The Zika virus and intimate contact

The Zika virus mosquito (Aedes aegypti) is a mosquito that can be recognized by white markings on its legs and a marking in the form of a lyre on the upper surface of the thorax. The mosquito originated in Africa but is now found in tropical and subtropical regions throughout the world and in 2015 was recognized as starting the Zika virus caused epidemic of babies with microcephaly. The term refers to a rare neurological condition in which children have unusually small heads. In many cases it also means a baby's brain is smaller and may not have developed properly.

Lost Cove, Virgin Gorda and Zika: The BVIs Government haven’t put out much on limiting exposure to the Zika virus which is sure to be in the BVIs by now. My caretaker staff overseen by the estate manager for my good friend Alice Duchess of M**** who has an estate just over the ridge from mine has her staff and mine ensuring there is no standing water where larvae can mature and power foggers are spraying insecticides routinely. I’m now really of two minds about vacationing at Lost Cove. I was so looking forward to several weeks of sun and sand away from the seemingly perpetual rainy skies and gales of the UK. Now it seems I may have waited too late to vacation there this year or until there is a vaccine for Zika. Not that I want to get preggers, but to have to use condoms with my partners while there and for several weeks afterward is such an inconvenience and robs the act of a lot of the skin-on-skin intimacy and the actual exchange of fluids which I so enjoy.

Still, our female staff and guests are cautioned to be sure to use their birth control method(s) correctly since a bite from a Zika carrying mosquito can doom a wanted pregnancy. So far the hierarchy of the Catholic Church in Brazil where the virus seems to have first appeared in the Americas has not officially taken a stand on aborting a Zika damaged pregnancy, although one bishop has said a fetus with severe birth defects is no reason to abort the pregnancy. Easy for him to say! He is undoubtedly the very model of a 19th centaury prelate poncing about in a dress with his head up his a**!  The Church is going to spring for the care of all the micro cephalic infants that weren’t aborted because abortion is illegal in Catholic South American countries, aren’t they! Oh yes! No one should hold their breath! One hopes that South American political and Catholic Church officials come to their senses and show the compassion they continually preach to their flock in the cases of impaired pregnancies.  

The 2016 Summer Olympic Games: There are reports that health authorities in Brazil (where the pandemic began in South America) are not being cooperative in supplying samples of the virus to other countries researchers. This is said to have something with Brazilian laws! Go figure! Earlier reports of sewage filled swimming competition locations were bad enough! However, with what is known now about the hazards of Zika (unless there is some miraculous scientific breakthrough that eradicates Zika) I can’t imagine the 2016 Olympic Games (the XXXI Olympiad) being held as currently scheduled in Rio de Janeiro from August 5th August 21st.  Or if the games are not canceled the huge disappointment to young couples trying to get pregnant canceling attending, or risking infection and birth defects by attending.

Zika can be transmitted through sex: The virus can stay active in blood for about a week and can be transmitted through unprotected intercourse and active virus has now been found in saliva and urine. It is not clear that Zika can be transmitted through saliva and urine, but some authorities in Brazil recommend avoiding kissing. As this is written it is unclear how long secretions from a Zika infected person can transmit the virus to his or her partner. The US CDC is working on an answer. For penile-vaginal intercourse this is making latex or nitrile condoms the safest way to have sex with a person who has been in an area where Zika is known to be active, but saliva and urine as possible virus transmitters makes it a whole new ballgame putting kissing not to mention cunnilingus and fellatio risky as well. Since the HIV virus can penetrate natural lamb condoms they probably aren’t safe to use as protection from Zika either. Nothing has been said (yet) about the virus in perspiration.

Zika is an Arbovirus so there is no quick field test to determine if a person is infected. Blood test intervals of 4 to 14 days can be expected depending on lab load which peaks in summer months.

The Costume Club and Zika:  The transmission of Zika through human secretions has caused precautions to be taken with members of my London club in Kensington off Holland Park road. The Costume Club, AKA Clever Cunts), is requiring members to have a current passport showing they have not been in Central or South America or other Zika affected areas for at least three weeks before being allowed to exchange secretions with a Hostess. Otherwise it is using latex or nitrile condoms for every act of anal, vaginal or oral intercourse. The Aedes aegypti mosquito likes warm weather so England in the winter isn’t likely to breed many, but travelers can bring the virus back to the UK. It would be bad enough if a Hostess or two contracted Zika from a member, but since they are all using very effective LARC methods of contraception there is little likelihood of a Hostess becoming pregnant with an infant with serious birth defects. It is thought that once infected with Zika a person builds up immunity and can not be reinfected.

However, an infected Hostess could spread Zika to other club members which is unacceptable. It is far more difficult to be dismissive about a member contracting Zika from a Hostess and giving it to his mistress and/or pregnant wife. The board of governors acted quickly and unanimously on that so while there has been some grumbling, members understand.

The North Atlantic Drift: I’ve got a few minutes to myself so I’m actually missing the desert weather in Las Vegas. Go figure! The forecast for Llanidloes, the closest forecast to the castle, calls for heavy rain for the next three days from all the relatively warm air coming off the Atlantic dropping its moisture as it rises over the Cambrian Mountains. It’s making me want to risk a trip to Lost Cove regardless of the Zika virus.  

