no grit can pit the surfaces the stronger seal makes them better for our training needs, especially for asphyxia training. On the surface even when the wearer is breathing through the ambient breathing valve and the spider has been loosened the mask sealed against a polished latex hood will cling to the hood until the seal vacuum is broken.

Sizing inflatable penis gags for FFMs: We have been fitting my AST students for inflatable penis gags for use while taking BDSM dive training. It’s more difficult than you might think since the length (as determined by the gag and the pressure in it) can block the wearer’s airway and suffocate her or trigger a gag reflex and cause her to drown in her own vomit. But it is a huge rush to have your gag inflated until it blocks the airway just as you reach orgasm! One of the first things a BDSM dive student must do is to find a decongestant that works well for her w/o any side effects like making her drowsy to ensure her nasal passages are wide open when her gag is inflated. Most of the escorts and students prefer Sudafed.

The gag is inflated by means of a LP-hose with an inflate/deflate valve connected to a modified accessory Port on the FFM so that the gag can be controlled by the diver, her instructor or Dom. That means that her SCUBA must be fitted with an octopus and her FFM must have a vacant accessory port available. The most difficult part of using an FFM mounted inflatable penis gag safely is learning to inflate and deflate the gag as the diver’s depth varies, especially during ascent where if the gag isn’t deflated sufficiently the diver can easily suffocate especially if she has been to considerable depth (below 30 feet - 2 ATA) for a prolonged length of time and needs time at a deco stop.

The scent of ‘ripe’ vaginas: An important part of AST is teaching the students how to change the scent of their vaginas to appeal to the tastes of different men. Things that will change the scent of a healthy vagina are: 1) to wear a cervical barrier, diaphragm or cap, continuously for more than 24 hours. The interaction of vaginal fluids with the device will cause a musky odor to develop. The odor may develop faster and stronger with a latex device than a silicone one because the surface of natural latex is porous. 2) Having a man ejaculate directly in the vagina w/o wearing a condom will change the scent of her vagina. 3) And, man’s diet will change the taste and scent of his semen. Meats and spicy foods and alcohol can make the taste/scent of semen unpleasant while eating sweet fruit such as pineapple can sweeten the taste/scent and semen after he eats asparagus can have a very unpleasant aroma. So a male partner’s diet can contribute to a range of vaginal odors. Of course if a woman’s vagina has a colored and unpleasant smelling discharge she probably has a vaginal infection that needs immediate attention.

She will wear her Penetrator at all times (except to remove it for cleaning) using it to become accustomed to the sensation and strengthen the grip of her vaginal muscles during her training except when she is with a male trainer.

AST and Penetrator plugs: All our AST students at St Lucy’s have been fitted for Penetrator plugs to be worn in the classroom and when not involved in encounters requiring vaginal penetration. The shaft of the Penetrator will allow the student to clench her vaginal muscles gripping the plug to strengthen the bands of muscles surrounding her birth canal. All the students have been training with Ben Wa balls for at least a year so this is a step up in training. Wearing a Penetrator requires that the woman’s pubes be totally nude so we recommend Brazilian waxing (Body Buffers has a salon on campus) but a few students initially prefer to shave or use a depilatory cream. Those students almost always decide waxing is better and simpler before the first semester is over. The plugs anti-expulsion ridge that fits up behind the woman’s pubic bone prevents clenching of her vaginal muscles from expelling the plug so she can really bear down while clenching.

All Penetrator plugs are custom made to the exact dimensions of the woman’s pelvic anatomy. The thickness of her pubic bone, the depth of her introitus, the depth and angle of her vagina when not aroused, the width of her perineal canal, the width and length of her vulva and the size of her clitoris that will be under the grope shield. The Plug is a precision fit so DiveGel+ is used as an intimate lubricant and sealant so that once she becomes used to it the woman can wear her plug comfortably all day if necessary.
