NuvaRing the vaginal contraceptive ring

Lost and found in the sheets: Housekeeping found a NuvaRing in the sheets of one of my New Years house party women guests’ room. She had either expelled it or a lover had hooked it with the corona of his glans and accidently pulled it out. Obviously she was unaware that it was no longer inside and providing protection. When he withdrew she should have immediately checked to see that the ring was still in place and perhaps she did. If she rolled over for a post coital nap and passed gas if she doesn’t have a pronounced post-pubic vault with the back of the pubic bone for the ring to rest against her pelvic contractions could have easily squeezed the ring and it slipped out. I knew who was assigned the room so I called her and mentioned a ring of hers had been found. At first she thought I was talking about a finger ring and assured me she had checked all her jewelry was packed before she left the castle. Then I was a bit more specific that it was a vaginal contraceptive ring.

 

She gasped and there was a noise as she tried to catch her cell as it slipped from her fingers. Moments later she picked it up and in a shaky voice said her ring was missing. Case solved! NuvaRing has an outer diameter of 54 mm and a cross-sectional diameter of 4 mm. and releases on average 0.12 mg/day of etonogestrel and 0.015 mg/day of ethinyl estradiol.  It had been out for much longer than the three hour removal limit during the first two weeks so she will have to use another method (condoms or a diaphragm) for 7 days while the hormones from the new ring reestablish her hormonal protection. She wasn’t sleeping with her husband. It depends on what night she lost the ring as to how serious her situation is. She should have been protected if she lost the ring on the last night. She is newly married and wants children, but not with possible paternity questions.

 

Blackthorn Priory girls training an update: Now that the holidays are over I was able to get back to taking care of my other responsibilities around the Barony one of which is tending to the sexual health of the girls school at Blackthorn Priory. I visited briefly at Christmas to distribute small gifts to girl each as none the students has family or home other than the sisters of the order and her schoolmates.

 

Returning readers may recall what I wrote about Blackthorn Priory in my December 31, 2014 post entitled ‘My Welsh Roots’: “There is nothing to suggest St. Candice (as I tend to think of her) was in any way a disciplinarian. However, she is patron saint of a religious community of women living in a surprisingly large medieval Priory the early parts of which date from the 11^th century.  The Priory is on Blackthorn Barony land within a few miles of the Baroness’s relatively small but superbly constructed 13^th C. castle…”

Pelvic exams and diaphragms: It is the custom at Blackthorn Priory as with many girls’ schools to give Priory girls reaching menarche their first gynecological exam and fit them with an effective barrier contraceptive so they can become accustomed to wearing it before they move to fifth and sixth forms at Barrow Priory in Middlesex where they will need it during social encounters as guests at dances at a neighboring boys school. Yesterday Chris, the male Gyn who works in my woman’s clinic, and I fitted all the students 14 and older with new FS latex diaphragms.

 

The primary training barrier (fitted at menarche) in use by Priory girls is the silicone Semina diaphragm which the girls think looks like a pink soap bubble a fact that helps the girls remember to insert their diaphragms for flow control on light days during menses. However, the smallest Semina is 60mm so any student requiring a smaller device had been fitted with a 22mm FemCap and a menstrual cup for flow control. Now that the Barony’s women’s clinic has flat spring rim diaphragms down to 50mm I have changed the regimen for girls 14 y/o and older to use latex flat spring diaphragms if they are not allergic to latex and any younger girls who at menarche need a diaphragm smaller than 60mm. A flat spring rim seals as well and is nearly impossible to under-thrust and the heat transfer properties of latex are far superior to the silicone of a Semina and silicone lube can be used with a latex D something that has become more important recently. And for divers a FS latex diaphragm can be safely worn at any depth. So wearing a latex diaphragm has become another rite-of-passage on the way to becoming a woman with all the responsibilities that entails. However, there is the fact that oily lubes of any sort, petroleum or vegetable, shouldn’t be used with a latex device as oil will destroy the latex, but that is a lesson best learned when young, impressionable and under supervision so it won’t be easily be forgotten or ignored.

 

Of course because of the age of consent the diaphragms provided to students younger than 16 are so they can become used to correctly wearing a cervical barrier using it exclusively as a menstrual cup for sanitary purposes.

 

Some readers may wonder why students reaching menarche are not immediately placed on hormonal contraceptives since these days so many girls routinely go on contraceptive hormones with all the negative side effects accompanying them. Some few Blackthorn girls are put on hormones if they have very painful periods or their cycles won’t stabilize. However, the Order feels that it is important for young women to experience the ebb and flow of their hormones while cycling naturally. Diaphragms let them become accustomed to handling their genitals w/o fear or shame (as so many young women these days have not been taught to do) and can experience the marvelous surge in creativity when fertile. Additionally, the discipline of inserting the diaphragm every evening before bed and removing and cleaning it each morning gets each into a routine that will be beneficial when they become sexually active.

