Channeling Giulietta: Returning readers will recall that in my post of September 29 entitled “Clinching the sale” over dinner Ron and I had discovered we were both Moira Shearer fans and talked about the films she was in. In addition to The Red Shoes we both loved the 1951 film The Tales of Hoffman in which she performed two roles; 1) as Stella a ballerina who (costumed as a praying mantis kills her partner) and; 2) Olympia (a doll that comes to life). We also agreed that another favorite - and to me a very empowering - character in the Hoffmann film was the role Ludmilla Tchérina performed as the Venetian courtesan Giulietta, who stole Hoffmann’s soul.  

So I was astounded when Ron gave me the most amazing compliment as he walked me to the car that would take me back to Blackthorn House. He called me Giulietta! I was very pleased and tried not to blush while thinking he was a lot more perceptive than I first thought! He continued to hold my hand after he helped me into the car and before releasing me he put my hand to his lips and kissed it! I gave him one of my most provocative smiles as he closed the door. I was delighted as I thought of the twenty-four hours we spent together, the intimacies we had shared and thought perhaps our encounter had put him in my thrall. But one can never be certain about these things so early in a relationship and with a man in such an exalted position one should proceed slowly if at all, certainly letting him make the first move if he is interested in a relationship. Sophie was still inside so didn’t see his kiss. I certainly don’t want to beguile him so thoroughly that I take him away from Sophie as he needs a young, lovely and accomplished partner who will give him children since the need to continue his line is a must.

Physically I’m not exactly like the Giulietta who stole Hoffman’s soul and turned it into an exquisite necklace. However, I do have a lovely figure with long shapely legs, high cheekbones and long hair. My hair is red not dark and I don’t often dress all in black as Giulietta does, but it suits her as black is suggestive of the black arts at which she is adept. As much as I admired Moira Shearer and Cyd Charisse for the most part I see them as “good girls.” However, when I go clubbing with girlfriends looking for an encounter I think of myself as channeling Giulietta (she of the feminine art of seduction and a distant relative of my Goddess Aphrodite) since she was a powerful enchantress who could steal a man’s soul by just looking at him. So when I’m dressing for a night of clubbing I insert one of my flat spring latex diaphragms with DiveGel+ (a lube, biocide and spermicide) in the dome to ensure I’ll be ready should an opportunity for an encounter with an ardent and marvelously hunky man appear.

Over the years as I became proficient and confident with my pelvic skills I developed the ability to quickly enthrall a man if I develop a physical interest in him as I can cloud a man’s mind if we lock gazes however briefly. I’m told my look suggests exquisite sexual pleasure in the male mind and I find considerable pleasure and power in clouding men’s minds that way. When I was younger I occasionally (unintentionally) put one of the less interesting men in my thrall enrapturing him with a psychological penis leash. Since I gained full control of my powers that hasn’t happened in a long time.    

Sophie and her slit-kit: As agreed, the next day Sophie came to Blackthorn House, my home on Eaton Square in Belgravia to discuss her training equipment. She is in a private car with smoked windows and comes through the Mews entrance into the rear court so she can’t be identified by people in Eaton Square since seeing her as a regular visitor could start rumors. I welcomed her to Blackthorn House and over tea I presented her with a slit kit that I give all my pelvic students and we went over the uses of the items. It contained:

• 2 flat spring latex diaphragms. One for use and one for backup.
• 1 22mm FemCap for flood insurance during shallow (less than 25 feet) dives.
• 5 10ml prefilled vaginal applicators of silicone based DiveGel+ to be used as a vaginal lube for ballet and dive-sex.
• 4 each of 4 sizes of ultra-sheer latex condoms if her partner askes to use one.
• 6 tampons in her size (Playtex Sport regular) in applicators used to absorb residual discharge after Kegeling coitial discharge if her costume is too revealing to wear a pad.
• 2 packets of hygienic hand wipes for menstrual and coitial discharge.
• 3 pairs of Nitrile exam gloves in case a man who has been eating hot wings with his fingers wants to give a pelvic exam w/o washing his hands.
• 20 25mg sildenafil citrate (Viagra the “Pfizer riser”) tablets. 50mg is the normal dose one hour before sex in case her partner(s) need a performance enhancer.
• 2 1.0-inch diameter sets of Ben Wa balls. One a solid borosilicate glass set weighs a total of 1.3 oz. while the second a solid surgical stainless steel set weighs a total of 4.3 oz. Both are used for vaginal muscle training.
• A place for her Sports Plug for which she had been fitted when she got her diaphragms at my Gyn clinic. The Sports Plug is used primarily to prevent vaginal penetration when being groped, while out with rich men. Like Donald Trump was talking about on the video recently introduced into the U.S. presidential race.