The North Atlantic Drift is the eastern branch of the Gulf Stream that turns south along the coast of Europe. The relatively warm waters of the North Atlantic Drift are responsible for moderating the climate of Western Europe, so that winters are less cold than would otherwise be expected at its latitude. Without the warm North Atlantic Drift, the UK and other places in Europe would be as cold as Canada, at the same latitude. For example, without this steady stream of warmth the British Isles winters are estimated to be more than 5 °C cooler, bringing the average December temperature in London to about 2°C, 35.6°F. Due to climate change the west coast of England and Wales is expected to get wetter and warmer over time.

The weather: We made the trip safely landing in Edinburgh and taking a helicopter to His Grace’s hunting lodge in the mountains to the east of Ullapool. The weather here at the lodge is very cool, in the mid 50s F with a brisk wind and light to heavy rain.  The forecast for tomorrow, Sunday September 22 is about the same, but at a much higher elevation it will be colder so it looks like the girls and I are going to perform while soaking wet, unless there is divine intervention. The thought of being cold and wet again brings to mind how delightful it would be to wear a swimskin, but a few sips of The Old Bull’s Gorse brandy before ‘Showtime’ and the stack of soft waterproof blankets made of red deer skins should take care of the cold, for a while at least. We are all going to bed very early tonight as we are still jet-lagged and have to get up at 4:00 AM to have our buns on the altars at sunrise, which is at 7:06 AM.

A complication: Chris, our male Gyn who travels with us, had gone on ahead to the hunting lodge in the hills overlooking Loch Broom. There he met Taryn who cut classes at Cambridge to join him and they met with the Dukes Veterinarian who looks after the flocks of Scottish\h Blackface sheep he raises for their wool, which goes into making Harris Tweed, and meat.  The most delicious rack of lamb is served at His Grace’s table!

Chris was sent ahead to check the sexual health of the boys (13 to 18 years of age) who had been chosen as candidates to be seed planters in celebration of the Autumnal Equinox. There were 20 candidates, all from his Grace’s extensive sheep farming operations in old Wester Ross-Shire. Three of the candidates, two 17 and an 18 y/o, all from the same farm, tested positive for Chlamydia C. abortus.  The chlamydia bacteria are zoonotic. It can be spread to other species including goats, cattle and occasionally humans. C. abortus is the most virulent strain and so while being treated with antibiotics and recovering the boys were out of contention for seed planting on an altar at Location Z on the equinox

Spew in a ewe: More worrying for them however, is the very real possibility of losing their jobs because of transmitting Chlamydia to His Grace’s ewes which can kill their lambs.  Not because they were fucking ewes, which Jack says is common on sheep farms in the Highlands, but because they rode bareback when they were told to wear condoms. Being tight and with a body temperature of 102 – 103 degrees F Jack (speaking from experience) says a ewe makes a delightfully tight warm fuck and there’s no danger of having to pay child support, though the smell leaves something to be desired. I couldn’t really be upset with him, as sheep fucking is something of a male right of passage in the farming communities in the Highlands. Not to mention that I’ve had sex with a Great Dane, a tiger and several dolphins so I couldn’t honestly be upset with him for taking his pleasure where he found it in remote locations w/o women.

Now that chlamydial abortion has been diagnosed the entire flock is being treated with tetracycline. Fortunately the outbreak occurred in the smallest of the flocks so is thought to be isolated and well contained.  Human cases of C. abortus always involve pregnant women, most of whom will miscarry. Of course my circle and I all test to make certain we aren’t pregnant. Even though we all use DiveGel+ with a biocide that should kill the Chlamydia bacteria and we know our partners are clean it’s still scary to know that we could have an STI bacteria deposited in some of us if we hadn’t been pro-active about STI testing since we need to take our equinox altar partners bareback.  

Partner distribution: Two other candidates had minor injuries from recent farm related accidents that made them ineligible this year so we were left with fifteen healthy participating males. The plan is that we make an equal distribution over the five (one principal and four cardinal compass point secondary) altars which will give us three each. I’m taking the youngest, the two thirteen and one fourteen y/os. Bea gets the three fifteen y/os and Cyndi and the twins share the seventeen and eighteen y/os. If any of the girls can’t handle three I’ll take the ones they can’t accommodate and Jack and if the Old Bull feels up to it I’ll service him as well.

Hoods from hares: The Mountain Hare (Lepus timidus) is Britain's only native member of the rabbit/hare group. The women of the village supporting His Grace’s hunting lodge have made rabbit fur hoods with attached capes over the neck, throat and shoulders, for all the visiting altar girls, from the white winter coats of the Mountain Hare. They are beautifully made delightfully warm and were fitted individually over our hair after we arrived. They are ceremonial altar clothing for warmth and head protection while being repeatedly inseminated on behalf of the earth goddess at sunrise on celestially important occasions when ritual seed planting is called for.

The Swan twins, an update: Odette and Odile seem to have adjusted well to their GyneFix IUDs with no pain or spotting after the initial two day discomfort from having the knot buried in the fundus. Not that a woman in a bar sling at Naughty’s is likely to fall very far, physically I mean. Morally is an entirely different matter. If she’s new to the experience she might fall a considerable distance and with the right man enjoy the encounter immensely while buckled into a bar sling at Naughty’s.

Women and the Sling Bar at Naughty’s: In order to attract men young, unaccompanied, attractive women get priority at the velvet rope and discounted drinks. And they are used to stock the Meat Market, Naughty’s singles bar. For use in the Sling Bar a Gamma Harness is modified to line the inside of the straps, the side that is against the wearer’s body, with tough but soft nylon satin fabric for comfort. Of course the harnesses are fully sanitized after each wearing.

