I call them my Alpha and Omega shoes because in them I celebrate the beginning and the end of life. I wear black leather pointes made of traditional materials on those occasions (with fitted slacks over tights) to experience their discomfort. I see it as a masochistic offering of joy to welcome a new life entering the world or in sorrow with the family who lost a member to death.

Copper IUDs, menstrual cups and gushing: A copper ParaGard T380a IUD with a frame is more likely to cause heavier bleeding during menstruation than does a frameless standard 6 bead GyneFix 300. The mini-GyneFix 4 bead 200 causes almost no increase in bleeding during menstruation. Heavier bleeding and the sometimes attendant anemia can necessitate an iron rich diet to stay healthy.

This subject came to mind the other day when I was CD3 and flowing heavily with a Diva cup inserted for flow control. Fortunately Nikolai likes the scent (metallic) and taste (coppery) of my flow as well as going swimming in the ‘Red Sea’ so I have had no decrease in menstrual sex between my former lover, Marvin, and Nikolai.

My menstrual schedule for the Celestial celebrations: I am scheduled to be CD5 and nearing the end of my period on the Autumnal Equinox. For the Winter Solstice I should be CD11 and fertile, which is a very good omen! When my wards were living and training with me in Vegas they were in menstrual synchrony with me, but now their cycles are drifting as they are no longer training with me enough for my pheromones to keep them in synchrony. 

The seasons, celebrated as did the Ancients: I’m already making plans for my wards and I to celebrate the last two significant celestial events of the 2015 calendar year the Autumnal Equinox and the Winter Solstice.  The Autumnal Equinox occurs on September 23^rd which is a Wednesday, and the Winter Solstice occurs on December 22^nd which is a Tuesday. As we have for the past few years we will be celebrating sunrise at Location Z, the ancient holy site in the mountains North East of Ullapool Scotland. Getting permission from my ward’s tutors at their Universities has been no problem as they are known to be favorites of His Grace who continues to be a generous donor.

For the Autumnal Equinox my wards and I will be traveling from London to Inverness aboard ‘The Dragon’, my private rail cars, and staying with the Duke of M**** at his castle, Crag Abbey, in the mountains to the west of Loch Ness on Saturday September 19^th. Then convoy to his Grace’s hunting lodge below the summit of Loc. Z on the 21^st as a staging point for ascending to the celestial temple very early on the morning of the 23^rd Afterward returning to Crag Abbey for a day or two of relaxation before Returning to London on Saturday the 26^th. Nikolai has already been invited as everyone wants to meet the ballet star and my new lover. From the 19^th to the 26^th he will miss four performances of Romeo and Juliet out of a total of eighteen so he was granted leave for those.

For the Winter Solstice my wards and I will again be traveling from London to Inverness aboard ‘The Dragon’ and staying with the Duke of M**** at his castle, Crag Abbey on Saturday the 19^th. Then convoy to his Grace’s hunting lodge below the summit of Loc. Z on the 20^th as a staging point for ascending to the celestial temple very early on the morning of the 22^nd. Afterward we will return to Crag Abbey and remain as guests of His Grace and his son Jack (one of my former lovers) until after 12^th night which in 2016 is Tuesday January 5^th. Nikolai was again invited as my partner. However, since the ballet will be performing The Nutcracker during December and early January he won’t be able to get away as he would miss twelve performances between 12/19/15 and 1/5/16 which the mandarins’ would quite rightly be upset about. So, Marvin, Morning Wood, will be partnering me as he won’t be teaching (at Cambridge) then since Michaelmas term ends on Friday 4 December and Lent term doesn’t begin until Tuesday 12 January. I think that will work out quite nicely as it will give Marvin and me time to reestablish our relationship and let him know he is very precious to me.

Nikolai’s sperm count: Now that I have managed my lover’s small harem so he can be ‘unfaithful’ to me during the work day, a situation I encourage to keep him sexually satisfied until we can be together during evenings and weekends, I conducted some tests of his semen with a home sperm check kit to get an idea of his sperm count and I was pleased to find he is still off the high end. I was curious to see if his sex drive is affecting his ability to father offspring. I’m sure his count has dropped some with him having an average of four acts of intercourse per day, but his enthusiasm for fucking my brains out has not diminished and the volume of semen he releases is quite high, the equivalent of a man who paces himself and amazing for a man who goes after women as though there was no tomorrow as Nikolai’s libido drives him to do.  At his latest physical he masturbated into a specimen cup and the lab did a thorough test. He had a very high sperm count per ml of ejaculate and they were all perfectly formed and strong swimmers. So my lover can be very dangerous around a fertile and unprotected female even at the pace he is setting for himself.

Thrust-drop injury in Freeds: It’s happened already! One of Nikolai’s Odalisques has dislocated a big toe in a thrust-drop injury while wearing Freed Classics. I had warned the girls in his harem as well as others that switching to Gaynor Minden pointes for ballet-sex play was what they should do. However, this woman’s shoe allowance had been spent on Freeds and she didn’t want to be out of pocket £ 86 – £95 for a pair of shoes to seduce Nikolai when she had a fresh supply of Freeds for the fall season. Contraception prep for college

The photo: Ladies, you know the feeling when something in there moves that shouldn’t; a diaphragm, FemCap, menstrual cup or an IUD. The Skyla is a smaller version of the Mirena hormone releasing IUD and was designed for nulliparous women. Skyla releases the progestin levonorgestrel at a rate of 14 mcg/day after 24 days and declines to 5 mcg/day after 3 years at which time it must be replaced.  Menstrual cups can usually be used safely with IUDs as the cup should be worn low in the vagina where it should not come in contact with the IUD strings. Of course in the case of my wards their GyneFix copper bead IUDs were implanted w/o strings so they can wear diaphragms or FemCap with no fear of interacting with the strings and causing an expulsion.