Latex catsuits: Sophie asked me about wearing latex other than her diaphragm, specifically latex catsuits as Ron had mentioned that he thought she would look stunning in one. She saw my latex closet when I was showing her around Blackthorn House and admired my collection. I certainly agree as she has a wonderfully toned athletic figure, a bit curvier than a Balanchine ballerina body, but she has kept in excellent shape by all the ballet training she takes. So we agreed that I would take her to my fave custom latex shop to be fitted for several catsuits if she meets the training schedule we agreed upon to prepare her for Pompoir.

At first I thought I had made a mistake in showing her some of my latex collection. However, rethinking the situation I’m glad she saw it as getting fitted for a latex catsuit is just the thing to increase her incentive – though I don’t think she needed it - to meet or better the schedule we set. When she is properly fitted for a latex catsuit I’ll have her take pointe class with me while we are wearing full latex. That is a huge stamina builder if you begin slowly and stay well hydrated with sports drinks that replace electrolytes as the sweat pours out of the wrist and ankle openings and soaks the pointes so that’s another reason to wear Gaynor pointes as they retain their shape and support when wet and sueded tips minimize the likelihood of slipping on sweat dampened Marley floors.

Hurricane Matthew: For returning readers who recall that I have a winter get-away in the BVIs – unused in the past year because of the Zika virus - I’ll just mention that as of Monday October 11^th Virgin Gorda where I have a villa on Lost Cove was fortunate to have had Matthew pass to the south and west a few days ago so we avoided major damage from a very large storm. I feel so sorry for the people in Haiti and in the U.S. living on the southern east coast who got battered quite hard.

The perils of Gas Fisting: Gas Fisting for the purpose of this post is defined as having compressed air intentionally forced into the vagina of a willing (or unwilling) woman. As far as I know being Gas Fisted is very rare and extremely dangerous which probably explains its rarity. I’m writing about it now because one of my students asked me about it today in my class on Advanced Sexual Techniques. Her boyfriend wanted to do it to her telling her that it was a quick way to flush coital discharge from her vagina. She asked if a diaphragm being used for contraception or as ‘flood protection’, to prevent the hydraulics of a partners thrusting from forcing water into her uterus, would provide effective protection if he forced pressurized gas into her. The short answer is no, it will not!

No currently available cervical barrier or FC2 condom will provide any protection if a woman is Gas Fisted. The few incidents I’m familiar with have all occurred where the woman is in a bikini or beavertail jacket and SCUBA gear where there was easy access to her vulva and a source of compressed gas from a buddy reg on an octopus or her partner’s reg was available and have all occurred w/o the victim’s permission.

Present Barrier protection is ineffective: Readers of my posts will recall that I routinely use the latex Reflexions flat spring contraceptive diaphragm as effective ‘flood insurance’ while diving to prevent water or gas bubbles from being forced into my uterus by the hydraulics of a partner’s thrusting. Neither the Reflexions nor any other cervical barrier currently commercially available will protect a woman against serious injury if high pressure gas is released into her vagina.

That’s because a jet of compressed gas from a 2^nd stage reg will stretch the vaginal rugae inflating the vagina like a balloon causing the bladder to immediately empty and occasionally the bowels as well. Worse however is that the nearly instantaneously inflated vagina will break even the strongest seal of any diaphragm or FemCap being worn for protection because diaphragm rims and FemCap brims form their seals by pressing tightly against the vaginal walls. The seal of a cervical barrier being breached allows the gas and any water it is pushing to be forced into the wearer’s uterus and fallopian tubes where it could cause PID and infertility or a gas bubble could possibly enter the bloodstream if she was menstrual or recently had an abortion or given birth.

Effective protection: The only cervical barriers that will provide effective protection from the serious side effects of Gas Fisting are the latex Prentif cavity rim cap and the much smaller transparent silicone Oves cap because they suction directly on the walls of the cervix so external pressure will force them down tighter on the cervix.  Unfortunately, neither Prentif nor Oves are still being made. However, there are a few of us still using a Prentif or Oves who can survive a mischievous partner Gas Fisting us.