She is also given a disposable mesh bag for her shoes while she is hanging in her sling. Condoms of various sizes are in bowls on the bar so couples can protect themselves from each other’s fluids, or not. The women’s need to be very sexually adventuresome and STI screened limits the number of applicant, but increases their quality significantly, especially their beauty. Why the more adventuresome are more classically beautiful is a mystery.

Mr. Right Now: I have my own Gamma harness that Jack, Viscount Sandbach, calls my ‘glamour harness.’ I keep it in a small gym bag with a selection of appropriate Sling Bar clothing. I buckle myself into it when I’m lonely and want a new man. Not Mr. Right, just Mr. Right Now! No more intently watched twenty feet of catwalk exists in Vegas than that strip through the Meat Market between the sling boutique and the Sling Bar. Shoes for a sling-girl can depend on where she is in her cycle, if she is commando and if she plans on condoms being used. A sling-girl shouldn’t wear fabric covered shoes unless she doesn’t mind them being ruined. There are purse hooks at each location where a sling girl can hang her mesh shoe bag to prevent splattering her shoes with coitial discharge. However, after putting her shoes back on she still may have to walk through lover’s fluids to leave the bar so platform soles are a good idea. Typically a sling-girl will wear a thong so after she Kegels out any discharge the remainder can drain into the liner in her thong. The guys fit the custom boots Gepetto crafts and sells, custom adjusting the shanks and padding and for some women they are the first to penetrate them in their new boots getting their ‘boot cherry’. St Lucy’s AST slang now includes ‘boot-dicks’ = Gepetto’s boot techs and ‘boot-cherry’ for a girl who is not a virgin, but is fucked for the first time while standing in her new ballet-boots. The Okamoto 004: A few girls under the age of 16 are fitted with Gepetto’s ballet boots even knowing they will outgrow them in six months, so you know their parents have money! Fortunately none are from St Lucy’s because of the 4 years of pointe requirement. Gepetto has a rule that his boot techs must all use condoms if the woman he’s fitting asks to see how well her new boots feel during ballet-sex. That service is called a ‘field trial’ and covered in the cost of the adjustment visit(s) which is built into the starting price of the standard (unarmored) boots at $2,500 per pair.

Condoms are required because even if the girl says she is on the pill young women aren’t too good at remembering to take them and Gepetto doesn’t want to make the headlines by being involved in paternity problems. The condoms are also for the techs protection since most of the professional KP boot fighters are promiscuous (Go figure!) so the possibility of STIs is more likely while field trialing with a professional boot fighter. The techs use Okamoto 004 Condoms which the guys say are amazing and feel as though they are wearing almost nothing,

Gepetto started out with only one board certified Orthopedic specialist, but found that even as virile as he is he couldn’t keep up the level of service required given the number of field trials he was expected to perform, so a second hunky orthopod was brought in to spread the load which has worked a lot better, especially when there is a KP boot competition coming up and the professional fighters are in town and want their boots checked. Well, some of us are just curious about the new tech and want to see how good he really is. A really good tech will check his patients foot health; the length of her nails, her calluses, ankle mobility, toe pads and give her a foot massage. It’s here that a tech must have a really good bedside manner to be successful. I walked around in my boots getting used to the wonderful feel, how comfortable they were with the weight I can put on my heels and my weight pressing my insteps down tighter in my boots until I took a little of my weight on the tips of my toes compressing my gel-pads into the quarter of a stress ball in the toe boxes used to cushion the tips of the wearer’s toes. Murder case dismissed in death of newlywed wife

(CNN February 23, 2012) -- An Alabama judge on Thursday abruptly dismissed the murder case against a man accused in the scuba-diving death of his newlywed wife off Australia's coast. The decision from Judge Tommy Nail came near the end of the second full week of David Gabriel "Gabe" Watson's trial in Birmingham. The entire article can be found HERE

Personal comment: It was a difficult case to make and the way the judge summed it up the prosecution didn’t present a convincing case. Gabe Watson has married again. It would probably be a good idea if his new wife doesn’t go SCUBA diving with him.

The quadrivalent cervical cancer vaccine (Gardasil) is more cost-effective than the bivalent vaccine (Cervarix) if the two are equally priced, concludes a study published on bmj.com today. This is despite the fact that Cervarix may provide better protection against cervical cancer.

However, the authors stress that considerable uncertainty remains about the differential benefit of the two vaccines.

Human papillomavirus (HPV) is a common sexually transmitted infection seen most often in young women and adolescents. There are more than 100 types of HPV - some cause genital warts, but others cause cancers including cervical cancer.

Two HPV vaccines (Cervarix and Gardasil) are currently available. Both protect against HPV types 16 and 18, which cause over 70% of cervical cancer cases, as well as several other types of cancer. Gardasil also protects against types 6 and 11, which cause the majority of genital warts as well as a rare disease called respiratory papillomatosis.

In 2008, the UK government chose the bivalent vaccine Cervarix for its HPV vaccination programme, based partly on analyses by Mark Jit and his colleagues at the Health Protection Agency suggesting that Cervarix would have to be £15 to £23 cheaper per dose to be as cost-effective as Gardasil. However, the choice of vaccine will be re-evaluated when the current tender for the vaccination programme ends.

Since then, further evidence has emerged to differentiate the two vaccines. For instance, Cervarix appears to give better protection against cervical cancer caused by HPV types other than 16 and 18. Gardasil has now also shown protection against vulvar, vaginal and anal cancer.