Student’s contraception prep for college: While most teens starting college are on hormonal contraceptives a few use the copper ParaGard IUD and a few still use cervical barriers, a silicone Milex diaphragm or a FemCap. For readers who may not be aware of it the silicone Ortho All-Flex diaphragm that had been available for years by Rx through most pharmacies in the U.S. was discontinued in December of 2013. That leaves only the silicone CooperSurgical Milex- in two rim styles arcing and coil spring – and FemCap as options for women in the U.S who want or need a cervical barrier or the latex Reflexions flat spring which can be ordered from the UK..

This past week we have seen the usual summer increase in teen cervical barrier fittings as they prepare to go away to college for the first time or are having their annual well-woman pelvic exam and diaphragm fit check before returning to sexual activity with college men and some professors.  I fit three 17 y/o patients with 60mm wide seal Milex Omniflex (coil spring) diaphragms. They were the first diaphragms for all three because of health problems after having used only condoms.

One girl has Factor V Leiden a mutation of one of the clotting factors in the blood called factor V. This mutation can increase the chance of developing abnormal blood clots made worse by taking anything with estrogen in it. Factor V Leiden while it can appear in men is primarily found in women. The second, diaphragm patient was a petite gymnast with a full athletic scholarship to a major Midwestern university. She has terrible side effects from the progestin Levonorgestrel in the Skyla IUD that was designed for use by small women and she is too small to have ParaGard inserted. And the third girl does not want to use hormonal birth control of any sort and has a nickel/copper allergy so can’t use a ParaGard IUD.

Their moms came with them and were comfortable that while their daughters ages were below the age of consent for some relationships they were open about sex with boyfriend and the mothers only concern was that their daughters would be well protected by a method under their control. The girls assured me they would wear their diaphragms whenever they were around men as sex can often be spontaneous and the need for effective protection is very important to all of them. The girls had no problem inserting and removing the Milex Omniflex as they all use Diva menstrual cups so were familiar with their pelvic anatomy and folding a device for insertion. They see wearing their diaphragms as a rite of passage and an act of independence accepting the responsibility for managing their own fertility and being able to enjoy the delights of sex with a man while reducing the likelihood of unintended consequences to an acceptable level.

I fit another 17 y/o with a 22mm FemCap as her post-pubic vault was far too shallow to enable her to effectively use even a Reflexions flat spring diaphragm. She and her mom did not want her using anything with hormones in it and they both think IUDs are too invasive so a cervical cap was the only option left for a method over which she will have control. The good news is that for her age she is tall and has a relatively long vagina when not aroused – so a partner is unlikely to hit the removal strap during deep thrusting - and she also has long fingers which allow her to reach her cervix when squatting and pushing down, an ability needed to place a FemCap effectively and equally important to be able to reach it to break the seal for removal.

Elbow hinge arcing spring Vs. latex flat spring: Another patient, a returning UNLV grad student, came in to discuss changing the style of diaphragm she is using. She was last fitted with 70mm latex Reflexions with which she was very pleased. However, she has developed a latex allergy and she wanted something that would provide the same protection and pleasure for her and her very well endowed partner during deep thrusting encounters. What I recommended was a Milex wide seal arching spring which has two elbow hinges that fold for insertion. Once inserted and open when thrust into by a penis the rim flexes only slightly remaining circular and stiff (as long as the device does not rotate significantly in either direction), but not as inflexible as the Reflexions allowing the silicone dome to be stretched across the cervix and glans as it is pushed into her anterior fornix by a thrusting penis.  

There are tradeoffs using a silicone arcing spring instead of a latex flat spring. However, for anterior fornix thrusting I think the two can be equally pleasurable. With the silicone arcing spring the dome is stiffer than the flat spring so the rim flexing is offset by the less stretchy dome which puts the same or perhaps a bit more pressure on the tip of the cervix to push the uterus up and to squeeze the glans simultaneously during maximum anterior fornix thrust depth.  Too, the central dome logo on the Reflexions used as a stimulator for men long enough to reach it is replaced by the extrusion bump in the center of the Milex dome that is easily felt by a thrusting penis when the silicone dome is stretched into the anterior fornix.  The only detracting factors in comparing the Milex to Reflexions is that that Milex costs more and the heat transfer properties are much better with latex. Factors against the latex Reflexions are that couples with latex allergies can’t use it. Latex is more easily destroyed by oily lubes or meds so its useful life is much shorter and latex is more likely to contribute to infections than is silicone. The only major difference in favor of the flat spring rim is that it can be worn effectively by some women with shallow post-pubic vaults. 

The only unknowns with the Milex Arcing spring diaphragm used for deep thrust anterior fornix play is how much the diaphragm will rotate which can affect the pressure deep thrusting will develop and how well the elbow hinges will withstand the reverse pressure when the dome is stretched into the wearer’s anterior fornix by a well endowed partner. The good news is that if the elbow hinges begin to fail a replacement is easily obtained.

Breeding dancers: I was going to title this section Breeding Ballerinas for the alliteration, but while I have helped some of my ballet dancer friends conceive most of the women who come to me for help getting pregnant are statuesque full figured show girls 5’10” or taller so are well over 6” in heels or en pointe; lovely but not with ballet bodies, technique or speed necessary for classical ballet.