 

New Years dildo presentations: In my annual New Years presentation as Baroness I presented each newly blossomed young woman with two dildos: her first borosilicate (Pyrex) glass dildo, a 1 ¾ inches in diameter and 8.0 inches long glass shaft in the shape of a phallus with a handle to use while on her back – a nice feature about Pyrex is that it can be warmed or cooled for use in hot or chilly weather. And since all the Priory school girls take ballet/pointe a high quality silicone one of the same size with a suction cup base with which to solo train for male penetration in the ballet studio. This second item is especially useful in training for deep penetration of the posterior fornix while en pointe during ballet-sex – suctioned on to the mirrored wall in a studio where the user can hold on to the barre rise en pointe and back on to the dildo. Fitting the local swim team with latex diaphragms: Today Chris and I fit the local swim team with latex FS diaphragms.  The custom made 55, 60 and 65 mm latex flat spring rim diaphragms, to be used as ‘gas-guards’ since there have been two recent injuries involving women in the UK getting air or water in their uteri while playing or having sex in a pool which have gotten a lot of publicity in the medical press in the last few months. So since most adults in the surrounding villages work for the Barony either directly or indirectly my clinic is fitting the swim team with diaphragms at no cost to the swimmer or her family.

 

As my readers know latex diaphragms are particular effective for pool-sex since silicone lubricant can be used w/o harm to a latex diaphragm. Returning readers may also recall that a ‘gas-guard’ is a cervical barrier to prevent air being forced through the cervix into the uterus by a lover blowing into his partners vagina or when playing around with SCUBA gear where a mischievous friend holds the mouthpiece of his or her reg against a partner’s vulva and depresses the purge button sending a jet of compressed air into the vagina. All the swim team have mini (4 bead) frameless copper GyneFix IUDs implanted, (also free from the Blackthorn Clinic) with their strings trimmed off so they can wear cervical barriers w/o the possibility of pulling out the IUD while removing the cervical barrier when no longer needed.

 

FemCap wasn’t a choice since the depth limit of a FemCap is about 25 to 30 feet before squeeze on the dome makes it too uncomfortable to wear. And some of the girls are too small to effectively wear the smallest FemCap which is 22mm.  Caya was not considered because of the small sizes needed, rotation and the need for silicone lube.

 

 



The Ring-Maidens

NuvaRing an etonogestrel/ethinyl estradiol dispensing vaginal contraceptive ring

The photo: NuvaRing is a non-biodegradable, flexible, transparent, and colorless to almost colorless, combination contraceptive vaginal ring, with an outer diameter of 54 mm and a cross-sectional diameter of 4 mm. It is made of ethylene vinylacetate copolymers and magnesium stearate, and contains 11.7 mg etonogestrel and 2.7 mg ethinyl estradiol.

NuvaRings dosage and strength: When placed in the vagina, each ring releases on average 0.120 mg/day of etonogestrel and 0.015 mg/day of ethinyl estradiol over a three-week period of use. However, NuvaRing can remain continuously inserted for a maximum of four weeks w/o reducing the user’s protection. 

The Ring-Maidens: My wards, Willow and the Swan Twins, Odette and Odile, have girlfriend from UNLV visit us in London for a few weeks before they all start grad school in the fall. The girls have ballet backgrounds, willow met them as students in the pointe class she taught at UNLV, having all had at least ten years of ballet and six years of pointe. One of the reasons they came to visit, in addition to the marvelous shopping opportunities in London, is that when they would meet in Vegas to gossip Willow would tell them of the awesome ballet-sex she was having which led to the friends developing ballet-sex fantasies and this summer was the perfect time to bring those fantasies to life. Willow and I teach technique and pointe classes in the studio at Blackthorn house and dive-sex in the basement pool.

As much as I’ve tried to get their friends to switch to IUDs or Nexplanon the three of them all use NuvaRing for contraception. NuvaRing is effective if you lead a vanilla sex life which is not what our visitors wanted or are having so there have been some unexpected complications. Two have had their rings disappear during dive-sex in the pool at Blackthorn House in Eaton Square, but I was able to get replacements immediately through the Blackthorn supported clinic where I fit contraceptive barriers. The girls have all had their Gardasil shots and passed full panel STI tests before they left Vegas so as Ring-Maidens they could safely become sexually active right away which is what happened as Willow likes to get her guests into the swing of things right away.

