My cup switch: Several months ago, I switched to a Diva 2 since the manufacturer recommends cup wearers older than 30 or who have given birth would have less likelihood of leakage if she used a Diva 2. I tried a Diva 2 and found I didn’t care for the larger size. My vagina is tighter than most nulliparous 20 y/os and I’ve had no problems with leakage and I don’t need the additional capacity as I can go more than 12 hours w/o emptying on almost all days with the smaller size so I’m back to a size one again. 

The Diva Cup at St Lucy’s: While St Lucy’s girls should already have the three shot regimen of Gardasil inoculations required for maximum protection from HPV viruses that cause cervical cancers as well as some genital warts – they can start the series of injections as early as age 9 - students aren’t fitted for menstrual cups and contraceptive cervical protection until menarche. So this week I’ve been teaching a group of three girls who just reached menarche how to correctly insert a Diva Cup so it doesn’t leak and how to tell by feel and experience when the cup needs emptying by how heavy it feels, and the interval difference between light and heavy days. With just a little practice they became proficient at insertion and removal and now they love their Diva Cups!

Insertion is a several step process. First before preparing any vaginal device for insertion wash your hands. Then it’s a matter of folding the cup for insertion, spreading the labia, pushing it partly in horizontally toward the tailbone then letting it spring open. Once it’s in the ringed bottom of the cup is grasped and rotated one full turn to ensure it is fully open and seals properly. During the full turn it should rotate smoothly an indication that the cup is fully open. Then push it further in until the stem is just inside the vagina where it can be easily grasped for removal.

The Cup should sit low in the vagina away from the cervix. Placed higher it might leak and it could interfere with the strings of an IUD. Once they are confident that the cup fits properly and they can insert it correctly so it doesn’t leak they can do away with their tampons and panty liners and not have to worry about using a Milex diaphragm for flow control that was tricky to empty w/o spilling the contents… unless they want to use the diaphragm for flow control during menstrual sex. The students are so excited to have reached menarche and get their first diaphragms and menstrual cups as now they are biologically adult and their reproductive years have begun.

For removal the ringed bottom of the cup can be gently pulled to break the seal. If that isn’t successful then insert a finger between the vaginal wall and the cup to break the seal and then gently pull it out grasping the ringed portion of the lower dome while sitting on the toilet. Then it should be washed in a mild unscented soap in warm water dried and reinserted. The cup can darken with use and may be boiled for 10 – 15 minutes if it develops an odor.

Forcing Kassi’s period, an update: On Tuesday Oct 7 she took her first 5mg Norethindrone tablet. She will continue for nine more days stopping on Oct 16th when I will be CD27. That should cause her to spot on the 17th and start bleeding (her new CD1) on Saturday the 18th when we all should be CD1.

Kassi and fellatio:  Even though she has had her Gardasil shots there is still a chance of an HPV strain causing throat cancer a few years in the future so unless the men are screened the safest thing to do is to suck a man off while he is wearing a condom. Contraception at St Lucy’s: As returning readers know the faculty and students at St Lucy’s take contraception very seriously. Using an effective method of birth control correctly permits worry free participation in intimate aspects of the curriculum as well as recreational sex. There is of course the problem of STIs some of which have become life threatening in the last 25 years with the possibility of contracting HIV. We minimize the likelihood of students being exposed to STIs by having the males employed as instructors and lab partners for students tested frequently with full STI panels and since it’s possible that students can have sex with men other than their instructor/partners the girls are tested as well. Rather than male condoms Student Health advises students to use the nitrile FC2 condom while being intimate with men who have unknown sexual histories. The FC2s wide anterior opening offers far better protection for the student’s vulvar tissues than if her partner wears a male condom. Emergency contraceptives, Plan B one step (effective for up to 72 hours after a birth control failure) and ella (effective for up to 120 hours after a birth control failure) are available free of charge to students and female faculty. And if all else fails terminations using Mifeprex or by means of a menstrual extraction are paid for by insurance every female at St Lucy’s has through Student Health.

Sorority Tampon troubles: I was talking with Taryn’s friend [I wrote about her in my entry for April 14, 2011] who is a second year student at UNLV. She said the school’s Student Health found that three members of her sorority’s cosplay club had their menstrual supplies, specifically tampons, tampered with. This is of particular interest to me at the moment because today I’m CD3 and bleeding heavily myself. Of course I’m not using tampons I’m using a diaphragm for flow control and I make certain that my menstrual and contraceptive supplies can’t be tampered with. Tampering with something as intimate as a woman’s menstrual protection is pretty creepy.

The problem they had is that the three members have come down with pelvic infections that are traceable back to the fact that they were menstrual and all using tampons, though there were three different kinds being used. It’s not Toxic Shock Syndrome (TSS) which immediately springs to mind when someone mentions an infection traceable to menstrual products, its chlamydia. Chlamydia is probably the most common STI and is known as a "silent" disease because the majority of infected people have no symptoms and it can be spread by oral, anal or vaginal sex. One of the infected women has multiple partners, but is careful to use condoms and says she had only vaginal sex and was symptom free when being routinely tested for STIs when she tested positive. The two other women are in monogamous relationships. One was having pelvic pain and the other had no symptoms, but both went to be tested when their sorority sister unexpectedly found she had chlamydia.