My method has been fairly successful and is low cost, fun to use and counter intuitive as a conception aid. I have the friends I’m counseling use a silicone Milex wide seal Omniflex (coil spring rim) breeding diaphragm – one that is two sizes smaller that the size the woman should use for contraception – and have the couple lubricate the dome and vagina with pre-seed, a pH balanced sperm friendly gel, rather than acidic spermicide. This allows a partner to easily under-thrust the rim of the diaphragm especially when the woman is being penetrated from behind while bent over holding on to a barre, chair or table or on her hands and knees.

In that position with the trunk of her body parallel to the floor the uterus is pulled down and back by gravity increasing the depth of the vagina. As the vagina lengthens the rim of the diaphragm in the posterior fornix will be pulled deeper with the cervix which in turn pulls the anterior rim further away from the pubic bone where it can be more easily under-thrust by the smooth upper side of her lover’s glans. Once under the dome the frenulum of an uncut man can freely caress his lover’s G-spot bringing her to orgasm with its strong vaginal contractions that squeeze the thick cream rich with millions of sperm from his vas glands where it spews against her ripe unprotected cervix. Once her breeding partner withdraws the diaphragm reseals against the vaginal walls trapping the ejaculate in the prepared dome along with her fertile cervical mucus.

A few minutes later, the inseminated woman basking in the afterglow lies quietly on her back as her vagina returns to its unaroused state submerging the tip of her ripe cervix in the pool of fertile cervical mucus, pre-seed and semen contained in the dome. So far with the small group of friends who have tried to conceive this way almost 50% have been successful within three cycles, but the group is far too small to be able to make any sort of meaningful claim.

Today is a soft, disposable polyurethane foam sponge containing 1000 mg of the spermicide Nonoxynol 9 (N9) and is slightly less than two inches in diameter. The Today sponge while recommended for only 24 hours of continuous wear was originally approved for a 48 hour continuous wearing interval. However, wearing a sponge continuously makes it more likely that the wearer may become allergic to the N9 spermicide and that the sponge will develop an odor. The concave side fits over the cervix and the manufacture says it is effective protection for an unlimited number of acts of intercourse during the approved wearing interval making it ideal for a teen get-away weekend with a spongeworthy man..

Sponge pregnancies: For women who have given birth the contraceptive sponge failure rate is very high (20%) even when used perfectly so is not a good alternative. However, for a young woman who hasn’t been pregnant and has the willpower to use it consistently and correctly the Today vaginal contraceptive sponge can be an attractive birth control alternative if hormonal contraceptives; diaphragms and caps or a LARC method do not suit in her circumstances.  Walmart currently has a three count box advertised for less than eleven USD or about $3.33 each for 48 continuous hours of protection which is pretty good if you are going on a very spontaneous weekend with a new love interest. Of course it provides no STI protection, but does offer some thrust buffering if the man is large and frequently hits the cervix and it is readily available in most pharmacies w/o an Rx.

Bea has had some St Lucy’s girls to our home for weekend sleepovers with escort trainees for entertainment as an ice breaker for new girls entering the prep school during the spring semester. I was surprised to learn that several of the new girls, all Bea’s age, were using the sponge for protection.  There is no danger of contracting STIs on the sleepovers as the escorts and students have all been screened, but the likelihood of them using the sponge correctly at that age isn’t very good. So I made sure they all took the EC pill ella before they left for school the following Monday.  There is some risk but it was a major bonding experience for everyone involved. And I talked with their sexual health counselor at school to see if the girls can’t be convinced to switch to IUDs.

Proper use means thoroughly wetting the Today Sponge with tap water before insertion, leaving it in place for six hours after the last act of intercourse, and having it in place every time intercourse occurs. The Today contraceptive sponge does not protect against STIs and should not be used during the woman’s menstrual period or for contraceptive protection during underwater sex. However, it is said to be safe to wear during bathing and while swimming.

Vegas vaginas and the contraceptive sponge: We are seeing a spike in sponge pregnancies coming in to our clinic for blood tests and terminations. That seems to be related to holiday (three day) weekends where the users are trying to stretch a single sponge to a continuous wearing interval of three days, 72 hours. Assuming that all other requirements had been met that leaves them in uncharted territory (as far as the sponges efficacy goes) for the last 24 hours and they are far more likely to have an allergic reaction to the continuous N9 released against delicate vaginal tissue. The scent of a sponge left inserted for three days is pretty ripe, but that is just from the interaction of the polyurethane foam with the wearer’s natural secretions and her partner(s) semen a reaction that occurs with diaphragms and cervical caps left inserted for too long as well.

Birth control hormones, copper IUDs and divorce: Our clinic is beginning to see a pattern in some patients who were recently married (in the past three years), who had been on hormonal contraceptives since their teens and who had come off hormones to get pregnant. The pattern is that the women are coming in for a ParaGard copper IUD before conceiving and having the strings removed so their husbands can’t tell they have An IUD inserted.

I’ve spoken to several of them and they have a common story; that when they came of contraceptive hormones each found they had married the wrong man, meaning that he wasn’t masculine enough for her. All are preparing themselves to divorce their husbands and while hormone free are searching for a more masculine and ruggedly compatible man. I’m not saying this is common, but it does fit with a study that was published last year which found that women who met their partners while they were on hormonal contraceptives chose less masculine men than those who were cycling naturally.