Willow, now back dancing with her company at Covent Garden, has fixed her guests up with male ballet dancer friends who are working for Taryn’s procuress, Tanaquil, who is based in Munich. Eventually the ballet boys will be moonlighting as escorts, but they are now in training to build up sexual stamina before they can be placed with the sort of up-market sexually predatory women who are Tanaquil’s best customers. So our Ring-Maiden guests are having as much or more sex with strong, handsome, virile ballet partners than they can handle. Well, perhaps not that much, but they are all having a marvelous time in bed or backed against a mirrored wall or on their toes sur les pointes a la seconde bent over holding on to the barre while being thrust into from behind and are sore in the mornings!

Willow will be taking the Ring-Maidens as well as Bea and the Swan Twins to tea at the Royal Opera House in the beautiful surroundings of the spectacular, vaulted glass and cast iron Floral Hall constructed in 1858 by Edward M. Barry, which was renamed Paul Hamlyn Hall in 2007 in honor of the philanthropist. It was once a lovely part of the Covent Garden flower market. Afternoon tea will be available on weekends at 2:00 PM from 1 August—28 November 2015. The tickets are expensive, but it will be a wonderful experience and a delightful break from shopping to gossip and people-watch. And Marvin and I are taking them and their dates on another day to eat dinner at The Paul Hamlyn hall Balconies Restaurant before seeing a performance.

Tanaquil’s men are all well-hung so I’ve cautioned our Ring-Maidens to make sure their NuvaRings are still inserted when their partners withdraw because with well endowed men the corona of the glans has been known to hook the ring and pull it out, which has I mentioned above has already happened. If the woman is in bed when the ring is pulled out it can often be found, but if it comes out while she is in a pool it can be easily lost. I think the best thing for NuvaRing users to do when being penetrated by a large penis is to remove it before penetration - especially if the woman has a shallow post-pubic vault which makes the ring easier to accidently come out - since it can be safely left out for three hours with no decrease in the wearer’s protection.  

Yule at Crag Abbey

The Yule log: “Because there are no accounts of the custom in Great Britain prior to the 17th century, some historians and folklorists have theorized that it was not an ancient British custom but was in fact imported into Britain from continental Europe in the early modern period, possibly from Flanders in Belgium, where the tradition thrived in this period.

The first written account of a Yule log in Britain is by the clergyman Robert Herrick, from the 1620s or 1630s. Herrick called the tradition a "Christmas log" and said that it was brought into the farmhouse by a group of males, who were then rewarded with free beer from the farmer's wife. Herrick claimed that the fire used to burn the log was always started with a remnant from the log that had been burned in the previous year's festivities. He also said that the log's role was primarily one of bringing prosperity and protection from evil - by keeping the remnant of the log all the year long the protection was said to remain across the year.

His Grace’s family has always lit their Yule log from the remnant of the log from the previous year and they cut and prepare the following year’s log a year ahead so it has time to season before its use. It is lit on December 21^st, the Winter Solstice, and it continues to burn through the Twelve Days of Christmas, beginning on Christmas day and lasting until January 5^th. It is a massive log and is positioned in the massive fireplace in the great hall by six Crofters and two mules – that live in stables retained at the abbey especially for their care during the winter holidays -  and is constantly attended for the entire sixteen days.

His Grace and Marvin: Himself has become noticeably more at ease with me since his son and heir, Jack, Viscount Sandbach, has become smitten with Ashley a RB ballerina from a noble family who is his current bedmate. Not that His Grace was ever rude as he enjoys sex with me too much to take his pique to the point that I would cut him off. But now his sexual overtures are much more sweetly and gently expressed. Actually, he is a better lover now that he doesn’t feel I pose a threat to the longevity of his dynasty. His attitude toward me has changed to the extent that he is using his influence with The College of Arms to speed up the investigation (to the extent possible through the old-boy network) of a claim the Jack first discovered of my possibly being the heir to an ancient Welsh Barony previously thought to have become extinct.

The Duke approves of Marvin because he seems to have diverted my attention from getting Jack to marry me.  And in addition, as a physician, and psychotherapist with an advanced degree from Cambridge Marvin enjoys elevated social status (for a commoner) and is such an amazing conversationalist – part of his gift that makes him such an amazing therapist - that the Old Bull enjoys long conversations with him and wants to take him hunting if the weather ever clears

Sleeping arrangements: On holidays like this no one is monogamous. We women all have individual suites at the Abbey and the men rotate between us by some arrangement among themselves that gives all the men an opportunity to spend at least one night with each of us.

Tanaquil sent over a 23 y/o Russian ballet dancer, Maya, for His Grace’s holiday pleasure. Jack has Ashley, Marvin has me and Tanaquil sent virile male soloists from the Paris Opera Ballet chosen for their looks and sexual stamina for Bea, Willow, and the Swan Twins. My Dresser is partnering Chris, our male Gyn.