Using this latest evidence within a mathematical model, Mark Jit and his team set out to re-evaluate the two vaccines to inform the next round of vaccine tendering.

They found the price differential between the two vaccines was larger than their 2008 analysis. Based on the most recent evidence, Cervarix would have to be £19 to £35 cheaper per dose to be as cost-effective as Gardasil, mainly due to a lack of protection against genital warts.

When all differences between the two vaccines are considered, "the quadrivalent vaccine (Gardasil) is still more cost effective if the two are equally priced," conclude the authors. However, they stress that "considerable uncertainty remains about the differential benefit of the two vaccines."

An accompanying editorial says that modeling can help decision makers, but ultimately the tender price is the final determinant of cost-effectiveness.”

Personal comment: St Lucy’s was fortunate that we had good advice when we chose Gardasil as protection against cervical cancer as now it has been found to protect against genital warts as well as vulvar, vaginal and anal cancer. This year all new St Lucy’s students must have completed the series of three Gardasil injections over an interval of six months before they are allowed on campus for orientation. All our male partner/instructors have had the full Gardasil regimen as well to provide maximum protection to themselves and their student partners. Gardasil has recently become a topic of debate among candidates for the Republican nomination for the 2012 presidential election. The conservative candidate Michele Bachmann seems to be very poorly informed and continues to blunder on about mental retardation and the dangers of vaccines. Sigh!

New St Lucy’s entry requirements, Pt II: I was remiss when posting about changes to St Lucy’s entry requirements for the 2011/12 school year. I failed to mention that all entering students must have had at least four years of ballet training consisting of not less than four hours a week for at least fifty weeks per year. We aren’t a ballet academy, but the physical and mental development (poise, grace, discipline, confidence and pelvic muscle development) of the students is so closely tied to ballet training and discipline that any less and a student has a very difficult time keeping up. One might think that this requirement would dissuade parents from enrolling their daughters at St Lucy’s, but that hasn’t been the case as highly qualified applicants far outnumber the seats available for new students. Parents (especially their mothers) are determined to begin preparing their daughters years in advance to get into St Lucy’s.

Pelvic Familiarization for post-menarche students: Because the health syllabus at St Lucy’s is heavily weighted toward teaching female sexual skills we require all post-menarche students to take a pelvic familiarization (PF) course. This course covers menstruation, feminine hygiene products and their proper use, reproductive tract devices for contraception (IUDs, condoms and cervical barriers), dealing with the hymen and sex toys. The course purpose is not only to familiarize the student with the range of products and devices available, but to have her become comfortable and confident about touching her own genitals so she won’t be afraid or embarrassed when she takes Contemporary Sexual Health and Advanced Sexual Techniques. To that end we teach and recommend the students masturbate to learn what arouses them and what doesn’t so they can guide a male partner/instructor when that time arrives.

Students in this course usually range in age from 13 to 15 as all train heavily for ballet which often delays menarche. You would be surprised at the number of girls who have been brought up to think that touching themselves “down there” is dirty or shameful so PF has reduced the number of girls needing counseling due to psychological issues from becoming more intimately aware of their own bodies. We are very pleased with the PF student’s response to the classes on the clitoris, condoms, dildos and vibrators and all have delighted in their new found source of self-pleasuring with the toys provided each student for lab exercises.

In the past we have had an occasional case of stress related vaginismus after a student has been accepted so our psychologists believe the PF course should prevent that from occurring

The pill and condoms

Under the LoSeasonique extended-cycle regimen, women take of 84 orange combination tablets containing 0.1 mg of the progestin levonorgestrel and 20 mcg of the estrogen ethinyl estradiol, and 7 yellow tablets containing 10 mcg of ethinyl estradiol. Levonorgestrel is one of the older and safer progestins, not nearly as likely to contribute to blood clots as etonogestrel (NuvaRing and Implanon) and drospirenone (Yaz and Yasmin) though the likelihood of severe side effects on etonogestrel or drospirenone is still very low. The problem with levonorgestrel is that it doesn’t have as long a half-life in the body as the newer progestins and therefore pills that use it are more sensitive to level fluctuations if not taken correctly.

I’m posting this because: This is the most thorough discussion I’ve found of things to consider and what a woman’s (or a couple’s) reasoning should be when deciding if ‘the pill’ offers enough protection for the woman, or if the couple should use condoms to provide added protection. I am having my Contemporary Sexual Health students at St Lucy’s bookmark this article on the Scarleteen site.

This article addresses things that impact the real-world effectiveness of birth control which Eric, a good friend and frequent reader, obliquely raised in a comment to my May 10, 2011 post ‘Effectiveness rates using 2 methods of birth control.’ He wrote: “I wonder if anyone would be willing to try putting these numbers into real-world practice?” Many women for whom 95% effectiveness isn’t good enough use condoms to improve their chances of avoiding pregnancy and condoms are a must when having sex with a partner whose sexual history is unknown.

If You Use the Pill, Do You Still Have to Use Condoms?

“Kori asks:

I'm and 18 years old and have been having sex for a year and been on the pill for about a year. I take my birth control like a ritual at the same time every day (the combination pill). Sometime my boyfriend and I don't use a condom in the beginning to get him hard then we always put one on. My question is, when on the pill do you absolutely have to use condoms? They say that every time you have sex you NEED to use a condom. I know it is the most effective way, but I thought that the one of the points of the pill is so you don't need to use a condom.

Heather Corinna replies:

We get asked about this a lot; about whether once you have a more effective method of contraception than condoms, like a hormonal method or IUD, if they're still needed to prevent pregnancy.