Getting Willow kitted out: The morning after the family meeting I had time to take Willow around to make sure she got kitted out properly and introducer to my friends. The main areas I concentrated on were:

Her Gyn visit: We went to the large Women’s Clinic where I fit diaphragms and she had her pelvic exam, full STI panel, was fitted for a Reflexions FS diaphragm, a 70mm, a 22mm FemCap and had a GyneFix copper IUD implanted all taken care of by Chris our male Gyn who goes with us on trips. Chris also fit her for a custom Penetrator plug that my circle and I find useful to wear when around men who want to grope us, keeping their fingers out of our vaginas. Chris is such a good man to know for women as sexually active with multiple partners as my circle and I are. Willow has had all three of her Gardasil HPV vaccine shots so she is good there and has a lovely deep post-pubic vault Ideal for diaphragms and Ben Wa balls and her cervix enters her vagina at just the right angle that FemCap is a perfect fit.

Slit Kit: I got her a Coach cosmetic case at Nordstrom’s to use as a fitted slit kit and we filled it with Japanese super sheer condoms in several sizes, A ten-pack of prefilled 10ml disposable applicators of DiveGel+ (with 4% Octoxynol-9) and a small Diva cup ,her diaphragm, FemCap, four sets of Ben Wa balls: 1.0 inch solid Pyrex and .75, 1.0 and i.25 inch solid stainless steel balls as well as a place to store her Penetrator plug.  

Ballet-boots: After her Gyn visit she was feeling well enough that we went to Gepetto’s to have her measured for the standard four pairs of lightly armored ballet boots with lace splitter heels and spider filament laces to replace the fine quality heavy English leather BBs she brought with her. The English boots would be fine for training, but are unarmored so are a safety hazard if worn in public to fetish clubs here in town.

Pointe shoes: Our Freed rep is in town so after a consultation with the rep and verifying Willow’s measurements against the specs on her special order Classics she trains and performs in. I had Willow place an order for two dozen pairs since with a small troupe like mine she will be in pointes an en pointe much of the time she is performing. Performers feet change over time so it’s a good idea to be sure the old measurements are still good before ordering special order shoes that can’t be returned.

Ballet boot abortions: We are seeing 16 and 17 y/o high school girls coming in for abortions after having sex in ballet boots. These are wannabes for ballet sex whose feet and ankles aren’t strong enough to support them on their toes to be able to wear traditional pointes, even Gaynors while being inseminated. The glutes and pelvic floor muscles aren’t clenched as tight as when balanced over the blocks in pointes, but a wannabe can’t tell the difference and if she’s not sufficiently aroused or lubricated she is tight enough to burst or pull off her trainer’s condom. All the chicks who have come to us for terminations claim to have been impregnated during ballet boot training and said they were on some form of hormonal contraception.

However, they were all taking meds that reduced the effectiveness of their protection or had forgotten to take their pills. Two are on antiseizure meds one was on an antibiotic for a sinus infection and three were on diet pills that speed up metabolism and flush out the protective hormones too quickly and one had forgotten to start a new pack of pills on time. There are a lot of overweight teens who are on weight reduction meds who apparently don’t realize their hormonal birth control is often compromised by their weight loss pills. All had vacuum aspirations and ParaGard copper IUDs inserted. Both of the ballet schools that were offering boot-sex classes paid for the terminations and IUD placements. The school that had the antibiotic and two antiseizure meds failures is still in business as it is part of a larger organization. The smaller ballet school that had the weight loss pills and missed pills pregnancies has, thankfully, gone out of business.  I’m betting the remaining school either closes or continues to have unplanned pregnancies during ballet boot training from poor sexual health protocols. The only good news is that the pregnancies were caught early between six and eight weeks because when they missed their pill periods the girls began testing right away.

Pleaser ballet boots with 7 inch heels: The boot-chick wannabes that take these classes are often overweight and wear poorly fitting boots leading to frequent boot injuries. A medium quality (getting started) pair for ballet-sex is Pleaser’s 7” heels in flat black that can take a good bit of standing in before the toe boxes begin to collapse and they are reasonably priced at about $225 USD or so. (When this was written) But fit is everything so buyers should definitely try them on before buying and get a size smaller than what seems a good fit because like pointes they need to be very tight. In the case of ballet-boots they should be laced very tight across the instep for support to prevent the toes slipping very far into the toe box and the ankles laced tight to minimize the likelihood of sprained or broken ankles from an ankle giving way and falling off pointe.

Very few St Lucy’s girls and none of the local wannabes can afford Gepetto’s ballet boots so good leather English boots are worn by St Lucy’s girls while Pleaser (or Devious, which are cheaper, but not as good) are what the wannabes wear.  Of course with both Pleaser and Devious boots inseminated standard laces are time consuming to clean and restring clean laces in the four sets of grommets after each class, As a result semen stained laces are considered normal on a wannabe’s boots. Perhaps it’s because newbies think being worshiped in their boots that way is a thrill and are flaunting it!  

Boot maintenance: Actually, I’m not sure boot oiling and laces cleaning is taught at schools other than ours because I’ve seen small groups of ballet boot wannabes at Panera where the girls and I go for lunch and the first thing the two groups do is look at each other’s boots. I’ve never seen a pair of commercial leather ballet boots worn by wannabes that have been oiled and many wannabes haven’t brushed the flakes of dried semen out of their laces. Once one girl’s laces were so smelly from recent built up that there was a strong scent of estrogenic sweat and decaying semen. I took her aside and spoke to her about it and she seemed appreciative that I mentioned that sort of sent was only useful in boot clubs when trolling for men where the wearer needed to be seen as a player.  Panera and other local casual restaurants around town are boot friendly locations so fetishists who are confident about our kinks often wear them there as well as to the grocery and pharmacy.