Ashley, Maya and me: I had met Ashley, the RB dancer who is Jack’s current bedmate, at the Party here at the Abbey when Scotland rejected independence from England this past Fall. I thought I’d dislike her but I don’t. Actually she’s quite sweet and in her mid 20s is just learning about fetish sex so I have been tutoring her in certain aspects of that skill. Maya dances with the Mariinsky and speaks fluent English with a Berkshire accent. She is much more knowledgeable about fetish sex than Ashley and in some respects possibly me as well since I don’t usually do anal or double or triple penetration.

I saw them both with the Abbey Divemaster trying on wetsuits and adjusting the spiders on their OTS Guardian FFMs so afterward at lunch I asked if they were going to dive the cistern with Jack and His Grace and when they said yes I asked if they had ever had dive-sex.

Just as I suspected, no one (all men who had been prepping them) had.  So I fitted Ashley with a 70mm latex Reflexions flat spring rim diaphragm and a 75mm for Maya who is taller. I explained the Reflexions is nearly impossible for a mischievous partner to under-thrust. Fortunately neither is allergic to latex and both have lovely deep post-pubic vaults so no partner should feel the rim during dive-sex. Then I showed them how to insert and remove their diaphragms and let them practice insertion and removal several times and gave them both  12-packs of 10ml prefilled disposable applicators of DiveGel+ for pre-lubing before a dive.

During menses both use a menstrual cup so they are familiar with inserting a device in their vaginas which made it much easier for them to place the Reflexions correctly. Maya has a ‘normal’ – 31mm x 24mm - German Medusa Goldring Ag+Au (silver+gold) IUD inserted from which the strings have been removed. She has been wearing it for a year w/o problems even when being thrown and landing jumps so she should be safe from pregnancy.

Cerazette: Ashley is on the progestin-only-pill (POP) Cerazette as so many other European dancers are, so in theory she shouldn’t need the diaphragm for contraception, but only when she dives for protection during underwater sex.  A Cerazette pill contains 75 mcg of desogestrel which metabolizes into etonogestrel that is used in Nexplanon and NuvaRing. Etonogestrel primary effect is prevention of the follicle ripening and releasing an egg. However, our clinic in Vegas has had quite a few women on Cerazette come in for abortions so perhaps because there is a long half-life of the progestin being used the women are not as careful as they should be about taking them correctly.

Ashley seems to have adjusted well to the hormone in her pills with no decrease in libido, or increase in weight, dryness or mood swings and she has few instances of spotting which makes it a great pill for her. I cautioned her not to leave her pill pack unattended while at the Abbey as there could be mischievous guests who think it great fun to sabotage a guests birth control for any number of reasons including but not limited to; jealousy, revenge or just curiosity to see what their target would do if she found herself pregnant.  

Other etonogestrel contraceptives and their dosages: For readers wondering about the daily hormone release amounts of other popular etonogestrel progestin contraceptive methods the two most popular are: 1) Nexplanon a single rod progestin-only-implant. The release rate is 60-70 mcg/day in week 5-6 and decreases to approximately 35-45 mcg/day at the end of the first year, to approximately 30-40 mcg/day at the end of the second year, and then to approximately 25-30 mcg/day at the end of the third year. 2) NuvaRing releases on average 0.12 mg/day of etonogestrel and 0.015 mg/day of ethinyl estradiol over a period of three weeks.

Gifts from St Nicholas: No one got coal, or peat, which is a primary fuel here, in their stocking.  There were a lot of gifts of condoms (which no one plans to use while here as everyone has past a recent full STI panel. There were performance enhancers, Cialis and Viagra, for the men and ellaOne (the UK EC brand) for the ladies. Just in case since one can never be entirely certain that contraception will work all the time.

Fet-tex rubber catsuits: The big presents for the women, (I even managed to get one for Ashley) except for Maya who I hadn’t known was coming, and eye-candy for the men were new custom latex catsuits made of Fet-tex, a new formulation of latex rubber from Jeff at Labia Labs. Fet-tex has all the benefits of natural latex such as marvelous heat transfer properties, but is molecularly stronger to resist punctures and tearing and is far stretchier – think cotton Lycra - with immediate return to original size when no longer being stretched so can comfortably be worn during ballet class, if the wearer can stand the heat. It is also more resistant to oily lubes, medicines and perfumes that contain natural oils and isn’t sticky when dry (w/o being chlorinated) so it is much easier to get into w/o using talcum powder or a water based lube. All the other women’s Fet-tex catsuits have dual-pull back-zips. Mine is dual-pull front-zip so I can easily breastfeed as well as open the crotch for relief or sex and the Fet-tex cups expand as my breasts fill so the suit is amazingly supportive w/o binding or pinching. The zipper is miniaturized and more flexible so a front-zip in a Fet-tex 2mm suit is as flexible as a back-zip suit in standard natural rubber latex.