Because both monogamous couples were in a very early stage in their relationships they were just starting hormonal birth control and waiting for the hormones to build up to an effective level so they were still using condoms and their male partners hadn’t been infected. At that point all the sorority sisters were tested and no one else tested positive. The infected women were treated with a single dose of azithromycin and are recovering. Student Health sent the samples to the pathology lab on campus for testing where it was discovered that the samples that tested positive had a tracking agent in the bacteria showing that the strain of bacterium, Chlamydia trachomatis, had been lab cultured by a biology student in the PhD program indicating that the strain had somehow escaped from the laboratory.

Someone in Student Health thought to test the tampons the infected women were using, perhaps because the men in the women’s lives were free of the infection and the bacteria was home grown and they were looking for something that served as a delivery medium. They tested two partial boxes of tampons; the other woman had finished her partial box, and in both boxes they found several tampons with punctures in the individual wrappers and when they tested the tampons they found the chlamydia bacteria with the marker! So then the question became how someone, almost certainly a woman, got access to the women’s menstrual supplies. They are checking the backgrounds of the cleaning crew from a local maid service the sorority uses occasionally, the lab assistants and have questioned the sorority members individually and haven’t turned up anything that they are willing to discuss. But the intentionally spread infection has increased the member’s paranoia and led to all the bedroom doors being fitted with good locks and being kept locked when their occupants aren’t there. Going a step further the members checked all their other menstrual and contraceptive supplies for tampering and found no indication of tampering.

Birth control pill recall: On 09/15/2011 due to a packaging error Qualitest Pharmaceuticals, a manufacturer of generic BCPs, voluntarily recalled certain lots of Cyclafem, Emoquette, Gildess, Orsythia, Previfem and Tri-Previfem. As a result of this packaging error, the daily regimen for these oral contraceptives may be incorrect and could leave women without adequate contraception, and at risk for unintended pregnancy. My Menstrual extraction: I’m cycle day two (CD2) today after starting my period on Friday. Below that the rim of a diaphragm begins to distort due to the increased pressure which may cause it to leak. The 10 meter limitation wouldn’t be a limitation for anyone else around here that has dive-sex in pools because I’m the only one who has a pool deeper than 10 meters. Anyway, I’m looking forward to tomorrow when I screw an Oves back on my cervix and then I’ll be protected at any depth.

Sexual entrapment: There is an old scam that’s new again here. It appeared again as the economy tanked last year as a way for girls to earn a bit of extra money by convincing a rich John that she wants to feel him inside her without the decrease in sensitivity that accompanies even the best condoms. She tells him she is well protected because she is not only on the pill but is also wearing a diaphragm so he has nothing to worry about and God knows when a pretty girl wants her partner to go w/o a condom so she can feel him get off inside her he believes her because even the most skeptical man is blinded by a predisposition to believe he is irresistible! Of course all this is occurring while she is fertile during the few days prior to her releasing an egg.

The thing is that while she really is wearing a diaphragm – she lets him watch her insert it - it is at least 2 sizes (10mm) too small and in this scam she has him take her from behind so that once her lover is inside her she can adjust her hips and cause him to thrust under the diaphragm rim. If he notices, and hardly any man does when that happens, the change is an increase in sensitivity as he is now skin-on-skin caressing her G-spot rather than the soft thin stretchy dome of her diaphragm and no man complains about that! Once under the rim and inside the dome he may or may not give her an orgasm. What she is interested at that point is that he not fall out and have to reinsert where he may or may not slip beneath the dome again. So she works her muscles to get him to shoot his wad under the dome and then has him quickly withdraw. Then while still bent over she inserts a finger and reseals her diaphragm against the front wall of her birth canal trapping his ejaculate inside. If all goes as planned two or three weeks later she will miss her period and will wait another week or so before testing to confirm that she is preggers with her target’s baby. Vaginal sealant: While I was having my ME yesterday I was catching up on Gynecological gossip and female reproductive tract related crimes with my Gyn while she was sucking out my endometrium. I had already heard that we are beginning to find independent party-girls – there have been three that we know of so far - with their vaginas plugged with pink silicone rubber potting compound, the type that is used primarily in military applications. My Gyn was able to fill in some details. What appears to be happening is that a latex condom is rolled over the tip of a potting gun nozzle loaded with the sealant in semi-liquid form. Then the nozzle is inserted all the way up the woman’s birth canal and the trigger pulled releasing the sealant and filling the condom. My Gyn was told by the authorities who were consulting with her about the crimes that the victims were held briefly with their hips in the air to prevent the liquid rubber from running out until it starts to solidify which takes not more than a minute or two. When the potting compound begins to cure heat from the hardening silicone plug melts the latex condom and the plug sears and adheres to the delicate tissues of her vaginal walls. All the girls that have been found plugged with potting compound were wearing ball gags and leather BDSM helmets with the eyes zipped shut so none have any idea who their attackers were. The last thing they recall is drinking from their own water bottles.