Too, for a very few of us wanting to tempt fate we can screw a Prentif or Oves down tight on our cervix and have a lover intentionally Gas Fist us or do it ourselves to prove to we still have the nerve or luck to survive.  There is no accounting for the extent to which some of us will go for thrills. Experiencing the massive high that sort of ‘cheating death’ recklessness brings when I successfully survive an intentional Gas Fisting leaves me boneless and gasping savoring how marvelously alive I feel!  

Latex Prentif cavity rim cervical caps like the ones shown above are the only devices other than Oves that will provide effective protection for a woman who is Gas Fisted.

Lesbians and FemCap: Our clinic fits quite a few Lesbian ballet dancers with FemCap, a transparent silicone contraceptive device the brim of which seals against the vaginal walls. Since I joined the clinic at the first of the year I’ve been doing most of our Lesbian patient’s contraceptive barrier fittings and teaching them insertion and removal techniques.  They are very straightforward; for women new to cervical barriers I always have them first insert two fingers to feel how the vagina s slopes up and then back and have them feel their cervix so they know what it feels like and how far in it is when not aroused. That way they have an idea about how to push the folded cap down the vagina toward their cervix. Then compress the brim with one hand and spread the labia with the other and insert the cap with the concave side of the dome up. FemCap is pushed along the back wall of the vagina and when it hits the cervix the brim is tilted up with the fingers and should slip into the posterior fornix with the dome over the cervix. Once on the cervix a finger sweep all around the cervix and feeling it through the dome ensures the cap is properly in place. At that point the removal strap is pushed down forcing the base of the cap as deep as it will go in the fornices and should be held there for ten seconds while a strong suction is developed.

 

Some users new to FemCap panic when they try to remove it the first few times and find that even when depressing the dome enough of the vacuum isn’t released to remove the cap by tugging on the removal strap. While that can be initially disconcerting it shows how well the cap fits since with suction that strong it is very unlikely to be dislodged. For women who can’t release enough suction to comfortably withdraw the cap a long handled ice tea spoon can be used as a removal aid. With the convex side of the bowl against the vaginal wall slip it between the wall and the brim. Then push the bowl of the spoon into the anterior fornix and twist the handle to break the suction between the brim and the vaginal wall. Lesbians who don’t need contraceptive protection can wear FemCaps dry, no spermicide in the dome, and the cap can be safely worn continuously for at least forty-eight hours though I’ve worn mine for four days with no ill effects.  FemCap shouldn’t be worn while menstruating as the flow can float it off the cervix.

 

Most of our cervical barrier fittings for Lesbians are for FemCap, but a few are for latex Reflexions flat spring diaphragms if the patient needs dive protection for underwater dildo play at depths greater than 25 feet. Below that the pressure squeeze on the FemCap dome becomes very uncomfortable. Now that the Reflexions is no longer being made our clinic is having our own latex flat spring diaphragms produced as there is still a small but pressing need for that style rim and material. FemCap is preferred for penetrative surface sex protection and shallow dive sex by Lesbians because it allows the skin-on-skin fingering of the wearer’s G-spot and as I mentioned above when a vibrator head is pushed against the FemCap removal strap and dome the entire device vibrates gently and the impulses are transmitted to the wearer’s A-spot and P-spots simultaneously which almost always generate powerful orgasms

.

Lesbian Dommes and the need for contraception: Most Lesbians who come to us are cycling naturally and enjoy their cyclical hormonal swings.  Spermicide is not needed when FemCap is used in a pure Lesbian Domme/submissive relationship. However, there are instances when a spermicide should be used. One fairly common example is when the Domme has the hots for a Bi-sub - who is on the pill or a LARC - and having sex with unprotected males. In that situation the Domme needs to use an effective spermicide (usually ContraGel with lactic acid as the active ingredient) to protect herself if playing with the Bi-sub when she and her partners fingers are in each other’s vaginas while her Bi-sub is still draining fresh semen.

 

You would be surprised by how many gorgeous Bi-sub ballerinas there are who are as comfortable with bedding a marvelously virile man as sleeping with a hundred and ten pound Domme and taking them quickly one after the other. Since it is impossible to know for sure if the sperm in coitial discharge is dead to be safe a Bi-sub should only be played with while wearing an effective barrier contraceptive with spermicide in the dome since transferring motile sperm from one vagina to another with the fingers resulting in the pregnancy of the woman who did not have penile/vaginal intercourse is quite possible.

 

Sizing: Most Lesbians take the smallest FemCap size, a 22 mm inner diameter designed for women who have never been pregnant although some few take the larger sizes 26 mm if they have been pregnant, but have not delivered vaginally (which is my size though I’ve never been preggers) or the 30 mm if they have delivered vaginally. And we do have one or two patients who have found they are primarily Lesbian after being in unsatisfactory traditional heterosexual marriages and having children.