The only right answer to that question, no matter who asks it, is that it really depends on what you and your partner want and need.

All methods of contraception, including condoms, can prevent pregnancy, but some methods are more effective than others. If someone had to choose between condoms and the pill, and just wanted to choose one based ONLY on which was the more effective method of the two in preventing pregnancy when used properly, then they'd choose the pill, because it's more effective in both typical and perfect use. But that's not usually the only factor in people's decisions about contraception or condom use, and plenty of people don't have to choose just one.

If you want the most effective, foolproof protection you can get from unwanted pregnancy and still want to have the kinds of sex that present pregnancy risks, then you'll want to use dual protection: to use two methods, not just one, because using two is always more effective. If you're comfortable with or prefer less protection than two methods can offer, then it's okay to only use one method.

There are no have-to's here, there's just what you want to choose to do as an individual, based on what you want, feel most comfortable with and are or are not willing to take a given level of risk with. If you need a little help making that decision, I can certainly walk you through some of that process, and give you some information that might help inform your choices.

How effective at preventing pregnancy a given method is is often one of the biggest, if not the biggest, factors for people making these choices. So, let's have a look at that first. The combined birth control pill is over 99% effective in perfect use, and 92% effective with typical use (same goes for the ring and the patch). With the pill, specifically, some studies have found that typical use rate is substantially lower for young adults younger than 19, but it sounds like you do use your pill properly, and have for some time, so that probably doesn't apply to you.

What any of those percents mean in the real world is this: 92% in typical use means that for every 100 women who report the pill is the method of contraception they have used in one year, 8 of them became pregnant while using that method, and 92 of them did not.

My best advice when people are trying to figure out if they're in perfect or typical use is to remember that perfect use rates are usually from controlled clinical trials, and typical use rates reflect self-reporting and more typical, real-life use, which includes things like taking a pill late or even missing one now and then in a given year (which is very common: taking a pill at exactly the same time for one whole year, never missing any, never throwing one up, and so forth is pretty tough to pull off). To be safest, it's usually best to consider typical use rates when choosing a method or methods, especially with an ongoing method like the pill where how you take it every single day influences your risks, not just how you take it on the day you have sex, unlike with something like condoms, where how you use it only matters each time you use it. To be sure that's clear, how someone used condoms three days ago has no impact on how well using condoms today will protect them, whereas how someone used the pill three days ago IS relevant to possible risks with sex they have today.

But if you know you use your pill as properly as possible, and have swung that for a year of use to know you can do so for a full year, if you average the typical use rate and the perfect use rate, you probably have a good sense of what your effectiveness is. For the combined pill, that'd be 95%.

Once you know the numbers like that, the question to ask yourself is if that's okay with you. Are you comfortable with something around that 5% in-one-year chance of pregnancy? If you are, then it's okay for you, according to you, to use the pill alone when it comes to preventing pregnancy.

If you're still on the fence and want a comparison, let's do the same math once you add a condom. Condoms are 98% effective in perfect use and 85% effective in typical use. We show estimated rates for using combined methods of contraception here. [This information is found in my post ‘Effectiveness rates using 2 methods of birth control’]. On that page, you'll see the combined rate for perfect use of both methods is 99.99% effective the estimated combined rate of both methods used more typically is 98.8%. If we do that same averaging we did with the pill, for those who use condoms really well, that gives us a 91.5% effectiveness. For those who use both methods very well, then we're looking at just over 99%, a little more than a 4 percent difference between using the pill alone, and just about the same effectiveness as the pill by itself in the kind of perfect use found in a controlled study.

Again, in more practical terms, that means that for 100 people using both the pill and condoms really well over one year, that probably less than one will become pregnant as opposed to five with the pill alone or close to nine with condoms alone. In both cases, that's a pretty big difference.

How big or small a difference that seems to you, though, is going to be about you. There are people who don't want to become pregnant and want to try and prevent it, but who also feel like it would not be the hugest deal if they became pregnant. There are other people who earnestly feel the world would come to an end and to whom pregnancy seems like the worst thing that could happen to them at a given time. Sometimes during one time of life or relationship, while we don't feel a pregnancy would be ideal, we feel like it would be okay and that whatever choice we might make, we could deal with that choice. Other times, for whatever reason, it really is just the last thing we want to have happen. I'd say that if and when you're in the world-will-end, last-thing-I can-deal-with group, that if you're going to have the kinds of sex where pregnancy is a risk, dual contraception is the way to go. If a possible pregnancy feels way less dire than that, then it's much sounder to consider only using one method.

This is something you can also talk with your partner about. There are a few reasons why some couples prefer using condoms, specifically, with another method to prevent pregnancy. One of the biggest perks is that then both people get to share responsibility and both people also get to have some control over the prevention of pregnancy. While a pregnancy can't impact your partners' body and health as it would yours, and often asks less of them in terms of the impact on their whole life, it still can have a big impact on their lives, no matter what choice you make with it, particularly should you become pregnant and choose to parent.

Some folks feel fine without that personal control of their own, while it makes other people really uncomfortable. Some folks also react particularly badly to an unintended pregnancy when they didn't have control: it's easy to blame the other person who had it when it happens, even when a birth control failure wasn't that person's fault. I also don't know what your feelings are about taking all the responsibility for birth control here. If you would rather it be shared, that's one reason to keep using condoms, but you also have the option of sharing responsibility for the method you're using alone, too, like having a partner split the bill for it.