A catsuit made of 2 mm Fet-tex is nearly indestructible, a feature that any woman who has been held against a masonry wall while being fucked wearing a latex catsuit will appreciate.  With her partners hands clutching her buttocks and her legs wrapped around his waist while he pumps seed deep inside her the up and down motion of his thrusts cause the back of a typical natural latex catsuit to shred against the rough texture of the wall so a new $200 suit can be easily ruined in less than five minutes. However, Fet-tex catsuits like all latex apparel are impervious to moisture so they are quite hot to wear if the wearer is exerting herself and so are very dehydrating. The sweat drains down the legs soaking the boots or shoes of the wearer, which is another reason to wear polymer block Gaynor Minden pointes which won’t melt when in a rubber catsuit. Staying well hydrated in any rubber catsuit is extremely important.

Visual erotica: The men enjoyed us modeling our Fet-tex suits and depending on the men who were taking us we wore either ballet-boots or pointes to minimize the thrust-drop while we enjoyed having them spew their seed quickly, almost as though they were prematurely ejaculating, during ballet sex. Having men that anxious can be fun when there a lot of them as there are here now. The Fet-tex rubber has a very distinctive latex smell mixing with our estrogenic perspiration, the scent of our shampoo, perfume, our pointes or boots (and in my case breast milk) and the scent of the man we are with so the bouquet just screams fetish sex! The FC2, NuvaRing and CSH at St Lucy’s

The Photo: An FC2 female condom made of nitrile Correct use of the FC2 is the starting point for St Lucy’s students in the contraceptives portion of my Contemporary Sexual Health course and an option in the dive-sex portion of my Advanced Sexual Techniques course.  The FC2 is a nitrile sheath or pouch 17 cm (6.7 in) in length. At each end there is a flexible ring. At the closed end of the sheath, the flexible ring is inserted into the vagina to hold the female condom in place. The other end of the sheath stays outside the vulva at the entrance to the vagina.

St Lucy’s Contemporary Sexual Health (CHS) course: All St Lucy’s students who have reached menarche are required to take CSH. Lab training begins with the FC2 female condom. First, how to insert correctly, then with condoms inserted the entire class practicing penetration with dildos in an encounter salle and then in an encounter pool and after that individually with lab partners from UNLV.

Really, would I lie about something that important? Between our sexual encounters as my therapist he is trying to get me to concentrate on three areas: 1) my teaching the art of irresistible love-making at St Lucy’s, 2) my helping fit cervical barriers at a local women’s clinic and 3) celebrating the celestial events ushering in the seasons for Aphrodite all of which I see as my callings in support of the Goddess, and all in addition to my responsibilities as Artistic Director and choreographer for my ballet troupe. With some playtime left for me - in addition to mind-blowing sex with my therapist - of course.

Conservation and Jenna’s Bloch Alpha pointes: I’ve been trying to wean Marvin off Jenna’s traditionally made pointe-shoes. The ones she bought and sweated in are becoming a scarce and non renewable resource. Not because Bloch Alpha shoes aren’t available, but because new ones that I break in won’t have her scent in them.  So I have been very slowly bringing him around to accepting me in Gaynor Minden pointes with suede platforms as I  break in new pairs, getting him use to the scent of my sweat, blood and toe-cheese as it interacts with the inside lining of well used GM shoes. Hard shanked suede tipped GM pointes are so much more comfortable to wear, last so much longer and provide so much better support during ballet-sex.

Diva Cup and coitial discharge: An off list use for a menstrual cup (my circle and I prefer the Dive Cup, but any brand that fits should do) is as a collection device for coitial secretion control and reuse. In addition to it’s primary purpose of flow control a menstrual cup is particular useful while producing fertile cervical mucus (FCM) and after bareback intercourse if inserted shortly after withdrawal before the semen liquefies as the contents of the cup are kept away from air – to minimize odor - and at body temperature until they can be consumed by fetishists at some later more convenient time.

DCS at Splash: Divemasters at Splash watch the Towel Girls dive computers to ensure the girls don’t exceed table limits which would require deco stops to minimize the likelihood of getting decompression sickness, DCS. Occasionally a Towel Girl will tamper with her computer to show fewer hours or shallower encounter depths, or both, in order to get more work. When placed in the vagina, each ring releases on average 0.120 mg/day of etonogestrel and 0.015 mg/day of ethinyl estradiol over a three-week period of use after which it is removed for one week. Then a new ring is inserted. And, the ring can be worn under a diaphragm if the woman needs flood protection for dive-sex. 