  

Comfort and uterine protection: Since most Lesbians don’t have to worry about the discomfort of a large male partner hitting the removal strap they can comfortably and confidently wear a FemCap for safety since it is important during cunilingus to prevent air blown into the vagina from entering the uterus and possibly causing an air embolism. This is especially true during dive-sex when a playful partner could use her buddy reg to push against her partner’s labia and hit purge sending a jet of compressed air into her partner’s vagina, through her cervix and into her uterus, unless she is wearing a well suctioning cap. And FemCap performs a cervix thrust-buffering function when a dildo is being used on the wearer by her partner by substantially lessening the discomfort of the dildo ramming her cervix.

The weather: It’s been raining and cold, around freezing, for the last few days with occasional snow flurries so the Barony’s Land Rovers with their all terrain tires and large heaters are a very comfortable and safe ride for Charles, my Chamberlain of the Household, and me to travel in to the Priory and while making the rounds of shops in the nearby villages to introduce myself. 

Priory girls training: Returning readers may recall what I wrote about Blackthorn Priory in my December-31, 2014 post entitled ‘My Welsh Roots’: “There is nothing to suggest St. Candice (as I tend to think of her) was in any way a disciplinarian. However, she is patron saint of a religious community of women living in a surprisingly large medieval Priory the early parts of which date from the 11^th centaury.  The Priory is on Blackthorn Barony land within a few miles of the Baroness’s relatively small but superbly constructed 13^th C. castle…”

“The Priory is part of the Barony of Blackthorn and the office of Prioress of the Order is in the gift of the Baroness Blackthorn. Maintenance of the priory and the Order are paid for from a small percentage of the profit from the minerals mined on the Baronial estates. The Order of St Canna stresses discipline, obedience and service.  Blackthorn priory’s primary function seems to be to raise female foundlings selectively chosen from the slums of major cities in the UK.  Their schools taught by Sisters who have attended the best universities, prepare the girls for university educations and socialize the young girls through exchange programs with other elite public (read private) schools in the UK.”

Pelvic exams, diaphragms and dildos: It is the custom at Blackthorn as with many girls’ schools to give Priory girls reaching menarche their first gynecological exam and fit them with an effective barrier contraceptive so they can become accustomed to wearing it before they move to fifth and sixth forms at Barrow Priory in Middlesex where they will need it during social encounters as guests at dances at a neighboring boys school.

The primary barrier currently in use by Priory girls is the silicone Semina diaphragm (pictured at the top of this post)  which the girls think looks like a pink soap bubble a fact that helps the girls remember to insert their diaphragms for flow control on light days during menses. The smallest Semina is 60mm so any student requiring a smaller device is fitted with a 22mm FemCap and a menstrual cup for flow control. There were four girls who had reached menarche in the last several months and Chris, my Male Gyn and new member of the Blackthorn Clinic, gave them their pelvic exams. All had deep post-pubic vaults, took 60mm Seminas and none needed a hymenectomy.

Some readers may wonder why students reaching menarche are not immediately placed on hormonal contraceptives since these days so many girls routinely go on contraceptive hormones with all the negative side effects accompanying them. Some few Blackthorn girls are put on hormones if they have very painful periods or their cycles won’t stabilize. However, the Order feels that it is important for young women to experience the ebb and flow of their hormones while cycling naturally. Diaphragms let them become accustomed to handling their genitals w/o fear or shame as so many young women are taught and can experience the marvelous surge in creativity when fertile. Additionally, the discipline of inserting the diaphragm every evening before bed and removing and cleaning it each morning gets them into a routine that will be beneficial when they become sexually active.

I fitted them with their Seminas and Diva menstrual cups and discussed the insertion and removal procedure for their barriers and Diva cups then let them practice insertion and removal of both devices. Fortunately, in my opinion, none required a FemCap since a cap shouldn’t be used during menses and it has depth limitations during dive-sex.

Then in my first presentations as Baroness I presented each newly blossomed young woman with two dildos: her first borosilicate (Pyrex) glass dildo, a 1 ¾ inches in diameter and 8.0 inches long glass shaft in the shape of a phallus with a handle to use while on her back – a nice feature about Pyrex is that it can be warmed or cooled for use in hot or chilly weather. And a high quality silicone one of the same size with a suction cup base with which to solo train for male penetration in the ballet studio. This second item is especially useful in training for deep penetration of the anterior and posterior fornices while en pointe during ballet-sex – suctioned on to the mirrored wall in a studio where the user can hold on to the barre rise en pointe and back on to the dildo. All the girls were eagerly anticipating trying them out.