You also might want to consider how much you and/or your partner tend to worry about this stuff. Any kind of worry or anxiety can impact the quality of our sex life and our whole life, so if using one less method = more worry for either of you, that may not be so ideal. If, on the other hand, using two methods alleviates worries, it may be a real positive. And if it seems to make no difference, then maybe this just isn't a factor for either of you at all.

How much you and your partner like using a given method and what kind of access you have to that method is often another factor. So, if the pill alone doesn't leave you feeling secure enough, but using condoms is an issue for any number of reasons, you might also bear in mind the pill isn't your only option and there are more effective methods than the pill, and, since the main difference with those others is mostly with typical use, far more goof-proof ones.

You may have the option of choosing a method of contraception that's more foolproof and effective in typical use. Sometimes people use the pill just because it's the only method they knew about or the only one a healthcare provider suggested: I don't know if you've considered other methods or not. Depo-Provera injections, the contraceptive implant and IUDs all have much higher rates of effectiveness in typical use, because they're really, really hard to screw up as a user. With IUDs and implants, there's nothing you really need to do at all, save replacing them every five to ten years. With an injection, you only need to remember to go get a shot once every three months. So, if you want to ditch condoms, but in looking at all this, you feel like the pill isn't leaving you feeling secure enough, you can talk to your sexual/reproductive healthcare provider about some of these other, more effective, options. The healthcare provider you see for contraception is also always a great person to ask the questions you're asking me.

This also isn't a choice you have to make in any permanent way. You may find there are times in your life or relationships where you feel just fine using only one method, or this one method, and times when you feel a lot better backing your method up. And sometimes you may need to use a different method temporarily, like if you've got to or want to go off the pill for any reason or you did wind up missing pills or need to take a medication which may interact with the pill. So, don't feel like this is something you only get to decide once and have to stick to ever after: it's not. You can always change your mind based on any changes in your feelings or circumstances.

Last, but absolutely not least, one of the biggest differences between all other methods of contraception and condoms is that only condoms can reduce your risk of sexually transmitted infections.

Many young people immediately stop using condoms once they have another method of contraception, even if they haven't yet done all they need to to reduce their STI risks. For our readers at Scarleteen specifically, this is a pretty big deal since the age group with the highest rates of STIs are those in their teens and early 20s, with around one in every four people in that age group contracting an STI every year in the United States alone. The risks of STIs are pretty big at your age, and more often that not, it's the folks who worry least about them who tend to be the most at risk, since they also tend to be the folks who don't do enough, or anything, to reduce those risks, which includes using condoms and other barriers for any oral, vaginal and/or anal sex.

My best advice is always to figure that for as much as you're concerned about unwanted pregnancy, you want to be sure you're just as concerned about STIs, and doing just as much to prevent acquiring or transmitting infections as you are in trying to prevent pregnancy.

If you do want that extra protection, either from pregnancy, STIs or both, and your boyfriend is having issues with erection when condoms are on right from the start, know that practice with condoms usually does make perfect. If he gets used to having condoms on from start to finish, chances are that this won't stay an issue like it has been. Of course, you both also always have the option to spend more time before intercourse with sexual activities other than intercourse (or oral sex, if you want to reduce STI risks for that activity by using condoms with it) to get him, and you, more aroused before intercourse starts. You can also try different styles or brands of condoms, including the female condom, so that it's not even going on him at all. Just know that if you're going to use condoms, using them properly is very important, just like using your pill is. Proper use includes a condom being used from start to finish with sexual activities, not being put on or taken off halfway through.

Again, there are no have-to's here. There are just your personal choices, made either on your own -- you are the one who would potentially become pregnant, after all -- or with a partner. I think it's always wise to make these choices very carefully and with as much information as possible, and when it comes to preventing outcomes we don't want, we just want to be sure that whatever choices we do make really fit with what we most want, don't want, and what we do and don't all really feel ready to handle.”

IF TEENS AREN’T going to get sex education from their parents, and they aren’t going to get it from their schools, then they’re going to get it somewhere. From music. Or TV. Or each other. And so, according to high school students from Jamaica Plain, here are some things their friends think they know about sex: That you can’t get pregnant if you have sex in water. That if you have oral sex, you won’t get an STD. That there’s no point in wearing a condom because it will probably break.

Hear this, and some of the current statistics about teen health in Boston become a little clearer.

Teen pregnancy rates have dropped over the past decade — the result, public health officials believe, of the increased availability of chemical birth control. But chlamydia cases are rising at alarming rates, a sign that teens don’t understand that certain types of birth control — and certain types of sex — don’t protect you from disease.

In other words, the burden of contraception falls largely on girls. Acting alone leaves them vulnerable. And sex education, for girls and boys, is haphazard at best. Some kids happen to attend a school that offers a meager program. Some attend one of the handful of schools where the Boston Public Health Commission or some third party has a clinic. But many get nothing, or not nearly enough.

Now, students are asking for more; a coed group of teens from JP’s Hyde Square Task Force is lobbying for comprehensive sex ed, including information about healthy relationships and, yes, free condoms for all. Not a bowl of Trojans sitting on a shelf, but a couple of designated adults in every school — ideally, one male, one female — who could distribute condoms to kids who ask, without passing judgment.

The kids are committed; they’ve put together an honest, funny video that states their case. If politicians are willing to be just as brave, we’re in good shape.

And there are definite signs of hope. The Boston Public Schools recently created a new Health and Wellness Department. The school system is partnering with the public health commission to develop a system wide sex ed program. City Councilor Ayanna Pressley has scheduled a hearing on sex ed and condom distribution.