When worn w/o a diaphragm it can be hooked and pulled out by a partner’s penis so a wearer should insert a finger immediately before and after each act to ensure the ring is in place. His Grace says that’s partly attributable to the secrecy of his grandfather and partly to the fact that the workman who helped hide the gold were sent to war as part of the British expeditionary force that was trapped on the beach at Dunkirk. During it’s evacuation in late May and early June of 1940 the boat in which His Grace’s estate workers were being taken off the beach was hit by artillery fire and they were all killed. There was also a shaft collapse at the mine in 1947 which killed the two supervisors who took the gold from the smelter to His Grace at the Abbey to be placed with the other gold. And lastly his grandfather had a massive heart attack and died w/o telling anyone where the hiding place is. The old bull, (Jack’s father) says that his father searched for years and supposedly never found the hidden gold, but that after a while seemed to take the loss in stride as the family is still extremely wealthy. However, the current Duke believes his father eventually found the gold as he would spend weeks at a time at the Abbey and then suddenly his personality improved markedly and he stopped spending so much time there, leading the current Duke to think his father finally located the treasure. If so the location was never shared with Jack’s father, as he tells the story.  This story has passed into legend in the UK financial community as a cautionary tale of do-it-yourself security going badly wrong. It also has fascinated bankers about Crag Abbey since that is where everyone thinks the gold was hidden.

Cistern bottom debris: When the Abbey cistern was first converted to a swimming pool / aqua-dungeon of sorts, in the mid 1970s it was thoroughly cleaned of bottom debris. Things like bits of armor and equipment, the bones of small animals that found their way into the vault and fell in and drowned and buckets that fell apart and sunk. However, since then it hasn’t been touched as far as bottom debris is concerned. Adolph gets a live-in teen: Adolph’s eighteen y/o niece, Andrea, will be staying with him for a while. Birth control at colleges and Prep schools: Most female students still use oral contraceptives (mostly the combined, estrogen/progestin, pill) for birth control and many of them don’t take them correctly and consistently causing that method on average to be about 92% effective. We are about six weeks into the school year and are beginning to see the first of the students with unintended pregnancies come in for terminations.

Being on hormonal contraceptives takes more care than just taking a pill every day at the same time. That’s primary of course, but the pill and even implants, transdermal, and transvaginal methods, hormonal IUDs and injection methods can be compromised by other meds and supplements. There are a lot of reasons that young women don’t take their contraceptive meds as they should or take interacting meds or supplements: Lack of an effective regimen at the same time every day. Unpleasant side effects that tend to cause users to ‘forget’ to take their pills.

And things to avoid that should have been explained to them such as: weight loss pills taken to counteract the ‘freshman fifteen’ can speed the metabolism to the extent that the contraceptive hormones don’t stay long enough in the bloodstream to keep an effective level to suppress ovulation. Or taking the EC drug ella and then not using another effective method for the rest of that cycle, because the steroid in ella binds with the woman’s progestin receptors and prevents progestin from any pills taken for the next several weeks from being used by her body, leaving her open to ovulation and conception after the encounter of concern for which ella was taken had long passed. And then there is reduced effectiveness caused by some antibiotics. Or Vomiting within two hours after taking a pill and the hormones may not have been digested.

My Wards after school Jobs: I’m pleased that we got Bea, Cyndi and the Swan twins using GyneFix copper IUDs so they can continue to cycle while being very effectively protected from pregnancy. That way they can be as sexually active as they like with only having STIs to worry about. I’ve been able to get the twins onto the male escort trainee’s partners list so they will get the benefit of training with professionals. If she is lucky she knows who’s so she can go to him for support if she wants to.

Most students seeking a termination want a vacuum aspiration.  While it is a bit uncomfortable it’s over in a half hour or so, while with Mifeprex, RU486, the process takes several days and still may require a vacuum aspiration. Too, there have been some fatalities with women using Mifeprex so it doesn’t have quite the cachet it had when first introduced several years ago.  

Contraceptive sabotage

Naughty’s and sabotage: For women new to the scene the front room of the Meat Market at Naughty Pleasures can be a thrilling but risky adventure. Condom sabotage is a fairly common pastime for some regulars at Naughty’s. We have undercover security women, who try and catch these guys, but they are like cockroaches and we can’t get rid of them all. On-site hookups or take-out relationships for women who rely solely on condoms and don’t use EC as back-up can often result in their partners leaving them with a little something to remember him by. If I’m looking for a quick load not from a Mister Right but a Mr. Right Now I use my own condoms. Not because I’m afraid of conceiving, but to minimize the possibility of being given a STI which can easily be transmitted through pin-holes. The back room at Naughty’s (the STI screened area) where sexually experienced women gather is far safer from the standpoint of exposure to STIs and unintended pregnancy.

Reduced effectiveness: It’s a shame so few women understand their bodies and how to safely use the various methods of contraception available. That goes for men as well if they are into fantasy conception. However, women who are with men into ‘reality breeding’ of women have a different problem if they are insistent on using contraception.