P.T. with the first of Marvin’s patients: I’m looking forward to the phys. therapy sessions with my first of Marvin’s patients who will be here this coming Monday. That will give the security group time to finish installing six HD cams in the castle ballet studio where the video will be stored on disks and satellite linked to Marvin’s office in London. If the videos are any good we may pixelize the faces and sell them on the porn market through Taryn’s org.

Maya arrives: His Grace accompanied Maya, his recent Russian ballerina bedmate, when she arrived this morning in his helicopter. He stayed for lunch and a brief tour of the castle before leaving to get ahead of the rain which appeared later this afternoon.  Chris has already set up a Gyn exam and procedure room in one of the unused interior rooms and fully stocked it with supplies from the Blackthorn Clinic in London.  He removed Maya’s Medusa silver and gold IUD, performed a vacuum aspiration to remove the reproducing tissue and inserted a frameless six bead copper GyneFix IUD this afternoon. Everything went well and she is on a ten day course of an antibiotic and was told no vaginal sex or heavy lifting for a week so she is looking forward to a restful few days of barre, reading and catching up on her sleep.

Edith’s accident: I hadn’t asked exactly how my great, great, great grandmother died. At 103 I thought ‘natural causes’ which for someone that age could have been a variety of ailments. However, when Emily, my Lady’s Maid, mentioned her mistress’s accident I asked what sort thinking it was a fall from tripping on a rug and hitting her head or some such thing. That wasn’t it at all. Emily found her previous mistress after she heard the Baroness moans and screams during orgasm suddenly stop while on a routine morning ride of her Sybian machine. She was found slumped forward with the top of her head on the floor while still pinned to the machine. Her doctor, from the local NHS practice, said it was a heart attack from the exertion and she died almost instantly. I should be so lucky being fucked to death when my time comes.

I wondered how she had managed her vaginal preparation for a machine fucking, but then remembered the hormone replacement therapy med pills and estrogen creams I saw when we went through her meds. Emily said she had no problems with dryness and was comfortable, relaxed and seemed rejuvenated by her daily ride.

New product lines: I’ve been working with my contacts in Vegas to establish commercial ties here with some of the Barony’s commercial ventures. Having a presence for Gepettos, ballet-boots and Labia Labs vaginal and fetishwear products is a must if we are to appeal to the up-market sexually liberated woman and ballet and rubber fetishists. So far I’ve been able to get the following companies to sell their products exclusively in my shops.

Labia Labs in the UK: I’ve gotten Jeff to agree that Labia Labs fitted devices such as sports plugs and Fet-tex rubber fetishwear will be licensed to Blackthorn Holdings Ltd as sole distributor for its vaginal and fetishwear products in the UK. Chris, my male Gyn, can fit vaginal devices and the shop assistants will be fitting Fet-tex catsuits at Blackthorn Ballet and Fetishwear.

Gepetto’s Ballet Boots in the UK: Gepetto will be sending his 26 y/o daughter Cleo to work at Blackthorn Ballet shoes and Fetishwear to fit his custom titanium unitized toe-box and shank leather ballet-boots. Cleo has been working in Gepetto’s Vegas shop for five years and is an expert fitter. The cost $2500 USD (about 1655 GBP) for a basic pair will eliminate wannabe clients and limit customers to dedicated fetishists and fashionistas.

Cervical barriers and menstrual cups: Wide seal silicone Milex Arcing spring and Omniflex (coil spring) rim diaphragms and Lamberts latex Reflexions flat spring diaphragms as well as FemCaps and several styles of menstrual cups are readily available in the UK and can already be fitted and purchased through the Blackstone Clinic.

It’s complicated: Marvin and I have been discussing my feeling of isolation from where the action is as I’m finding having nothing meaningful to do other than my ballet technique and pointe classes makes time seem to drag. Honestly, I need sexual stimulation and being in a castle, as lovely as it is – and I intend to make it even better - several hours by train from London is already getting on my nerves and I’ve been here less than a week!   I can become involved with the local woman’s clinic and will help the gardeners and cook revive the kitchen garden in the spring, but what I really need is male ballet partners to maintain my level of training and, quite frankly my need for sex.

 

 

More contraceptive sabotage, Seminarian training

So, my breasts are sore because I’m pregnant?