St Lucy’s ballet boot training:  St Lucy’s offers ballet-sex training in ballet boots for advanced ballet students 16 y/o and older, but we are very protective of their sexual health making sure they have had all their Gardasil shots and ensure they aren’t taking meds or supplements that reduce the effectiveness of their hormonal contraceptives. Returning readers will recall that at St Lucy’s the Nuns stress personal responsibility so each student is responsible for her own contraception. As a consequence all male training associates working with students during approved sexual training are very potent and work unprotected to motivate the students to make certain their contraceptive method is effective. Fortunately most sexually active students at St Lucy’s have had copper GyneFix IUDs implanted so they cycle naturally and can enjoy the ebb and flow of their natural hormones.

By the way - A spam update: It took a few months, but Google has done a good job of filtering out almost all spam from reaching my blog. At one time I was getting eighteen or more spam comments a day that appeared as valid comments. Now I may get one a month. The rest go into the spam folder. The spam seems to be coming primarily from Japan (since a great deal of it is written in Japanese) and usually appears during the early AM hours of PST

Naughty’s and sabotage: For women new to the scene the front room of the Meat Market at Naughty Pleasures can be a thrilling but risky adventure. Condom sabotage is a fairly common pastime for some regulars at Naughty’s. We have undercover security women, who try and catch these guys, but they are like cockroaches and we can’t get rid of them all. On-site hookups or take-out relationships for women who rely solely on condoms and don’t use EC as back-up can often result in their partners leaving them with a little something to remember him by. If I’m looking for a quick load not from a Mister Right but a Mr. Right Now I use my own condoms. Not because I’m afraid of conceiving, but to minimize the possibility of being given a STI which can easily be transmitted through pin-holes. The back room at Naughty’s (the STI screened area) where sexually experienced women gather is far safer from the standpoint of exposure to STIs and unintended pregnancy.

Reduced effectiveness: It’s a shame so few women understand their bodies and how to safely use the various methods of contraception available. That goes for men as well if they are into fantasy conception. However, women who are with men into ‘reality breeding’ of women have a different problem if they are insistent on using contraception.

If she fears her partner might sabotage her birth control the woman should be familiar with effective methods that can be used to sabotage her birth control so she can guard against them just as she watches her drink to see she isn’t slipped Rohypnol (flunitrazepam, also known as "roofies"). Here is a summery of some, but not all, methods that can be used to decrease the effectiveness of a woman’s contraception

Hormonal methods: Ideally, if a woman is using her hormonal method correctly her cycle is suspended and ovulation is suppressed so she is safe from pregnancy with an unprotected man. If she misses a pill or two or is late starting a new pack, inserting a new ring, changing a patch or getting her shot late her cycle can start and she will ovulate often w/o her realizing she is in danger unless she is familiar with her fertile signs; a ‘ripe’ high, soft, open cervix discharging a thick very slippery and stretchy cervical mucus and a greater interest in men and sex. If she isn’t impregnated about two weeks after ovulation she will have her period which will probably be off schedule which may be the first time she knows to review her regimen to see why she may have ovulated. If a woman is familiar with her menstrual cycle she can tell when she becomes fertile when she shouldn’t be.

If a woman on hormonal contraceptives takes a drug or herbal product that induces enzymes that metabolize contraceptive hormones the effectiveness of her birth control can be reduced. Some drugs in that category are barbiturates, topiramate (an antiseizure/ migraines and weight loss med), as well as some antibiotics. Ella (the EC pill) will block a woman’s progestin receptors for the rest of her cycle after taking it as EC. Over time these meds can work to reduce the effectiveness of Mirena, Skyla, Implanon/Nexplanon, Ortho Evra (the patch) and NuvaRing (the ring), Depo-Provera (the shot) as well as combined (estrogen/progestin) and mini-pill (progestin-only-pills). St John’s Wort will affect the estrogen in combined pills. There is no hormonal contraceptive that can’t be sabotaged if a man is clever and patient enough

Most of these interactive meds require an Rx, at least in the U.S. so women using them for medical reasons should be aware of their decreasing the effectiveness of hormonal birth control and avoid hormonal methods while on them. A man would need the assistance of a medical practitioner or pharmacist to obtain them for use against an unwilling woman.

NuvaRing: Women using NuvaRing (a vaginal ring) should be careful to destroy an old ring when removed. Otherwise a lover intent on sabotaging her protection can fish the old ring out of the trash and switch it with the new ring she just inserted during foreplay. If a guy wants to try that he should soak the ring he pulled from the trash in hot water for several days to ensure all the hormone is relased from the ring. Care should be taken that the water not be too hot that it melts the weld holding the ends of the ring together or he will have to fish another used NuvaRing out of the trash.

Barrier methods: Women using a barrier as their primary method of birth control are usually cycling naturally with a cycle of between 25 and 35 days. On average a woman’s natural cycle will vary around 28 days. Women cycling naturally usually keep a menstrual calendar so we aren’t surprised by our periods appearing w/o warning for those of us fortunate not to suffer from PMS.

Diaphragms, cervical caps and male and female condoms can be tampered with to lessen their effectiveness. Poor storage, of latex condoms in the glove compartment of a car in the summer or using after the use-by date has passed can be quite risky. Female condoms (the FC2) are made of nitrile and have no temperature storage limitations, just a 3 year shelf life set by the FDA. Latex condoms and latex and silicone cervical barriers can be damaged by using natural oils and petroleum based lubricants (Vaseline) with them.