But this kind of talk makes politicians and the public skittish. So it’s worth reminding everyone of the facts. According to the public health commission, nearly 70 percent of Boston teens report having sex by the time they’re 18. About half of sexually active teens have had three or more sexual partners by age 17.

Studies of condom distribution programs in Los Angeles, Chicago, and New York schools have shown that making condoms easily available doesn’t increase the number of kids who have sex. It merely increases the number of kids who use condoms. And even kids who might use condoms, if they were close at hand, will tell you they won’t bother to ask a stranger at the drugstore to open a locked cabinet.

They might, however, ask an adult they trust. And while it’s easy to say they ought to be asking their parents, Massachusetts law does not require parental consent for access to contraception or reproductive health, says Deborah Allen, who heads the health commission’s Child and Adolescent Family Health Bureau.

Besides, as teens from the Hyde Square Task force said, there are often giant barriers to talking about sex at home. Sometimes, it’s a matter of culture. Sometimes, it’s logistics: parents work two jobs, teens are caring for younger siblings, and there isn’t much time for sensitive conversations.

As Pressley and Allen point out, many parents have been asking for sex ed in Boston schools, too. Personal Comment: Here in Vegas our school district’s sex education curriculum remains unchanged — it is abstinence based, although students do learn about contraception. Can you imagine an abstinence based sex-ed program in Vegas? Through our clinic we try to encourage the patients to teach their children about responsible birth control and STI protection. At St Lucy’s because the girls are all taught intimate skills in conjunction with the other arts we take comprehensive sex education very seriously.

Nevada court hears ‘personhood’ measure appeal

Proponents of a ballot initiative to reshape Nevada abortion law asked the state Supreme Court on Tuesday to focus on the eight words that they said clearly define their goal.

"The term 'person' applies to every human being," Personhood Nevada leader Olaf Vancura said afterward, using the same eight words outside the state high court that are the focus of his group's appeal. He wants to be allowed to collect signatures to qualify the initiative for the November ballot.

In court, an American Civil Liberties Union of Nevada lawyer told the seven justices that voters wouldn't know that amending the Nevada Constitution to define "personhood" as beginning at conception would let advocates advance efforts to ban abortions, restrict common forms of birth control and affect end-of-life questions.

"These are hot-button, critical issues," ACLU attorney Lee Rowland said, urging the state's only appellate court to uphold a lower court ruling in January that the initiative is vague and violates state law limiting ballot questions to one subject.

"There needs to be some information in there that lets the voters know what's going on," Rowland said.

Justices appeared skeptical of overruling the lower court based on Personhood Nevada lawyer Michael Peters' argument that the lower court judge erred in examining anything more than the legality of the wording.

Justice James Hardesty asked Peters if there was a difference between considering the constitutionality of the proposed initiative and determining whether it "impacts other provisions in our laws beyond the stated purpose of the initiative."

"You're limited in looking at, in this case, the eight operative words in determining the single subject," Peters responded. "It's an 'on-its-face' analysis."

"Does this fully inform the voters, ultimately, as to what they're voting on, what they're amending in the Constitution?" Justice Michael Douglas asked. "Or is it so open-ended that they're not sure what the meaning is?"

Peters told Douglas, the first black justice on the Nevada Supreme Court, that the initiative aimed to extend civil rights to a person at conception.

"In essence you're asking us to define when life begins," Douglas said. "Does it do that? Is it clear in all cases?"

"It does define human beings and when life begins," Peters responded.

More than 50 people ranging from right-to-life advocates to Planned Parenthood representatives filled the courtroom gallery for the oral arguments in Las Vegas.

A day earlier, another branch of the national organization Personhood USA gained enough signatures to put an anti-abortion proposal on the Mississippi ballot in 2011.

A similar proposal goes before Colorado voters this fall, and Personhood USA leader Keith Mason said signatures were being gathered for constitutional amendments in Missouri, California, Florida and Montana. Mason tallied about 40 states in which the organization was working toward changing laws.

Rowland urged the Nevada high court to consider the effects of eight words "that are only fleshed out and that only have meaning once you read the description of effects."

"So your argument is that the sentence encompasses more than a single subject?" asked Hardesty, who posed most of the questions during the 30-minute hearing.

"Absolutely," Rowland replied. "Allowing it to go through simply because of its brevity, I think, poses a very real danger to the citizens of Nevada, which is that the initiative process starts to be used for vague, semantic and theoretical ideas."

If ‘personhood’ is defined as beginning at conception will that mean that the 20 to 30% of the fertilized eggs that aren’t implanted or that are spontaneously aborted (miscarried) many of which a woman is unaware of will require funerals? If so, how will anyone know? Will Rush Limbaugh, Pat Robertson, James Dobson or other men of that persuasion (the Conception Cops) be checking every woman’s required pregnancy test to see who conceived? Additionally ‘personhood’ would set the stage to promote the erroneous claim by radical conservatives that hormonal birth control is an abortifacient!



DA's sex Ed warning befuddles Wis. teachers, kids

MAUSTON, Wis. – Mike Taake has taught sex education for 30 years, and he says he knows what doesn't work: just telling kids to wait.

The Mauston High School health teacher has used abstinence-only and comprehensive curriculums, and he said students need all the information they can get about sex to make the best choices. But teaching them about contraceptives could land him and other teachers in court.