If she fears her partner might sabotage her birth control the woman should be familiar with effective methods that can be used to sabotage her birth control so she can guard against them just as she watches her drink to see she isn’t slipped Rohypnol (flunitrazepam, also known as "roofies"). Here is a summery of some, but not all, methods that can be used to decrease the effectiveness of a woman’s contraception

Hormonal methods: Ideally, if a woman is using her hormonal method correctly her cycle is suspended and ovulation is suppressed so she is safe from pregnancy with an unprotected man. If she misses a pill or two or is late starting a new pack, inserting a new ring, changing a patch or getting her shot late her cycle can start and she will ovulate often w/o her realizing she is in danger unless she is familiar with her fertile signs; a ‘ripe’ high, soft, open cervix discharging a thick very slippery and stretchy cervical mucus and a greater interest in men and sex. If she isn’t impregnated about two weeks after ovulation she will have her period which will probably be off schedule which may be the first time she knows to review her regimen to see why she may have ovulated. If a woman is familiar with her menstrual cycle she can tell when she becomes fertile when she shouldn’t be.

If a woman on hormonal contraceptives takes a drug or herbal product that induces enzymes that metabolize contraceptive hormones the effectiveness of her birth control can be reduced. Some drugs in that category are barbiturates, topiramate (an antiseizure/ migraines and weight loss med), as well as some antibiotics. Ella (the EC pill) will block a woman’s progestin receptors for the rest of her cycle after taking it as EC. Over time these meds can work to reduce the effectiveness of Mirena, Skyla, Implanon/Nexplanon, Ortho Evra (the patch) and NuvaRing (the ring), Depo-Provera (the shot) as well as combined (estrogen/progestin) and mini-pill (progestin-only-pills). St John’s Wort will affect the estrogen in combined pills. There is no hormonal contraceptive that can’t be sabotaged if a man is clever and patient enough

Most of these interactive meds require an Rx, at least in the U.S. so women using them for medical reasons should be aware of their decreasing the effectiveness of hormonal birth control and avoid hormonal methods while on them. A man would need the assistance of a medical practitioner or pharmacist to obtain them for use against an unwilling woman.

NuvaRing: Women using NuvaRing (a vaginal ring) should be careful to destroy an old ring when removed. Otherwise a lover intent on sabotaging her protection can fish the old ring out of the trash and switch it with the new ring she just inserted during foreplay. If a guy wants to try that he should soak the ring he pulled from the trash in hot water for several days to ensure all the hormone is relased from the ring. Care should be taken that the water not be too hot that it melts the weld holding the ends of the ring together or he will have to fish another used NuvaRing out of the trash.

Barrier methods: Women using a barrier as their primary method of birth control are usually cycling naturally with a cycle of between 25 and 35 days. On average a woman’s natural cycle will vary around 28 days. Women cycling naturally usually keep a menstrual calendar so we aren’t surprised by our periods appearing w/o warning for those of us fortunate not to suffer from PMS.

Diaphragms, cervical caps and male and female condoms can be tampered with to lessen their effectiveness. Poor storage, of latex condoms in the glove compartment of a car in the summer or using after the use-by date has passed can be quite risky. Female condoms (the FC2) are made of nitrile and have no temperature storage limitations, just a 3 year shelf life set by the FDA. Latex condoms and latex and silicone cervical barriers can be damaged by using natural oils and petroleum based lubricants (Vaseline) with them.

Most barrier methods can be punctured by a pin or torn accidentally or intentionally by a sharp fingernail. Male latex condoms are especially vulnerable to being punctured though the FC2 and the silicone Semina and latex Reflexions diaphragms are also relatively easily torn. The FemCap dome is so thick it can’t be easily torn by a fingernail, but can be punctured. If the woman has a well fitting diaphragm or FemCap and the dome has been punctured it won’t develop a tight suction which is a tip-off that there is something wrong. Older vintage true cervical caps, which grip the cervix, like the latex Prentif cavity rim and the silicone Oves can also be easily punctured and affected by oily or petroleum based lubes. Oil based or petroleum based lubes can be identified by their scents. A woman concerned that an incorrect lube might be used should provide her own water based lube.

Breeding diaphragms: [condensed from my December 18, 2011 post about breeders] Since there has been an increase in the number of men asking for breeding encounters with our escorts I’ve started training the girls to use a ‘breeding diaphragm’. For rape fantasies I take a different approach when providing contraceptive protection that ‘accidentally’ fails. Rather than sabotage my partner’s condoms which they hate wearing anyway I wear what I call a breeding diaphragm. A breeding diaphragm is one that is two sizes smaller than the size the woman should wear for it to be effectively used for contraception. And it is worn ‘dry’, that is w/o spermicide in the dome. I’ve found the type diaphragms that work best as breeders are the Ortho All-Flex (arcing spring) or a Milex Omniflex (coil spring) rim styles.