Most barrier methods can be punctured by a pin or torn accidentally or intentionally by a sharp fingernail. Male latex condoms are especially vulnerable to being punctured though the FC2 and the silicone Semina and latex Reflexions diaphragms are also relatively easily torn. The FemCap dome is so thick it can’t be easily torn by a fingernail, but can be punctured. If the woman has a well fitting diaphragm or FemCap and the dome has been punctured it won’t develop a tight suction which is a tip-off that there is something wrong. Older vintage true cervical caps, which grip the cervix, like the latex Prentif cavity rim and the silicone Oves can also be easily punctured and affected by oily or petroleum based lubes. Oil based or petroleum based lubes can be identified by their scents. A woman concerned that an incorrect lube might be used should provide her own water based lube.

Breeding diaphragms: [condensed from my December 18, 2011 post about breeders] Since there has been an increase in the number of men asking for breeding encounters with our escorts I’ve started training the girls to use a ‘breeding diaphragm’. For rape fantasies I take a different approach when providing contraceptive protection that ‘accidentally’ fails. Rather than sabotage my partner’s condoms which they hate wearing anyway I wear what I call a breeding diaphragm. A breeding diaphragm is one that is two sizes smaller than the size the woman should wear for it to be effectively used for contraception. And it is worn ‘dry’, that is w/o spermicide in the dome. I’ve found the type diaphragms that work best as breeders are the Ortho All-Flex (arcing spring) or a Milex Omniflex (coil spring) rim styles.

When a woman has a diaphragm inserted that is considerably smaller than her correct size it is much easier for her partner/fantasist to under-thrust the rim with his penis and plant his seed under or in the dome so that when he withdraws the rim will reseal on the anterior wall of her vagina trapping almost all of his semen inside the dome where the tip of her cervix will be submerged in a pool of semen while she sleeps. Or if the couple is into bondage and the woman is tied down and to be gagged during the encounter an additional advantage of using a diaphragm during a rape/breeding encounter is that it can be pulled out by her attacker and shoved in her mouth as a gag.

Ideally an Ortho All-Flex is the best breeder because the rim is the easiest to under-thrust. However, the smallest size All-Flex is a 65mm and some adult women might take a 60mm in a breeder so they would need an Omniflex which makes a 60mm in a coil spring rim.

Someone unfamiliar with cervical barriers might ask why not use a cervical cap, such as FemCap for breeding? The answer is that while an improperly sized FemCap can be displaced off the cervix with a thrusting penis it may fill with semen, but it is very unlikely to reseat itself on the cervix after withdrawal. And a diaphragm rather than a FemCap is far safer for the woman since a FemCap if pulled out and used as a gag can easily be lodged in the woman’s throat and her partner, with his mind elsewhere, may think she is squirming in ecstasy when in fact she is being asphyxiated, especially if she has been penetrated from the rear, taken doggie style, where he can’t see her face.

Men wanting to surprise a partner can order a diaphragm on-line that is two sizes smaller than her correct size and ask to insert it himself as part of foreplay. If she checks after him to make certain her cervix is covered an alert woman can tell he has substituted a smaller one, but if her mind is elsewhere he can often get away with the substitution. If she doesn’t check that the diaphragm immediately after she lets him insert it and he is gentle while under-thrusting the rim the next time she might notice something is wrong is when she has very little coitial discharge or has trouble reaching the smaller device to remove it. It’s best for a man to have her accustomed to him inserting her regular size diaphragm so she is less likely to check when she is fertile and he substitutes a breeder in its place.

A note to my women readers: ALWAYS check to see that your cervical barrier is properly placed and developing the proper vacuum in the dome before any penile penetration.

The copper IUD: The Nova-T and Flexi-T in Canada, the ParaGard in the U.S and GyneFix implant (which we are using on a clinical trial basis) are all copper IUDs. A copper IUD is immune to being tampered with by being given something that would decrease its effectiveness unknown to the wearer. My clinic prefers the three year GyneFix implant placed w/o strings so a cervical barrier can be worn with no danger of the barrier pulling out the IUD upon removal after using it as flood insurance during dive-sex.

The only possibilities for decreasing the effectiveness of a copper IUD are to cause the wearer such strong uterine cramps they might displace the IUD or to tug on the strings and pull the IUD out. The latter is not usually painful and if done during menstrual sex the woman may not feel a thing and only notice when she intentionally feels for the strings. Women with new male partners should always check the strings of their IUDs before and after an encounter to be certain it is still where it should be.

Super Bowl XLVII: I was very pleased that the Raven’s won 34 to 31 despite having to recover from the momentum sapping 34 minute power outage. Menstrual sex

Primolut-n, generic Aygestin (norethisterone acetate) 5-mg tablets for inducing periods.

Menstrual sex, inducing a period: Escorts specializing in menstrual sex can safely induce their periods every 3 weeks, that way they can have as many as 17 bleeds a year. The way this is done is to take a 5 day regimen of the progestin Norethindrone (nor eth in' drone) acetate (NEA) - either Aygestin or its generic Primolut-n - taking two 5 mg tablets per day. After stopping the drop in progestin level will induce the woman’s period. Patient information for Aygestin can be found HERE. The picture at the top of this post shows blister packs of the generic Primolut-n, which we dispense at the clinic. A single 10 tablet blister pack is sufficient for a 5 day regimen of 2 5mg pills a day. Another off-label use for 5mg Norethindrone acetate pills is to delay a woman’s period if she is on her honeymoon, a diving or hiking trip or on a photo shoot.