Juneau County District Attorney Scott Southworth last month sent a letter to area school districts warning that health teachers who tell students how to put on a condom or take birth-control pills could face criminal charges. The warning has befuddled teachers, school administrators and parents in Southworth's poor, rural county.

"Seems like a step back in time," Taake said of Southworth's logic.

Southworth, a Republican and a Christian evangelical, took issue with a law Democratic Gov. Jim Doyle signed in February requiring schools that teach sexual education to adopt a comprehensive approach.

Southworth warned that teaching a student how to properly use contraceptives would be contributing to the delinquency of a minor, a misdemeanor punishable by up to nine months behind bars and a $10,000 fine. He said it would be promoting sex among minors, who are not legally allowed to have sex in Wisconsin.

"It puts the school kind of in the middle between two sides, between the government and state telling us what should be taught and what people think should not be taught," said Scott Lenz, a health teacher in the New Lisbon School District. He said he would teach contraceptive use if he got the approval of his school board.

Southworth said he doesn't want to drag teachers into court but feels he was ethically responsible for warning them of the new law's potential consequences. He urged the school districts to refrain from offering sex education courses until the Legislature repeals the law.

"Listen, there's a real problem with the law," he said. "I didn't pick the fight. The Legislature dumped it in my lap."

Southworth didn't cite evidence in his letter showing that teaching someone to use contraceptives makes them more likely to have sex. But in an interview Thursday, he pointed to Milwaukee Public Schools, which teach a comprehensive sex education curriculum but still struggle with high teen pregnancy rates. Sex education experts, however, say many social factors influence teens' decisions to have sex, including lack of parental supervision and poverty.

Janine Geske, a Marquette University law professor and former state Supreme Court justice, said she didn't understand Southworth's legal logic. She said that if he tried to prosecute a teacher for adhering to guidelines approved by the Legislature and governor, the case would likely be dismissed.

"To be frank, I can't follow exactly what he's trying to get at," Geske said. "If a teacher is educating a student pursuant to state law ... I don't see how under any examination (that) could be criminal."

The state Legislative Council, a group of attorneys that provides legal advice to state lawmakers, issued a statement Friday saying it also believes teachers who follow the state guidelines would be safe from prosecution.

In Wisconsin, children under age 17 who have sex with each other can be prosecuted as juveniles. Seventeen-year-olds who have sex with one another can be convicted as adults of a misdemeanor.

Wisconsin schools aren't required to teach sex education. But under the new law, which was backed by Planned Parenthood of Wisconsin, schools that do must teach a range of topics, including the benefits of abstinence, the proper use of contraceptives, how to make responsible decisions and the criminal penalties for underage sex. Parents can choose to keep their children out of the classes.

Juneau County lies in the woods and bluffs about halfway between Madison, the state capital, and the Minnesota border. The teen birth rate here in 2008 was nearly 34 per every 1,000 births compared with the state rate of nearly 31 per 1,000, according to a report last year by the county and the University of Wisconsin School of Medicine and Public Health.

The county is about evenly divided between Democrats and Republicans. Voters narrowly backed former President George W. Bush in the 2000 and 2004 elections, but sided with President Barack Obama in 2008.

Southworth says he's not trying to bolster his reputation as a social conservative for a potential run for higher office, his stance has proved popular with anti-abortion groups.
Matt Sande, the legislative director of Pro-Life Wisconsin, which opposes the new law, said every district attorney in Wisconsin should follow Southworth's lead.

"We commend him for his courage, his frankness in exposing the consequences of this irresponsible new law," Sande said. "If I were a district administrator, I would want to know the impact."

And Janet McCauley, a speech teacher in the Mauston district, said she thinks the new sex education guidelines promote "sexual curiosity" and that Southworth's warning was necessary.

"This is dangerous to our young people. I just think the whole bill wasn't thought out enough."

But many parents said they were befuddled by Southworth's warning.

Audrey Jensen, whose 16-year-old daughter, Justina, is a sophomore at Mauston High School, said Southworth is trying to censor what students learn, usurping the role of parents. Children will have sex regardless, she said, and they need all the information they can get.

"I think he's actually a little unrealistic," said Jensen, 47. "Obviously he doesn't remember being a teenager."

Mauston High School quarterback Brady Nelson, 16, said sex education doesn't encourage teens to do it and Southworth should leave health teachers alone.

"It teaches you more about the bad side of it than the good," he said. "You're not going to learn any other way. You can't really charge a teacher for teaching us about the ways of life."

Taake said he intends to teach contraceptive use and isn't worried about being prosecuted.
"It's not just teaching them how birth control works. It's everything else that goes with it," he said. "To arrest me for teaching correct birth control and the student makes the wrong decision and gets pregnant, that's not my decision."

Personal comment: “Southworth, a Republican and a Christian evangelical” is undoubtedly attempting to burnish his conservative credentials with the intention of a career in politics. In the process he is trying to limit the effectiveness of his communities Sex Ed program by throwing up legal straw men and that’s a huge shame. It’s this sort of thinking that has kept the unintended pregnancy rate in the U.S. so high. Religious narrow-mindedness gives organized religion a very bad name.

Wet winter weather: As the storms that have pounded California moved east in the last week, Las Vegas set two daily rainfall records and had a total of 1.7 inches of rain as of noon today, which is more than the 1.59 inches recorded in all of 2009. For readers wondering where I’m going with the Vegas weather report its background to explain how we managed to have a rare opportunity for some of us to take our latex fetish into the mountains west of town to play in the icy water of a small sometime lake fed by rain swollen creeks that are normally dry gullies.