I just lift my sweat or tee or unbutton my blouse w/o having to unfasten the hooks in the back and pull the cups out of the way. And the support for ballet and thrust-drop training is marvelous with hardly a giggle.

Halloween contraceptive sabotage: I found out after the fact that Adolph had been lacing the six mistress’s food with ella during their stay at The Lorelei for the ten day training session prior to his Halloween Ballet assignation party. He admitted it to me when he said after the party “well I wonder if any of the women will become pregnant”. I said “they all were being very careful about their birth control given that they knew what was going to happen during the party.” He smiled and said, “Yes, but the ones on hormonal contraception might find their method wasn’t as affective as they would like”. I’m thinking, “OMG!” And asked, “What did you do? He said he had them dosed with ulipristal acetate, a progesterone agonist/antagonist that binds to the woman’s progestin receptors. It’s used in 30mg pills in the U.S. as the EC drug ella.

None of the women mentioned feeling a bit off or the food tasting strange which could have been from ingesting the drug during their training and I suppose the first indication that something happened is if one or more has a heavy period which would indicate she had ovulated but wasn’t pregnant. So we waited to see if any of the six became preggers. In the 12 days total they were here all of them had been with the six escorts training them and a minimum of three or more men at the party so of the nine or more men whose semen might have reached their tubes any of their partners could be responsible for any pregnancy. We found out the other day that one is pregnant and another miscarried. This sort of thing is the reason that a frameless copper bead GyneFix IUD is such a good option for a woman who is sexually active among mischievous men.

Still more contraceptive sabotage: The Swan twins have been saying for weeks that someone is lacing women’s drinks at fraternity parties with something that can significantly decrease the effectiveness of hormonal contraceptives which most of the undergrad women are on. And if a woman was already on a diet pill that increased her metabolism and therefore reduced her protection the woman might have very little hormonal contraceptive protection.  The twins really hadn’t paid that much attention before since they have copper IUDs implanted, but now that they are on Yaz to increase breast size it’s more of a worry. Chris. Our male Gyn, said that whatever is being used probably binds to a woman’s progestin receptors making it impossible for the progestin in her hormonal contraceptive to protect her, it’s the estrogen in the pills that the twins need for breast size. So in that respect they are ok as they are still protected by the copper ions from their GyneFix IUDs. However, if they ovulate on Yaz because of something they ingested they will be back to heavy copper IUD periods again and that hasn’t happened yet.

A Brit student friend of Willow’s is on a progestin only pill (POP) Cerazette: 0.075mg of the progestin Desogestrel - which metabolizes into etonogestrel used in NuvaRing and the implant Nexplanon. Because of the long half-life of etonogestrel Cerazette can be taken as long as 12 hours late and still provide effective protection so for a POP it is a great oral contraceptive.

However, Willow’s friend is also on the migraine med Topamax which will cause birth defects when taken while pregnant. And after not feeling quite right and being nauseous in the mornings for several days she tested and found she was pregnant. Knowing about the birth defects caused by Topamax and in no position to care for a baby she asked Willow to contact Chris about a vacuum aspiration as she didn’t want to go through Student Health since her parents would find out.  In the blood work done to validate the urine pregnancy test ulipristal acetate (a progesterone agonist/antagonist that binds to the taker’s progestin receptors) was found in her blood which, since she was six weeks preggers, meant that she was still ingesting the chemical at parties.

It is fairly easy to get Ulipristal acetate 30 mg Tablets (ella in the U.S and ellaOne in Europe) clandestinely and it is commonly used by mischievous men to decrease the effectiveness of their partner’s hormonal contraceptives.

A GyneFix is implanted: Willow sat with her friend while she was in the stirrups as Chris inserted a cannula and sucked out the reproductive tissue. And while she was there he implanted a six bead GyneFix frameless copper IUD so she should have very effective contraceptive protection for several years.

Bea as a Temptress: At seventeen Bea is coming into her own as a Temptress. Contraception prep for college

Oh my God! I think I’m expelling my Skyla!

The photo: Ladies, you know the feeling when something in there moves that shouldn’t; a diaphragm, FemCap, menstrual cup or an IUD. The Skyla is a smaller version of the Mirena hormone releasing IUD and was designed for nulliparous women. Skyla releases the progestin levonorgestrel at a rate of 14 mcg/day after 24 days and declines to 5 mcg/day after 3 years at which time it must be replaced.  Menstrual cups can usually be used safely with IUDs as the cup should be worn low in the vagina where it should not come in contact with the IUD strings. Of course in the case of my wards their GyneFix copper bead IUDs were implanted w/o strings so they can wear diaphragms or FemCap with no fear of interacting with the strings and causing an expulsion.