Norethindrone acetate side effects: The down side of inducing periods for menstrual sex is that the hormonal swings can be hard on the woman’s body and the loss of blood will cause anemia unless she takes iron supplements and eats an iron rich diet. Another problem is that Norethindrone acetate can contribute to fluid retention so diuretic pills are often required for the woman to retain her figure.

Escorts specializing in menstrual sex have copper IUDs inserted for two reasons; first because inducing will not necessarily prevent ovulation so unlike sex during a woman’s normal period it is frequently possible to conceive from induced menstrual sex. The second is that a copper IUD will often cause heavier menstrual flow thus giving her clients a more complete experience covering them with her blood. Both ParaGard and GyneFix IUDs are satisfactory; however our Gyns and escort clients prefer GyneFix because it is far less likely to be expelled during strong uterine contractions.

Norethindrone (a less potent form of Norethindrone acetate) is also used in much lower doses as a progestin-only-pill (POP) birth control pill for new mothers who are lactating since progestin will not affect milk production. Some POPs containing 0.35mg norethindrone (NE) are: Ortho Micronor, Nor-QD, Camila, Errin, Jolivette and Nora-BE.

Recent clinic patients:

An overview: Returning readers will recall that several of my primary interests are in cervical barriers and physically risky sex, excluding STIs of course, which I’m very careful to avoid. Also, the clinic at which I volunteer is known for it’s expertise in reproductive health and especially the now (in my opinion) underused contraceptive method of cervical barriers. Which explains why we have so many patients who need counseling, fitting and training in the correct use of cervical barriers. So while it might seem from my posts that every woman in Vegas is now wearing a diaphragm that’s not the case at all. It’s just that a very large percent of the ones who are or want to come through our fitting rooms.

In the last several years diaphragms and caps have begun to regain importance as effective protection as social mores change so that it’s no longer just escorts specializing in underwater sex who are the most enthusiastic users of diaphragms and cervical caps. Now even young vanilla women (especially college girls) who become more sexually adventuresome are finding they need effective upper reproductive tract protection, not for contraception, but to protect their fertility as they participate in extreme sexual adventures. Latex-grab: There have been several instances of pregnancies due to latex-grab for couples using both male condoms and latex flat spring diaphragms. From the patients our clinic is seeing this seems to be happening with younger UNLV girls being taken w/o sufficient lubrication. There have been three pregnancies that I know of. One from the diaphragm being pulled off the student’s cervix and the condom pulled off her partner. Another was from the diaphragm being pulled off the cervix and the condom tearing from friction. And the third was from friction wearing holes in both the dome and condom. This last is troubling because it means that the woman wasn’t inspecting her diaphragm for thin spots, cracks or tears in the latex membrane before insertion. The thing these three cases have in common (in addition to latex-grab) is that the women were on hormonal birth control but weren’t using it correctly. They had been fitted for diaphragms as a prerequisite for dive-sex training (not in my course) and were with new partners which accounts for them insisting on their partners wearing condoms. Perhaps they thought that when using three effective forms of protection nothing could go wrong and as a consequence used none of the methods correctly. Depo Provera and pregnancy: In spite of the possibility of serious side effects the popularity of Depo-subQ Provera 104, medroxyprogesterone acetate (DMPA), has increased since March of this year when women in the U.S. began reading the Fifty Shades trilogy, where the heroine, Anastasia Steele, was put on the contraceptive injection. While Depo is very effective it’s not foolproof and users must get an injection every 12 weeks. The user is considered safe for otherwise unprotected sex 7 days after the initial injection. The perfect use failure rate is .3% while the typical use failure rate is 3% because the user was either late getting her next scheduled injection or ignored the prohibition on unprotected sex for 7 days after receiving her first shot. The clinic is seeing pregnancies in teens on Depo primarily from those who have had what they say were “just this once” unprotected encounters with partners during the 7 day interval immediately after their injection while the progestin is suppressing ovulation. But we have also had 2 in girls who were late getting their next shot. Typically they want the pregnancies terminated by vacuum aspiration which is quick and very safe, and we can do that the same day if they can get the needed releases signed

Mellon's Next Move: Updated Tuesday November 15, 8.57am: Is Tamara Mellon set to become the UK's answer to Tom Ford? The Jimmy Choo founder, who surprised the brand's new owners Labelux when she resigned this weekend, is planning to launch her own lifestyle brand - like Tory Burch or Tom Ford - when she leaves Jimmy Choo later this month. Lara’s vacuum aspiration: In an earlier post, ‘Lara Croft and extended cycle BCPs’ (Nov. 14, 2011) I wrote about a UNLV grad student I called Lara Croft who was impregnated during an encounter at her sorority’s charity bordello over the Halloween weekend. The UNLV pregnancies: were terminated with Mifeprex, but one required a menstrual extraction (vacuum aspiration) when she didn’t pass the entire placenta. I think the stalker will probably just ‘disappear’. We are working toward that end. With Adolph he is fond of European girls who are in the country illegally so no one knows or cares who they are or questions when they move on. You would be surprised how many of them there are.