When a woman has a diaphragm inserted that is considerably smaller than her correct size it is much easier for her partner/fantasist to under-thrust the rim with his penis and plant his seed under or in the dome so that when he withdraws the rim will reseal on the anterior wall of her vagina trapping almost all of his semen inside the dome where the tip of her cervix will be submerged in a pool of semen while she sleeps. Or if the couple is into bondage and the woman is tied down and to be gagged during the encounter an additional advantage of using a diaphragm during a rape/breeding encounter is that it can be pulled out by her attacker and shoved in her mouth as a gag.

Ideally an Ortho All-Flex is the best breeder because the rim is the easiest to under-thrust. However, the smallest size All-Flex is a 65mm and some adult women might take a 60mm in a breeder so they would need an Omniflex which makes a 60mm in a coil spring rim.

Someone unfamiliar with cervical barriers might ask why not use a cervical cap, such as FemCap for breeding? The answer is that while an improperly sized FemCap can be displaced off the cervix with a thrusting penis it may fill with semen, but it is very unlikely to reseat itself on the cervix after withdrawal. And a diaphragm rather than a FemCap is far safer for the woman since a FemCap if pulled out and used as a gag can easily be lodged in the woman’s throat and her partner, with his mind elsewhere, may think she is squirming in ecstasy when in fact she is being asphyxiated, especially if she has been penetrated from the rear, taken doggie style, where he can’t see her face.

Men wanting to surprise a partner can order a diaphragm on-line that is two sizes smaller than her correct size and ask to insert it himself as part of foreplay. If she checks after him to make certain her cervix is covered an alert woman can tell he has substituted a smaller one, but if her mind is elsewhere he can often get away with the substitution. If she doesn’t check that the diaphragm immediately after she lets him insert it and he is gentle while under-thrusting the rim the next time she might notice something is wrong is when she has very little coitial discharge or has trouble reaching the smaller device to remove it. It’s best for a man to have her accustomed to him inserting her regular size diaphragm so she is less likely to check when she is fertile and he substitutes a breeder in its place.

A note to my women readers: ALWAYS check to see that your cervical barrier is properly placed and developing the proper vacuum in the dome before any penile penetration.

The copper IUD: The Nova-T and Flexi-T in Canada, the ParaGard in the U.S and GyneFix implant (which we are using on a clinical trial basis) are all copper IUDs. A copper IUD is immune to being tampered with by being given something that would decrease its effectiveness unknown to the wearer. My clinic prefers the three year GyneFix implant placed w/o strings so a cervical barrier can be worn with no danger of the barrier pulling out the IUD upon removal after using it as flood insurance during dive-sex.

The only possibilities for decreasing the effectiveness of a copper IUD are to cause the wearer such strong uterine cramps they might displace the IUD or to tug on the strings and pull the IUD out. The latter is not usually painful and if done during menstrual sex the woman may not feel a thing and only notice when she intentionally feels for the strings. Women with new male partners should always check the strings of their IUDs before and after an encounter to be certain it is still where it should be.

If a girl is caught with her suit unzipped during training hours she is dropped two places on the all important student standing list which determines when a trainee gets to choose the male partner she is teamed with for the day.

Death Rubber: It is also the first instance of a trainee wearing a professional quality sports-shield (gas-guard), the flat spring latex Reflections diaphragm although it’s not used. It just gives the trainee an opportunity to wear her FS diaphragm to get used to it and see that (when properly fitted and correctly inserted) it’s so comfortable she forgets she is wearing it. We have found that calling a cervical barrier a sports-shield is seen by students as a more user-friendly term than gas-guard. So far there haven’t been many trainees who have latex allergies and the ones who do usually drop out of the course since so much of a professional escort’s work can involve wearing some amount of rubber fetishwear. I keep a close eye on the trainees who wear the same size cervical barriers I do so that If any should have a fatal accident while training (Rare, but it does happen, especially during advanced training.) I can get first refusal of her sports-shield for my collection of death rubber.

The Ringlets and hCG: Returning readers will remember in my entry for December 16, 2011: ‘The Ringlets, NuvaRing and dive-sex’, A group of UNLV NuvaRing using grad students were going to Cancun on a dive trip on a live-aboard over the holidays and needed sports-shields that could prevent their NuvaRings from being accidentally pulled out during dive-sex. Now she has found that even though she took ella (30 mg of ulipristal acetate) she now is testing positive for hCG which she verified by having a serum pregnancy and she immediately took Mifeprex. Women think they can forget about precautions when they are on hormonal birth control and get away with it, but it just doesn’t work that way.

I think Adolph, Gepetto, yourself and the St. Lucy's administration are in the right to limit or block cell phone use in intimate areas.