Student’s contraception prep for college: While most teens starting college are on hormonal contraceptives a few use the copper ParaGard IUD and a few still use cervical barriers, a silicone Milex diaphragm or a FemCap. For readers who may not be aware of it the silicone Ortho All-Flex diaphragm that had been available for years by Rx through most pharmacies in the U.S. was discontinued in December of 2013. That leaves only the silicone CooperSurgical Milex- in two rim styles arcing and coil spring – and FemCap as options for women in the U.S who want or need a cervical barrier or the latex Reflexions flat spring which can be ordered from the UK..

This past week we have seen the usual summer increase in teen cervical barrier fittings as they prepare to go away to college for the first time or are having their annual well-woman pelvic exam and diaphragm fit check before returning to sexual activity with college men and some professors.  I fit three 17 y/o patients with 60mm wide seal Milex Omniflex (coil spring) diaphragms. They were the first diaphragms for all three because of health problems after having used only condoms.

One girl has Factor V Leiden a mutation of one of the clotting factors in the blood called factor V. This mutation can increase the chance of developing abnormal blood clots made worse by taking anything with estrogen in it. Factor V Leiden while it can appear in men is primarily found in women. The second, diaphragm patient was a petite gymnast with a full athletic scholarship to a major Midwestern university. She has terrible side effects from the progestin Levonorgestrel in the Skyla IUD that was designed for use by small women and she is too small to have ParaGard inserted. And the third girl does not want to use hormonal birth control of any sort and has a nickel/copper allergy so can’t use a ParaGard IUD.

Their moms came with them and were comfortable that while their daughters ages were below the age of consent for some relationships they were open about sex with boyfriend and the mothers only concern was that their daughters would be well protected by a method under their control. The girls assured me they would wear their diaphragms whenever they were around men as sex can often be spontaneous and the need for effective protection is very important to all of them. The girls had no problem inserting and removing the Milex Omniflex as they all use Diva menstrual cups so were familiar with their pelvic anatomy and folding a device for insertion. They see wearing their diaphragms as a rite of passage and an act of independence accepting the responsibility for managing their own fertility and being able to enjoy the delights of sex with a man while reducing the likelihood of unintended consequences to an acceptable level.

I fit another 17 y/o with a 22mm FemCap as her post-pubic vault was far too shallow to enable her to effectively use even a Reflexions flat spring diaphragm. She and her mom did not want her using anything with hormones in it and they both think IUDs are too invasive so a cervical cap was the only option left for a method over which she will have control. The good news is that for her age she is tall and has a relatively long vagina when not aroused – so a partner is unlikely to hit the removal strap during deep thrusting - and she also has long fingers which allow her to reach her cervix when squatting and pushing down, an ability needed to place a FemCap effectively and equally important to be able to reach it to break the seal for removal.

Elbow hinge arcing spring Vs. latex flat spring: Another patient, a returning UNLV grad student, came in to discuss changing the style of diaphragm she is using. She was last fitted with 70mm latex Reflexions with which she was very pleased. However, she has developed a latex allergy and she wanted something that would provide the same protection and pleasure for her and her very well endowed partner during deep thrusting encounters. What I recommended was a Milex wide seal arching spring which has two elbow hinges that fold for insertion. Once inserted and open when thrust into by a penis the rim flexes only slightly remaining circular and stiff (as long as the device does not rotate significantly in either direction), but not as inflexible as the Reflexions allowing the silicone dome to be stretched across the cervix and glans as it is pushed into her anterior fornix by a thrusting penis.  

There are tradeoffs using a silicone arcing spring instead of a latex flat spring. However, for anterior fornix thrusting I think the two can be equally pleasurable. With the silicone arcing spring the dome is stiffer than the flat spring so the rim flexing is offset by the less stretchy dome which puts the same or perhaps a bit more pressure on the tip of the cervix to push the uterus up and to squeeze the glans simultaneously during maximum anterior fornix thrust depth.  Too, the central dome logo on the Reflexions used as a stimulator for men long enough to reach it is replaced by the extrusion bump in the center of the Milex dome that is easily felt by a thrusting penis when the silicone dome is stretched into the anterior fornix.  The only detracting factors in comparing the Milex to Reflexions is that that Milex costs more and the heat transfer properties are much better with latex. Factors against the latex Reflexions are that couples with latex allergies can’t use it. Latex is more easily destroyed by oily lubes or meds so its useful life is much shorter and latex is more likely to contribute to infections than is silicone. The only major difference in favor of the flat spring rim is that it can be worn effectively by some women with shallow post-pubic vaults. 