Crush training: An old friend and frequent reader asked for more details about the crush training course I conduct occasionally. The classes are infrequent because there are so few women who are willing to commit the time and effort to learn the technique. Even with highly motivated students there is about a 50% drop-out rate. Women who are only into vanilla sex find it difficult to believe that the pelvic floor muscles can be developed to the extent that they can crush a vaginal penetrating penis, but it’s true. Think of the strength of the muscle contractions of women with vaginismus. We use those muscles and develop the ability to control when the muscles contract and by how much. If a woman can master that, and it takes months of intensive control and strength training to accomplish, she has a very good chance of destroying the erectile tissues of a penile shaft .penetrating her vagina. When I have the time I’ll try to write up something about crush training in a bit more detail.

AST and Penetrator plugs: All our AST students at St Lucy’s have been fitted for Penetrator plugs to be worn in the classroom and when not involved in encounters requiring vaginal penetration. The shaft of the Penetrator will allow the student to clench her vaginal muscles gripping the plug to strengthen the bands of muscles surrounding her birth canal. All the students have been training with Ben Wa balls for at least a year so this is a step up in training. Wearing a Penetrator requires that the woman’s pubes be totally nude so we recommend Brazilian waxing (Body Buffers has a salon on campus) but a few students initially prefer to shave or use a depilatory cream. Those students almost always decide waxing is better and simpler before the first semester is over. The plugs anti-expulsion ridge that fits up behind the woman’s pubic bone prevents clenching of her vaginal muscles from expelling the plug so she can really bear down while clenching.

All Penetrator plugs are custom made to the exact dimensions of the woman’s pelvic anatomy. The thickness of her pubic bone, the depth of her introitus, the depth and angle of her vagina when not aroused, the width of her perineal canal, the width and length of her vulva and the size of her clitoris that will be under the grope shield. The Plug is a precision fit so DiveGel+ is used as an intimate lubricant and sealant so that once she becomes used to it the woman can wear her plug comfortably all day if necessary. There is a urine vent in the convex head so it, unlike the earlier versions the plug, doesn’t have to be removed to pee. Once properly inserted the plug creates a vacuum in the vagina that requires being broken with the perineal removal tab, by pulling the tab down and forward, to break the vacuum before the plug can be pulled out.

The semesters first IUD expulsion: Last week we had a ballet student in my Contemporary Sexual Health course expel her ParaGard IUD during a lab exercise when her partner gave her a rare (for a student that young) cervical orgasm that caused her uterus to cramp so severely it bent the arms of her ParaGard and then expelled it. She was wearing a ball-gag to muffle her screams and afterward she said she rather enjoyed the pain. She was examined by our Gyn and her uterine wall wasn’t perforated, there wasn’t even any bleeding to speak of so she will be fine. In the meantime she will be using FC2s for protection. But it isn’t the IUD expulsion she is talking about it’s her cervical orgasm where her partner was thrusting into her posterior fornix. It’s really quite rare for a 16 y/o to have a cervical orgasm and she wants more.

The use of escorts during AST: For my Advanced Sexual techniques course at St Lucy’s we use both male and female escorts for lecture demonstrations. Females are used to demonstrate proper positions and pelvic muscle training and are selected for the depth and well developed muscle tone of their vagina’s as well as their beauty, personality and sexual experience. Male escorts are carefully selected and are chosen for the size of their genitalia and their skill in using it as well as their looks, personality and sexual experience. We have three times the number of men as women escorts because of the need to have a male lab partner (trainer) for each student. For the select few men and women chosen the STI screening is very thorough and frequent to ensure the trainer/demonstrator’s are sexually healthy at all times.

Male trainer rotation: To minimize the likelihood of students becoming emotionally involved with their male trainers we rotate lab partners. Rotation also gives the individual student experience with a wide variety of men, though are all well above average as far as equipment and skill are concerned. Given the emotional immaturity of some students occasionally our best efforts are circumvented and a relationship is established. However, if that should occur the man is eliminated from the training rotation and its associated perks. If that isn’t sufficient he is blacklisted from working in any capacity in Vegas. Male escorts selected as AST partner/trainers understand what a sweet arrangement they have that permits increasing their training experience with a variety of students at the age of consent and older while being paid for their work so they discourage emotional relationships with their training partners.

Vaginal biocide: To minimize the likelihood that having so many different partners will contribute to vaginal bacteria overgrowth problems such as BV, or yeast we all use DiveGel+ (that has a biocide in it) as an intimate lube so cases of bacterial overgrowth are very rare for students who routinely lube up with DiveGel+. Silicone based DiveGel is ideal as a base for application of biocides and spermicide and is safe for use with the latex Reflexions flat spring diaphragm with which all AST students are fitted and which is worn by our professional escorts for upper reproductive tract protection during dive-sex and to protect from under-thrusting when worn as back-up during a hormonal birth control failure.

AST and pregnancy: All AST students are tested for pregnancy weekly in the presence of a Student Health nurse so there is no possibility of a student using a foreign urine sample brought in to spoof the test. Most have had GyneFix IUDs implanted or one of the framed IUDs such as the copper ParaGard or the progestin Mirena, but even with those there is a very slight chance of pregnancy and with Mirena ectopic pregnancies make up a significant percentage of the ones that do occur. So since the AST students will all be taking dive-sex instruction during which wearing a gas guard is mandatory the AST girls are encouraged to always wear their Reflexions diaphragms when with their lab partner/trainers so upper reproductive tract protection will become a habit and a natural part of a students training regimen. Both Plan B one step and ella are available at all times for St Lucy’s students and employees who think they may have had a birth control failure. And should a pregnancy occur the student has the option of having it terminated (paid for by Student Health) or resigning. In the years I’ve been at St Lucy’s we have had no students who have resigned due to pregnancy.