The only unknowns with the Milex Arcing spring diaphragm used for deep thrust anterior fornix play is how much the diaphragm will rotate which can affect the pressure deep thrusting will develop and how well the elbow hinges will withstand the reverse pressure when the dome is stretched into the wearer’s anterior fornix by a well endowed partner. The good news is that if the elbow hinges begin to fail a replacement is easily obtained.

Breeding dancers: I was going to title this section Breeding Ballerinas for the alliteration, but while I have helped some of my ballet dancer friends conceive most of the women who come to me for help getting pregnant are statuesque full figured show girls 5’10” or taller so are well over 6” in heels or en pointe; lovely but not with ballet bodies, technique or speed necessary for classical ballet.

My method has been fairly successful and is low cost, fun to use and counter intuitive as a conception aid. I have the friends I’m counseling use a silicone Milex wide seal Omniflex (coil spring rim) breeding diaphragm – one that is two sizes smaller that the size the woman should use for contraception – and have the couple lubricate the dome and vagina with pre-seed, a pH balanced sperm friendly gel, rather than acidic spermicide. This allows a partner to easily under-thrust the rim of the diaphragm especially when the woman is being penetrated from behind while bent over holding on to a barre, chair or table or on her hands and knees.

In that position with the trunk of her body parallel to the floor the uterus is pulled down and back by gravity increasing the depth of the vagina. As the vagina lengthens the rim of the diaphragm in the posterior fornix will be pulled deeper with the cervix which in turn pulls the anterior rim further away from the pubic bone where it can be more easily under-thrust by the smooth upper side of her lover’s glans. Once under the dome the frenulum of an uncut man can freely caress his lover’s G-spot bringing her to orgasm with its strong vaginal contractions that squeeze the thick cream rich with millions of sperm from his vas glands where it spews against her ripe unprotected cervix. Once her breeding partner withdraws the diaphragm reseals against the vaginal walls trapping the ejaculate in the prepared dome along with her fertile cervical mucus.

Not that the Reflexions and FemCap she used during sex with other seminar participants couldn’t have leaked, but both the FemCap and Reflexions fit her very well and developed strong suction in their domes so it is unlikely.

She said DH had wanted a baby and so they had been TTC for some time so the excitement of the seminar and different partners was probably the stimulation they needed. But to me she didn’t sound all that pleased so I wouldn’t be surprised if she miscarried as so often happens during the first trimester. She didn’t seem the maternal sort and seemed to crave the excitement of sex with different men. She actually glowed after her sexual encounters with the other Gyns in the seminar. I understand the thrill she felt as I’m that way myself.

Morning Wood:  On mornings when I have slept over I Kegel out any of Marvin’s remaining spend from the previous evening and empty and wash the Reflexions and reinsert it with DiveGel+ spermicide in the dome. Topical testosterone dangers:  Among the escort candidates our medical staff has begun allowing the use of topical Testosterone products like Axiron and AndroGel, but primarily Axiron. This has complicated their training to an extent as their female trainers have to know who is using topical-T and be sure to avoid the areas (shoulders and under arms) on the men where topical-T has been applied.  Actually what we do is get printouts of the men on topical-T  and have the guy shower where we can watch and make sure he washes the area where he applied the testosterone product.  But some guys on Axiron do raise wood quicker and for longer too, than some of the other trainees. They aren’t necessarily better with their technique but they are really hard!

Topical-T is also becoming a problem in mini-clubs at Naughty Pleasures where men can at times go shirtless like the Boot-Bar and Toe-tape.  So now the bouncers route men to a health-care worker while the women have their bags scanned to swab the men’s shoulders and under-arms and test for topical-T at the door. If a male guest has it on his skin he has to wear a shirt and a different colored bracelet. Side effects for women who are exposed to topical-T are: Increased clitoris growth, breast growth, increased acne and facial hair growth among other things. It’s something that we really don’t want nubile teen girls and up-market escorts exposed to.

If I’m wearing Gaynors I can usually just not wear toe pads and be comfortable enough. Water pressure will almost always stop menstrual flow while swimming or diving. However, after coming out of the water there can be a gush that if it’s on a heavy day and she’s been in the water a long time could overload the dome and spill.
